Relationship between blood coagulability and sense of burden among caregivers of patients with dementia

SummaryThe authors previously reported a transient decrease in adhesive platelet count and an enhancement of blood coagulability after administration of a small amount of adrenaline (0.1-1 µg per Kg, i. v.) in man and rabbit. In such circumstances, the sensitivity of platelets to aggregation induced by ADP was studied by an optical density method. Five minutes after i. v. injection of 1 µg per Kg of adrenaline in 10 rabbits, intensity of platelet aggregation increased to 115.1 ± 4.9% (mean ± S. E.) by 10∼5 molar, 121.8 ± 7.8% by 3 × 10-6 molar and 129.4 ± 12.8% of the value before the injection by 10”6 molar ADP. The difference was statistically significant (P<0.01-0.05). The above change was not observed in each group of rabbits injected with saline, 1 µg per Kg of 1-noradrenaline or 0.1 and 10 µg per Kg of adrenaline. Also, it was prevented by oral administration of 10 mg per Kg of phenoxybenzamine or propranolol or aspirin or pyridinolcarbamate 3 hours before the challenge. On the other hand, the enhancement of ADP-induced platelet aggregation was not observed in vitro, when 10-5 or 3 × 10-6 molar and 129.4 ± 12.8% of the value before 10∼6 molar ADP was added to citrated platelet rich plasma (CPRP) of rabbit after incubation at 37°C for 30 second with 0.01, 0.1, 1, 10 or 100 µg per ml of adrenaline or noradrenaline. These results suggest an important interaction between endothelial surface and platelets in connection with the enhancement of ADP-induced platelet aggregation by adrenaline in vivo.

SummaryIn 382 patients, aged 21 to 76 years, including 39 healthy volunteers, a platelet count by Olef’s method, an adhesive platelet count by Moolten-Vroman’s method, prothrombin activity due to one-stage prothrombin time and calcium clotting time were measured. In 39 cases of myocardial infarction and 40 cases of cerebral thrombosis, both in acute stage, a marked decrease in the adhesive platelet count and adhesive index and an increase of prothrombin activity were observed with statistical significance compared to the healthy group (P < 0.01). In the recovery stage of myocardial infarction, cerebral thrombosis and other diseases, except for malignant tumors, acute infectious diseases, diabetes mellitus and blood or liver diseases, the above observations were absent or reduced. In the case of thrombosis, a negative correlation was seen between the adhesive platelet count and prothrombin activity (r — —0.238, P < 0.01). There is no correlation between the changes in platelet adhesiveness or blood coagulability and total cholesterol level in serum.

The plasma fibronectin level is determined in 39 patients with chronic active hepatitis and in 90 patients with cirrhosis of the liver by solid phase immunoenzyme method. All the patients with hepatitis and cirrhosis with signs of disseminated intravascular blood coagulability syndrome manifest a decrease of the fibronectin level, more pronounced in hypocoagulative phase of disseminated intravascular blood coagulability syndrome as a result of the consumption coagulopathy. A sharp decrease of the fibronection level (below 40% that of the norm) may suggest the possible prolongation of disseminated intravescular blood coagulability syndrome, the feasibility of the development of reticuloendothelial system block.

A single dose of 30 µg/gm body weight of depo-heparin was injected subcutaneously into white Swiss mice. At 1, 3, 6 and 12 hours after the injection, the blood coagulation time was measured and the activity of acid and alkaline ribonuclease of liver was determined. This single injection of depo-heparin significantly inhibited the acid and alkaline ribonucleases of liver 1 hour after injection. The enzymatic activities significantly increased after the blood coagulability was restored. The in vivo inhibition of acid and alkaline ribonuclease activity supports the explanation that the accumulation of ribonucleic acid in cells of tissue culture in the presence of heparin might be due to the inhibition of ribonuclease.
