This chapter provides the therapist with an outline of the COPE treatment and components of each session (e.g. check-in, review homework, post-traumatic stress disorder [PTSD] focus, substance use disorder focus). Questions regarding who can deliver the therapy are addressed, as well as questions regarding the role of medications. Finally, special considerations for delivering treatment to patients with PTSD and comorbid substance use disorders are reviewed for the therapist.

SummaryIn 55 chronic schizophrenics, the occurrence of infectious diseases during their mothers’ pregnancies was investigated. Different psychiatric diagnostic systems were compared. Infections were reported by the mothers of familial and sporadic DSM III-R schizophrenics in equal proportion. However, applying Leonhard's classification, the frequency of infections was found to be significantly increased in ‘systematic’ schizophrenia (mainly exogenously induced in the view of Leonhard) compared to ‘unsystematic’ schizophrenia (mainly genetically determined according to Leonhard's findings). Most of the infections occurred during the second trimester (nine out of 13). Thus, in the ‘systematic’ forms of schizophrenia (low genetic loading), maternal infections in this crucial period of neurodevelopmenl would appear to be important causative factors in the cytoarchitectural deviance delected in the central nervous system of schizophrenics.

Abstract We describe the use of a central laboratory to identify patients who may be candidates for a hypercholesterolemia treatment program and to direct their referral into this program. The laboratory, providing service for 16 medical practices in a rural area of upstate New York, served as the entry point to the treatment program for those patients with serum cholesterol > or = 5.18 mmol/L. This treatment program, designed to follow the National Cholesterol Education Program Adult Treatment Panel Guidelines, was provided by the lipid referral center staff, including a registered dietitian and a lipid specialist. After introduction of this program, 52% of eligible patients received nutritional counseling for hypercholesterolemia, compared with only 29% in usual care settings. This program represents an enhanced role for laboratories in the implementation of treatment protocols typical of those adopted by managed care networks.

Evidence for the importance of delta sleep is reviewed, and the hypothesis tested that marked deficits in these sleep stages are characteristic of chronic schizophrenics. The sleep patterns of 9 chronic schizophrenics were monitored for 8 consecutive nights by means of continuous all-night EEG, eye and chin-muscle recordings. The main finding was that all Ss showed dramatic and stable deficits in delta sleep stages 3 and 4. The role of factors other than chronic schizophrenia producing this finding is discussed.
