Corrected-QT intervals as related to methadone dose and serum level in methadone maintenance treatment (MMT) patients-a cross-sectional study

Sexual dysfunction has been extensively studied in methadone maintenance treatment (MMT) patients. However, little data is available regarding sexual inactivity in the MMT patient population. The objectives of this study were to determine the prevalence and putative risk factors for sexual inactivity in the MMT patient population. This cross-sectional study involved 25–71 year old MMT patients recruited from six methadone clinics. Two hundred and seventy-one patients were interviewed for demographic characteristics, comorbidities, concurrent medications used, and sexual activity. The prevalence of sexual inactivity in the MMT population was found to be 47.6%. Increasing age (p < 0.01) and being single/divorced (p < 0.01) were significantly associated with sexual inactivity. In subgroup analysis, increasing age was significantly associated with sexual inactivity in both single/divorced (p < 0.05) and married (p < 0.05) subgroups, while unemployment (p < 0.05) was only significantly associated with sexual inactivity in the earlier subgroup. Our results suggest that sexual inactivity is common in the MMT patient population. The putative risk factors are related to biological and sociocultural factors. Having specific comorbidities or being on certain medications were not correlated with sexual inactivity in the MMT population. Routine assessment of sexual problems is essential, and proper management should be performed for MMT patients.

Abstract BackgroundTo address the long-standing opioid abuse problem, Myanmar has established the methadone maintenance treatment (MMT) program in 2006. This study aims to determine client’s treatment satisfaction towards MMT program in Myanmar. MethodA total of 210 clients who have registered in MMT program with a minimum of six-month treatment history were recruited for this cross-sectional study across Myanmar. The Verona Service Satisfaction Scale for Methadone-Treatment (VSSS-MT) was administered to objectively measure methadone treatment satisfaction. ResultThe majority (89%, n=186) were satisfied with the current methadone treatment program. Specifically, 89.5% (n=187) claimed to be highly satisfied with the clinic staff professional skills (e.g. doctor, nurse, etc.), (91.9%, n=192) satisfied with the basic interventions (instruction between visits, side-effects etc.), and (74.6%, n=156) satisfied with specific interventions (e.g. individual rehabilitation and psychotherapy, group therapy etc.) categories. Among the different characteristics of the respondents, higher quality of life on physical, psychological, social and environmental categories, respondents who satisfied on current marital status and current leisure status, respondents who were on alcohol were associated with higher treatment satisfaction on methadone (p<0.05). The result of stepwise binary logistic regression showed alcohol use and physical health had significantly association with treatment satisfaction.ConclusionDespite the high methadone treatment satisfaction, treatment providers must make an attempt to address service limitations in order to maintain clients’ participation in MMT program especially for specific intervention categories (e.g. individual rehabilitation and psychotherapy, group therapy etc).

Methadone maintenance treatment (MMT) has been scaled up significantly in recent years. This study aimed to investigate the pattern of polysubstance use in 395 MMT patients and its contextualized associated factors. A cross-sectional study was performed in three outpatient MMT clinics in Nam Dinh Province. Multivariate Poisson regression was used to identify factors associated with polysubstance use status. The mean MMT duration and the current MMT dose was 3.3 years and 69.2 mg, respectively. Among participants, 24.8% reported daily alcohol use, 68.6% smoked regularly, and 6% used illicit drugs. Peer pressure and MMT suboptimal adherence were found to associate with continual usage of drugs (47.8%). Participants who lived with a spouse/partner, were self-employed, and smoked were more likely to drink alcohol. Those who drink were also more likely to smoke, and vice versa. Recommendations for policymakers include community-based education and promotional programs aiming to decrease substance usage in the community as well as encouraging and supporting the private health sector in establishing private MMT services and clinics. Further longitudinal studies on polysubstance usage among MMT patients should also be conducted.
