Management of Legg–calve–perthes Disease with Acupuncture: A Case Report

Legg–Calve–Perthes disease (LCPD) is a rare temporary hip joint deformity mostly effecting young children from 4–10 years of age. It involves mainly the head of the femur, which softens and breaks down due to interruption of blood supply (avascular necrosis). We report a case of LCPD that was treated late and had a poor prognosis, but improved significantly during a prolonged course of acupuncture. A 12-year-old boy reported to an orthopaedic clinic in 2006 with limping and was diagnosed with LCPD. Surgeons applied orthosis without improvement and decided to perform surgery in 2008. However, the parents declined the surgical option and took the boy to an acupuncture clinic the same year. Needle acupuncture for 20 min and laser acupuncture locally on the hip joint area for 5 min were applied. After 30 sessions of acupuncture, the boy started to improve clinically. Imaging studies showed that new bone cells started to develop in the femoral head. After 130 sessions in 2010 the radiographic appearance showed almost 90% improvement, and after 196 sessions, in August 2012, he was fully recovered. Needle acupuncture treatment combined with laser acupuncture may be an option for the management of LCPD.

Background: Legg-Calve-Perthes disease (LCPD) is a rare condition of idiopathic avascular necrosis of the femoral head, mostly affects children under fifteen old. Most cases only affect one femoral head; bilateral involvement only occurs in about 15% of cases. Early diagnosis and prompt intervention play an important role in the prognosis to prevent degenerative damage on the bones and joints. Objective: To describe a case of Legg-Calve-Perthes disease in a 9-year-old boy in Bethesda Hospital, Yogyakarta. Case Description: A 9-year-old boy came with one month history of worsening pelvic pain to Bethesda Hospital, Yogyakarta. He also showed limping and limited mobility. Initial imaging studies did not find any abnormalities. Further follow-up with MSCT scan related to the complaint of persistent pain showed defects of left hip joint indicating Legg-Calve-Perthes disease. The patient undergone skin traction treatment which shows modest improvement on serial radiographic examinations during hospital admission. Conclusion: The rarity of LCPD warrant thorough imaging studies on children with unexplainable severe hip joint pain. Treatment should be initiated as early as possible after diagnosis to prevent further bone damage and worsening prognosis. Conservative management with skin traction would benefit the patient in restoring normal anatomy and optimal joint congruence to prevent degenerative damage to the joints.

We investigated manual needle and laser needle acupuncture as a complementary therapy for patients with burnout syndrome. Twenty patients with a mean age ± SD of 38.7 ± 8.4 years were assigned to two groups, each consisting of ten patients. One group was treated with manual needle acupuncture and the other with laser needle acupuncture. Heart rate, heart rate variability (HRV), and a new score called dynamic acupuncture treatment score (DATS) served as evaluation parameters. The study documented significant effects on heart rate after needle acupuncture treatment and significant effects on HRV caused by both needle and laser needle acupuncture. Based on new neurovegetative acupuncture treatment evaluation scores, it can be stated that both noninvasive laser needle acupuncture and manual needle acupuncture have the potential to be a powerful approach for evidence-based complementary treatment of patients with burnout syndrome. Further transcontinental studies to verify or refute the preliminary findings are in progress.

Introduction: The Chiari pelvic osteotomy (CPO) has been recommended as a salvage procedure to improve head coverage in case of hip joint incongruence in paediatric hip disease. In this study, we aimed to assess the long-term results of CPO for severe Legg-Calvé-Perthes disease (LCPD). Methods: A total of 39 patients who underwent a CPO at our department between 1995 and 2010 were prospectively followed both radiologically (Stulberg classification) and clinically (Harris Hip Score [HHS], conversion into total hip arthroplasty). In this study, we retrospectively reviewed the cases of 12 hips (12 patients, 3 girls, 8 left hips) treated by CPO for severe LCPD (Catterall grade 3 or 4) with hip joint incongruence. Mean follow-up was 14.0 (range 7.6–21.3) years. Results: Mean age at surgery was 10.2 (range 8.2–17.8) years. Additional femoral osteotomy was performed in 8 patients. A good radiological result (Stulberg I or II) was achieved in 2 patients, a fair result (Stulberg III) in 4 patients, and a poor outcome (Stulberg IV or V) in 6 patients. Mean postoperative HHS averaged 93 (range 65–100) points. An excellent functional outcome (HHS 90–100 points) was achieved in 9 patients. No patient underwent total hip arthroplasty during follow-up. Postoperative limb-length discrepancy was found in 3 patients. Conclusions: CPO for severe LCPD with hip joint incongruence resulted in good long-term clinical outcome in about ⅔ of our patients after a mean of 14 years. Our results suggest that CPO can still be considered as a salvage joint-conserving procedure in this selected group of younger patients.

