Three experiments were conducted to explore the utility of magnitude estimation of loudness for hearing aid selection. In Experiment 1 the loudness discomfort level (LDL), most comfortable loudness (MCL), and magnitude estimations (MEs) of loudness were obtained from normal-hearing subjects. MCLs fell within a range of loudnesses that was relatively low on the loudness function. The LDLs were lower than previously published values. Experiment 2 was performed to identify the source of disparity between our LDL data and previously reported results. The effects of instructions are demonstrated and discussed. In Experiment 3 magnitude estimations of loudness were used to determine the loudness of tonal stimuli selected to represent ⅓ octave band levels of speech. Over the 500–4000 Hz range, the contributions of the various frequency regions to the loudness of speech appears to be nearly constant. Methods are proposed for (a) predicting the frequency-gain response of a hearing aid that restores normal loudness for speech for the hearing-impaired listener and (b) psychophysically evaluating the compression characteristic of a hearing aid.

Estimates of threshold, wide- and narrow-band noise masking, frequency and amplitude modulation detection, gap detection, and rate discrimination were obtained from 10 subjects with near-normal hearing at frequencies above 6 kHz, but severe-to-profound hearing losses at lower frequencies. The same measures were obtained from 10 young control subjects with normal hearing sensitivity for all frequencies up to 16 kHz. The hearing-impaired subjects were able to process sounds in the region of near-normal hearing sensitivity as well as the unimpaired control subjects. Performance in the low-frequency, impaired region depended on the lowest frequency of near-normal hearing sensitivity.

The masking level difference (MLD) at 500 Hz was examined in wide-band (960 Hz) and narrow-band (50 Hz) noise for normal-hearing subjects and subjects with symmetrical mild-to-moderate cochlear hearing loss. Monaural tasks of intensity discrimination, temporal resolution, and frequency resolution were performed in order to examine relationships between monaural dysfunction and MLD performance. Interaural time discrimination for a 500-Hz pure tone also was examined. The performance of the hearing-impaired subjects was poorer than that of the normal-hearing subjects for MLD, interaural Δt, and most monaural tasks. However, no significant relationships were found between monaural and MLD performance when effects of threshold were taken into account. MLDs were more reduced in wide-band noise than in narrow-band noise for the hearing-impaired subjects (when Contrasted with normal-hearing subjects). MLD performance was correlated with interaural time discrimination, and it is suggested that one reason for poor MLD performance with hearing impairment may be poor temporal coding of stimulus-fine structure.

Objectives This study investigates the hypothesis that hearing aid amplification reduces effort within conversation for both hearing aid wearers and their communication partners. Levels of effort, in the form of speech production modifications, required to maintain successful spoken communication in a range of acoustic environments are compared to earlier reported results measured in unaided conversation conditions. Design Fifteen young adult normal-hearing participants and 15 older adult hearing-impaired participants were tested in pairs. Each pair consisted of one young normal-hearing participant and one older hearing-impaired participant. Hearing-impaired participants received directional hearing aid amplification, according to their audiogram, via a master hearing aid with gain provided according to the NAL-NL2 fitting formula. Pairs of participants were required to take part in naturalistic conversations through the use of a referential communication task. Each pair took part in five conversations, each of 5-min duration. During each conversation, participants were exposed to one of five different realistic acoustic environments presented through highly open headphones. The ordering of acoustic environments across experimental blocks was pseudorandomized. Resulting recordings of conversational speech were analyzed to determine the magnitude of speech modifications, in terms of vocal level and spectrum, produced by normal-hearing talkers as a function of both acoustic environment and the degree of high-frequency average hearing impairment of their conversation partner. Results The magnitude of spectral modifications of speech produced by normal-hearing talkers during conversations with aided hearing-impaired interlocutors was smaller than the speech modifications observed during conversations between the same pairs of participants in the absence of hearing aid amplification. Conclusions The provision of hearing aid amplification reduces the effort required to maintain communication in adverse conditions. This reduction in effort provides benefit to hearing-impaired individuals and also to the conversation partners of hearing-impaired individuals. By considering the impact of amplification on both sides of dyadic conversations, this approach contributes to an increased understanding of the likely impact of hearing impairment on everyday communication.

Groups of monaural chinchillas trained in behavioral audiometry were exposed in a diffuse sound field to an octave-band noise centered at 4.0 k Hz. The growth of temporary threshold shift (TTS) at 5.7 k Hz from zero to an asymptote (TTS ∞ ) required about 24 hours, and the growth of TTS at 5.7 k Hz from an asymptote to a higher asymptote, about 12–24 hours. TTS ∞ can be described by the equation TTS ∞ = 1.6(SPL-A) where A = 47. These results are consistent with those previously reported in this journal by Carder and Miller and Mills and Talo. Whereas the decay of TTS ∞ to zero required about three days, the decay of TTS ∞ to a lower TTS ∞ required about three to seven days. The decay of TTS ∞ in noise, therefore, appears to require slightly more time than the decay of TTS ∞ in the quiet. However, for a given level of noise, the magnitude of TTS ∞ is the same regardless of whether the TTS asymptote is approached from zero, from a lower asymptote, or from a higher asymptote.

Residual inhibition, that is, the temporary suppression of tinnitus loudness after acoustic stimulation, is a frequently observed phenomenon that may have prognostic value for clinical applications. However, it is unclear in which subjects residual inhibition is more likely and how stable the effect of inhibition is over multiple repetitions. The primary aim of this work was to evaluate the effect of hearing loss and tinnitus chronicity on residual inhibition susceptibility. The secondary aim was to investigate the short-term repeatability of residual inhibition. Residual inhibition was assessed in 74 tinnitus subjects with 60-second narrow-band noise stimuli in 10 consecutive trials. The subjects were assigned to groups according to their depth of suppression (substantial residual inhibition vs. comparator group). In addition, a categorization in normal hearing and hearing loss groups, related to the degree of hearing loss at the frequency corresponding to the tinnitus pitch, was made. Logistic regression was used to identify factors associated with susceptibility to residual inhibition. Repeatability of residual inhibition was assessed using mixed-effects ordinal regression including poststimulus time and repetitions as factors. Tinnitus chronicity was not associated with residual inhibition for subjects with hearing loss, while a statistically significant negative association between tinnitus chronicity and residual inhibition susceptibility was observed in normal hearing subjects (odds ratio: 0.63; p = .0076). Moreover, repeated states of suppression can be stably induced, reinforcing the use of residual inhibition for within-subject comparison studies.
