O transtorno afetivo bipolar (TAB) é uma condição potencialmente grave, caracterizada por mudanças de humor e disfunção cognitiva e funcional. O presente artigo revisa as evidências atualizadas das intervenções farmacológicas e não medicamentosas direcionadas ao TAB em idosos. Diretrizes baseadas em evidências, embora sejam úteis na promoção de uma terapia racional e eficaz, ainda são relativamente reduzidas no TAB em idosos. As recomendações atuais para mania aguda incluem o uso de antipsicóticos atípicos (por exemplo, risperidona, quetiapina, olanzapina), lítio (de maneira criteriosa) e a eleição de valproato como terapia padrão ouro. Na depressão aguda do TAB, os agentes de primeira linha em monoterapia podem incluir lítio, lamotrigina, quetiapina e quetiapina de liberação prolongada. No que se refere à escolha do estabilizador de humor ideal, o maior potencial de efeitos colaterais relacionados ao lítio vem desestimulando sua prescrição em indivíduos idosos. A eletroconvulsoterapia, embora limitada pela evidência, pode ser uma opção para casos graves/refratários.

PURPOSE: To systematically review studies that used questionnaires for the evaluation of restriction on auditory participation in adults and the elderly.RESEARCH STRATEGY: Studies from the last five years were selected through a bibliographic collection of data in national and international journals in the following electronic databases: ISI Web of Science and Virtual Health Library - BIREME, which includes the LILACS and MEDLINE databases.SELECTION CRITERIA: Studies available fully; published in Portuguese, English, or Spanish; whose participants were adults and/or the elderly and that used questionnaires for the evaluation of restriction on auditory participation.DATA ANALYSIS: Initially, the studies were selected based on the reading of titles and abstracts. Then, the articles were fully and the information was included in the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) checklist.RESULTS: Three-hundred seventy studies were found in the researched databases; 14 of these studies were excluded because they were found in more than one database. The titles and abstracts of 356 articles were analyzed; 40 of them were selected for full reading, of which 26 articles were finally selected. In the present review, nine instruments were found for the evaluation of restriction on auditory participation.CONCLUSION: The most used questionnaires for the assessment of the restriction on auditory participation were the Hearing Handicap Inventory for the Elderly (HHIE), Hearing Handicap Inventory for Adults (HHIA), and Hearing Handicap Inventory for the Elderly - Screening (HHIE-S). The use of restriction on auditory participation questionnaires can assist in validating decisions in audiology practices and be useful in the fitting of hearing aids and results of aural rehabilitation.

AbstractThis article reviews the literature describing four chemical and nuclear accidents and the lessons learned from each regarding the evacuation of civilian populations. Evacuation may save lives however, if poorly orchestrated, it may cause serious problems. For example, an inaccurate assessment of danger may lead to the evacuation of the same population twice, as the area requiring evacuation becomes larger than originally expected. Evacuation programs should focus on the vulnerable components of the populations, such as the elderly, children, and the disabled, and also should include plans for the care of pets and other animals. Training programs for civilians living near industrial centers and other high-risk areas should be considered. Finally, pre-event planning and preparation can improve the evacuation process and prevent panic behavior, and thus result in fewer casualties.

Delirious mania has been well recognized in the published literature and in the clinic. Over the years there has been refinement of understanding of its clinical features, course, and treatment. The literature suggests that delirious mania should be considered in individuals who present with a constellation of sudden onset delirium, mania, and psychosis. However, delirious mania is not recognized under a formal classification system nor are there any formal guidelines for its treatment. We, as such, question if the concept of delirious mania in the elderly is valid. We present a case of an elderly man with marked features of delirium with minimal manic or psychotic features who had a previous diagnosis of bipolar I disorder. On thorough clinical assessments no identifiable cause of his delirium was found. We therefore considered his presentation to be more likely due to delirious mania. Electroconvulsive therapy was considered and offered to which he responded very well. We invite the reader to consider whether delirious mania is a valid concept in the elderly, where features of delirium may be more prominent than manic or psychotic features.