Examination and treatment of 524 children with coxalgia has been performed. Algorithm for early diagnosis of various hip joint diseases accompanied by coxalgia has been elaborated. The factors causing hip joint response resulting in different pathology including Leg-Calve-Perthes disease have been determined. In the majority of cases application of modified splint in combination with drug therapy enabled to normalize blood circulation in the affected joint and prevent the development of secondary deformities of the femoral head.

The increase in the number of patients with hip pathologies among children of different ages makes the diagnosis and treatment of these diseases one of the most actually existing problems in modern orthopedics. The aim of the work was to determine the features of the lipid peroxidation system - antioxidant protection and hemogram indices in children with various forms of hip joint pathology. The results of a preoperative examination of 47 patients aged 6-15 years (average age 10.3 years) who were treated at the clinic “FSBI NICC“ TO ”named after academician G. A. Ilizarova, Ministry of Health of the Russian Federation. There were 25 boys and 20 girls. Given nosology, patients were divided into five groups. The diagnosis was established on the basis of a clinical and radiological examination. Group I consisted of 8 male patients with stage II Perthes disease (mean age 8.75 + 1.63). Group II includes 16 patients (8 boys, 7 girls) with stage III Perthes disease. (average age 11.80 + 0.89). Group III included 12 patients (6 boys, 6 girls) with aseptic necrosis of the femoral head (average age 14.7 + 2.35). Group IV consisted of 4 patients (1 boy, 3 girls) with epiphyseal dysplasia. (average age 10.25 + 1.36). Group V includes 7 patients with hip dysplasia complicated by aseptic necrosis of the femoral head (mean age 8.33 + 2.11). The norm is the data that was obtained after examining 10 healthy male adolescents (age 13-14) and 5 female adolescents (age 8-14 years). Changes in lipid peroxidation rates and antioxidant activity are unidirectional in different forms of pathology of the hip joints in children, and the content of peroxidation products reliably correlates with hemogram values in stage II osteochondropathy and complicated hip dysplasia. In the complex of diagnostic measures for children with dystrophic lesions of the hip joint, to clarify the nature and stage of the pathological process, as additional criteria, hemogram indicators and lipid peroxidation systems - antioxidant protection can be used.

A 60-year-old woman presented to hospital with abdominal pain and massive weight loss. Imaging studies confirmed the presence of a tumor of the pancreas. Histologic analysis of the sampling performed by echoendoscopic ultrasound fine-needle aspiration found aspects evocative of adenosquamous carcinoma. This case report highlights the difficulties of clinical pathologic diagnosis for these occasionally composite tumors. The patient underwent palliative chemotherapy based on platinum and 5-fluorouracil, followed by second-line chemotherapy with FOLFIRI after progression. Adenosquamous carcinoma of the pancreas remains a rare tumor with very poor prognosis and limited therapeutic options.

Acupuncture manipulation is one of the key factors affecting the performance of acupuncture in Traditional Chinese Medicine. Lift-thrust and twirl-twist are two of the most commonly used manipulation methods for needle acupuncture. We previously have developed a novel laser acupuncture model that emulates lift-thrust operation. In this study, we intend to show the effectiveness of such a model by applying it on the Neiguan acupoint (PC6). Stimulation was reported to be beneficial for improving cardiac output and peripheral circulation. Therefore, we hypothesized that the stimulation of laser acupuncture may increase the temperature of the subjects’ fingertip due to increased peripheral blood flow. A thermal imager was used to measure the temperature change of subjects’ fingertips. Through regression analysis, it has been shown that while PC6 is stimulated, laser acupuncture with lift-thrust operation caused a more rapid, stable, and lasting temperature rise of fingertip than that without lift-thrust operation. On the contrary, we observe no significant temperature change when a sham point nearby PC6 (a point which is not traditionally defined as the acupoint) was stimulated. Our results suggest the emulation of manipulation methods such as the lift-thrust operation could be a potential direction for the future development of laser acupuncture.
