The neuropsychiatric systemic lupus erythematosus (NPSLE) has higher disability and mortality rates, which is one of the main causes of death in systemic lupus erythematosus (SLE) patients. Magnetic resonance spectroscopy (MRS) can detect the changes of metabolites in different intracranial areas in vivo in patients with SLE, so as to provide evidence for the early diagnosis of NPSLE. Different from the conventional single-voxel MRS, which can only screen one brain region with one metabolic change, we simultaneously detect 13 kinds of intracranial metabolic changes in nine brain regions by multivoxel proton MRS (MVS). We use a recursive feature elimination algorithm to select the most related metabolites for better identifying NPSLE. To accurately diagnosis NPSLE by these intracranial metabolites, we train a support vector machine deep stacked network (SVM-DSN) for quantitative analysis of these metabolites. Comparing with the conventional statistic method, which is about 70% of accuracy, the proposed model achieves 97.5% of accuracy for NPSLE diagnosis. We conclude the trained SVM-DSN can effectively analyze the metabolites obtained by multivoxel proton MRS for NPSLE diagnosis, which may help to early diagnosis and intervention of NPSLE, and alleviate the bias of manual screening.

ABSTRAKPendahuluan: Systemic lupus erythematosus (SLE) merupakan inflamasi kronis yang dapat melibatkan sistem saraf, membran mukosa, dan organ lain dalam tubuh. Avascular bone necrosis (AVN) merupakan gejala yang muncul pada penderita SLE. Maksilektomi dilakukan pada tulang maksila yang mengalami AVN. Penutupan celah pasca maksilektomi tersebut dilakukan dengan cara  menggunakan protesa maksilofasial intraoral yaitu obturator. Tujuan laporan kasus ini mengkaji rehabilitasi prostetik menggunakan protesa obturator definitif resin akrilik pada penderita SLE pasca maksilektomi. Laporan kasus: Seorang wanita berusia 21 tahun datang ke Poli Gigi dan Mulut RSUP Dr. Sardjito dengan keluhan bau mulut, hilangnya gusi pada langit-langit, dan kegoyahan gigi rahang atas. Pasien didiagnosis SLE sejak lebih dari 1 tahun yang lalu. Pada pemeriksaan intraoral, selain lesi pada mukosa palatum, ditemukan juga nekrosis pada tulang palatum, kehilangan gigi 14, 15, 16, dan 25, serta kegoyahan derajat 3 pada seluruh gigi rahang atas yang tersisa. Pasien dirawat dengan obat Myfortic (2 x 180 mg/hari) dan Fluconazole (1x150 mg/hari) kemudian dirujuk ke Poli Bedah Mulut untuk dilakukan maksilektomi, dilanjutkan dengan pembuatan protesa obturator oleh tim prostodonti. Pasien dibuatkan obturator pasca bedah untuk menutup celah palatum pasca maksilektomi. Pencetakan menggunakan bahan hydrocolloid irreversible sebelum operasi untuk pembuatan obturator pasca bedah. Insersi obturator menunjukkan celah palatum tertutup rapat oleh plat akrilik. Retensi didapatkan menggunakan kawat stainless pada titanium wire mesh pengganti tulang maksila. Tidak ada keluhan saat kontrol, penelanan baik. Tiga bulan pasca pemakaian obturator pasca bedah dilakukan pemasangan obturator definitif resin akrilik rahang atas. Pemeriksaan klinis menunjukkan suara sengau berkurang, estetis, dan pengunyahan baik. Simpulan: Protesa obturator definitif resin akrilik pada pasien SLE pasca maksilektomi dapat mengembalikan fungsi estetik, mengurangi suara sengau (mengembalikan fungsi bicara), mengembalikan fungsi penelanan, dan pengunyahan.

The review presents the data available in the literature on the use of the lupus band test (LBT) for systemic lupus erythematosus (SLE). LBT is a direct immunofluorescence method used to detect immunoglobulins and complement factors in the dermoepidermal junction of skin biopsy specimens. LBT may be applied as one of the diagnostic tests for early diagnosis of SLE in patients without skin manifestations and in those of incomplete SLE. Like the results of other laboratory tests, those of LBT may be taken into account when establishing a final diagnosis only in conjunction with other clinical, immunological and instrumental data.

Objectives: This study was to evaluate the incidence and ocular manifestation in systemic lupus erythematosus (SLE) patients. Methods: A total of 50 cases with age group 15 to 50 years were enrolled. A detail history, clinical examinations and relevant investigations were performed to all cases. Patients who were diagnosed with systemic lupus erythamatusus (SLE) by using American Rheumatologic criteria with or without ocular features were included in this study. Results: Data was analysed by using simple statistical methods with the help of MS-office software. All data was tabulated and percentage was calculated. Conclusions: Females were commonly suffered with systemic lupus erythamatusus (SLE) and it was commonly seen in age 15-25 years. Episcleritis was the most common symptoms in SLE. Second most common symptoms were conjunctivitis and scleritis. Right eye was more affected than left eye. Most of the cases had ANA positive. Hence, ocular manifestation is the most common in SLE patients. Early diagnosis and prompt treatment may give light of hope for SLE patients. And more research is needed in order to determine which therapy will provide the best prevention and management in SLE patients. We described a 35 years old female, who developed Chronic inflammatory demyelinating polyneuropathy as neurologic commitment during the early diagnosis in Systemic Lupus Erithematosus (SLE). While the neuropsychiatric commitment has a variable prevalence in SLE, the association that we describe is infrequent and it has important concerns during its treatment.
