SummaryThis narrative review summarizes current available information about cardiac arrhythmias (QT prolongation, Torsade de pointes Tachycardia [TdP], sudden cardiac death) associated with psychiatric medication. Among the most commonly used antipsychotics, amisulpride and ziprasidone are most frequently associated with TdP. Treatment with some antidepressants (SSRIs, tricyclic antidepressants) is associated with a 5- to 6-fold increase in the incidence of out-of-hospital cardiac arrest. Lithium is associated with bradycardia, T-wave changes and AV-block; anxiolytics of the benzodiazepine group do usually not have cardiac side effects. The combination of multiple drugs (including medications from general medicine) that prolong the QT interval has a particularly high cardiac risk.

Abstract:Excitable media, such as nerve, heart and the Belousov-Zhabo- tinsky reaction, exhibit a large excursion from equilibrium in response to a small but finite perturbation. Assuming a one-dimensional ring geometry of sufficient length, excitable media support a periodic wave of circulation. As in the periodic stimulation of oscillations in ordinary differential equations, the effects of periodic stimuli of the periodically circulating wave can be described by a one-dimensional Poincaré map. Depending on the period and intensity of the stimulus as well as its initial phase, either entrainment or termination of the original circulating wave is observed. These phenomena are directly related to clinical observations concerning periodic stimulation of a class of cardiac arrhythmias caused by reentrant wave propagation in the human heart.

This paper reports on two research results: ( 1) designing an English for Specific Purposes (esp) corpus architecture complete with annotations structured by regular expressions; and ( 2) a case study to test the design to cater for creating a specific vocabulary list using the compiled corpus. The first half of this study involved designing a precisely structured esp corpus from 190 veterinary medical charts with a hierarchy of the data. The data hierarchy in the corpus consists of document types, outline elements and inline elements, such as species and breed. Perl scripts extracted the data attached to veterinary-specific categories, and the extraction led to creating wordlists. The second part of the research tested the corpus mode, creating a list of commonly observed lexical items in veterinary medicine. The coverage rate of the wordlists by General Service List (gsl) and Academic Word List (awl) was tested, with the result that 66.4 percent of all lexical items appeared in gsl and awl, whereas 33.7 percent appeared in none of those lists. The corpus compilation procedures as well as the annotation scheme introduced in this study enable the compilation of specific corpora with explicit annotations, allowing teachers to have access to data required for creating esp classroom materials.

Objectives:: The present study was designed to investigate the effects of renin angiotensin system (RAS) blockade on cardiac arrhythmias and sympathetic nerve remodelling in heart tissues of type 2 diabetic rats. Methods:: Thirty-two male Sprague Dawley rats were randomly allocated into 4 equal groups; a) normal control group: normal rats, b) DM group; after type 2 diabetes induction, rats received 2ml oral saline daily for 4 weeks, c) DM+ ACEi: after type 2 diabetes induction, rats were treated with enalapril (10 mg/kg, orally for 4 weeks) and d) DM+ ARBs: after type 2 diabetes induction, rats were treated with losartan (30 mg/kg, orally for 4 weeks). Results:: In type 2 diabetic rats, the results demonstrated significant prolongation in Q-T interval and elevation of blood sugar, HOMA-IR index, TC, TGs, LDL, serum CK-MB, myocardial damage, myocardial MDA, myocardial norepinephrine and tyrosine hydroxylase (TH) density with significant reduction in serum HDL, serum insulin and myocardial GSH and CAT. On the other hand, blockade of RAS at the level of either ACE by enalapril or angiotensin (Ag) receptors by losartan resulted in significant improvement in ECG parameters (Q-T), cardiac enzymes (CK-MB), cardiac morphology, myocardial oxidative stress (low MDA, high CAT and GSH) and myocardial TH density. Conclusions:: RAS plays a role in the cardiac sympathetic nerve sprouting and cardiac arrhythmias induced by type 2 DM and its blockade might have a cardioprotective effect via attenuation of sympathetic nerve fibres remodelling, myocardial norepinephrine contents and oxidative stress.
