AimsTo report treatment outcomes of slow-coagulation continuous-wave transscleral cyclophotocoagulation (TSCPC) as an initial surgical intervention in patients with neovascular glaucoma (NVG).MethodsA retrospective study including 53 patients (mean age of 69.6±16.6 years and mean follow-up of 12.7±8.9 months) with a diagnosis of NVG and no previous incisional glaucoma or cyclophotocoagulation surgeries. All patients underwent slow-coagulation continuous-wave TSCPC (1250-milliwatt power and 4-second duration).Primary outcome measure was surgical success defined as an intraocular pressure (IOP) from 6 to 21 mm Hg with a reduction ≥20% from baseline, no reoperation for glaucoma and no loss of light perception vision. Secondary outcome measures include IOP, glaucoma medications, visual acuity (VA) and complications.ResultsIOP decreased from 40.7±8.6 mm Hg preoperatively to 18.4±12.2 mm Hg postoperatively (p<0.001). The preoperative number of glaucoma medications dropped from 3.3±1.1 at baseline to 2.0±1.5 at the last postoperative visit (p<0.001). The cumulative probabilities of success at 12 and 24 months were 71.7% and 64.2 %, respectively. Mean logarithm of the minimum angle of resolution VA was relatively unchanged from 2.27±0.63 to 2.25±0.66 at the last follow-up visit (p=0.618). The most common observed complications were decrease in baseline VA (13.2%) and anterior chamber inflammation (9.4%).ConclusionsSlow-coagulation TSCPC is an effective and relatively safe initial surgical intervention in medically uncontrolled NVG.

The study included 48vitreoretinal surgeries performed during 2014-2015 in 39patients with advanced fibrovascular stage of proliferative diabetic retinopathy. Visual acuity before the surgery averaged from pr. l. certae to 0.01. In 7cases patients underwent intravitreal injection of Lucentis 2–3weeks before the surgery. The surgery was performed according to the standard 3-port 25G vitrectomy. In some cases, short-term tamponade of vitreous cavity by fluid perfluororganic compounds (PFOS) was performed for 2–3days. This was due to prolonged bleeding during the surgery. In these cases, fibrinolytic of direct action (hemasa) was injected directly to the blood clot. In 4 cases circular retinotomy was needed. In 35cases tamponade was made by silicone oil 5700cSt, in 13 – by air-gas mixture. In cases of air-gas tamponade 3 patients had a relapse. After vitreous cavity revision it was plugged with silicone oil 5700cSt. In 17cases visual function remained unchanged, in 18 cases the changes were imperceptible, in 9cases visual acuity improved to 0.2, in 3cases – visual function increased to 0.4 and in 1patient – to 0.7. In postoperative period IOP increased in 7 cases. In 5 of these cases compensation was achieved with antihypertensive drugs. In one case the patient was implanted leukosapphire drainage, and in one more case transscleral cyclophotocoagulation about terminal aching glaucoma was performed. The silicone oil was removed in 6patients at the period of 6 months up to 2years.

Aim: To report a case of recurrent Ahmed glaucoma valve (AGV) tube exposure dueto scleral patch graft (SPG) melt Background: AGV is a useful modality in the management of medically refractory glaucoma. Human donor SPG has been employed to cover the external portion of the tube to prevent its exposure. Case Description : A 23 year old male with Anterior chamber intraocular lens (ACIOL)had undergone retinal detachment (RD) surgery with silicone oil insertion (SOI)followed by SO removal (SOR), re- RD and SO re-injection, followed by implantation of Ahmed glaucoma valve (AGV) for uncontrolled glaucoma in the right eye. Three months after the AGV implantation, he presented with exposure of the subconjuctival portion of AGV tube with melted, displaced and shrunk donor sclera graft, which was managed by placing larger sized, full thickness, donor scleral patch graft (SPG) over the exposed tube. Six months later, he presented again with tube exposure and the donor SPG was found to be retracted nasally and shrunk to one fourth of its original size. The AC portion of the tube was blocked by the SO bubble and the emulsified SO could be seen inside the exposed tube. The patient underwent SOR with tube explanation and intraocular pressure was later controlled with transscleral cyclophotocoagulation and medical management. Conclusion : SPG thinning and the overlying conjunctival erosion can occur after the AGV implantation. This can cause AGV tube exposure and hence, long - term patient follow up is essential to ensure timely identification of this possible complication and prompt treatment.

SummaryThe uptake and binding of antimitotic alkaloid colchicine has been demonstrated in washed preparations of human platelets. A silicone oil technique was adapted so that both uptake and binding of 14C-colchicine were examined in the same platelet preparations. The time dependence and amount of colchicine taken up and bound by different platelet preparations during a 90 to 120 min incubation period were highly reproducible. Both colchicine uptake and binding by intact platelets, and colchicine binding by preparations of lysed platelets were specific and temperature dependent. Colchicine uptake was slowly reversible. Magnesium and GTP enhanced colchicine binding by lysed platelet preparations but calcium decreased binding.Exposure of platelets to either cold (4° C) or to thrombin, which disrupt platelet microtubules, produced significant increases in colchicine uptake and binding. The thrombin effect was maximal at 37° C and resulted in a greater increase in uptake and binding than that produced by either cold treatment alone or, by cold treatment followed by incubation with thrombin at 37° C. The amount of increase in uptake and binding produced by thrombin was independent of both thrombin (1–5 Units/109 platelets) and colchicine concentrations (1–50 × 10−6M).It is postulated that thrombin may initiate the formation, or make available, colchicine binding sites (microtubule subunits) within platelets.

ABSTRACTImplants are essential for the repair of retinal detachments. The implant buckles the wall of the eye and apposes the detached retina with the choroid, thus restoring light sensitivity to the retina. The scleral buckling also relieves traction on the retina from a shrinking vitreous body. The implant materials most commonly used are solid silicone rubber and silicone sponges, but both types have some disadvantages. A poly(hydroxyethyl acrylate-co-methyl acrylate) hydrogel implant with improved properties of softness and antibiotic absorption is also available for retinal detachment surgery. Proliferative vitreoretinopathy involves various conditions of retinal detachment complicated by vitreous fibrosis, which, after vitrectomy, may be treated with intraocular injection of fluids that support the retina against the choroid. For conditions requiring a long-term implant, silicone oil although controversial is the material of choice. Many other substances have been investigated but none better has yet been found.

Lubricants and leather dressings are the most common treatments of dry and water logged historical leathers. Color change has a great importance during the time and treatment process, due to visual and aesthetic values of historic leather relics. Polyethylene glycol (PEG) and silicone oil (SiO) are frequently used leather dressings in the conservation procedures. Therefore, color stability of treated leathers with PEG and SiO were investigated before and after heat accelerated aging. Moreover, application of ascorbic acid was evaluated as an antioxidant additive for PEG (PEG+AA).Color change after treatment and aging were studied by colorimetry technique in the CIE *L*a*b system. Results indicated to severe color alteration in PEG treated and aged leathers with or without ascorbic acid. Whereas, SiO treated samples showed better stability and minimum color shift after aging. Silicone oil was characterized as the best dressing for historical leathers with compared to PEG and PEG+AA, due to its high stability and aesthetical properties.
