Pneumomediastinum in a cannabis smoker precipitated by vigorous sexual intercourse

Primary pneumomediastinum is the presence of air in the interstitium of the mediastinum. The exact aetiology is unclear; nevertheless, it has been reported more frequently in patients with asthma and in individuals who use recreational drugs. It is commonly preceded by a sharp rise in intrathoracic pressure as in a Valsalva-like manoeuvre. We describe a rare case of severe pneumomediastinum with a small pneumothorax related to cannabis smoking and aggravated by vigorous sexual intercourse. The patient was successfully treated conservatively due to clinical and radiological stability and the absence of secondary cause.

Penile fracture is not uncommon blunt trauma to the penis. Here, we present a rare case of penile fracture during sexual intercourse. The patient presented with penile swelling, bleeding per urethra, and inability to pass urine. Retrograde urethrogram showed significant extravasation of contrast from anterior penile urethra and no contrast passing to proximal urethra. Surgical exploration showed complete urethral rupture and bilateral cavernosal rupture. This case represents the value of urethrogram to evaluate the urethral injury and the association of complete urethral injury with bilateral ventral cavernosal injury.

Humans show a wide array of sexual preferences and behaviour. Zoophilia, or bestiality, consists of sexual intercourse by a human being with a lower animal. There is a paucity of literature on zoophilia. We describe a rare case of preferential zoophilia involving multiple animals. The perpetrator developed this habit only during episodes of depression. It is recommended that due to the apparent common occurrence of this condition, questions exploring a topic which appears to be routinely ignored should be included in the psychiatric interview.

Type IV diaphragmatic hernia is a rare case which has various clinical manifestations. The clinical manifestations as a result of protruded organ towards thoracic cavity in diaphragmatic hernia are varied from gastrointestinal symptoms such as epigastric pain, dysphagia and acid reflux. However, in a rare occasion, dyspnea may present as a result of compression atelectasis due to excessive increased in intrathoracic pressure. Here, we report a 57-year-old male patient who had spontaneous diaphragmatic hernia admitted with dyspnea as a chief complaint. This report might add another perspective to the physicians in facing the patient with atelectasis as the result of a high level of lung compression secondary to type IV diaphragmatic hernia.

Purpose: To present a rare case of unilateral visual loss episodes occurred during sexual intercourse in a young patient affected by unrecognized pigmentary glaucoma and previously undergone myopic refractive surgery. Case description: The patient presented surgically flattened corneas and markedly asymmetric pigmentary glaucoma. Conclusions: Previous refractive surgery, sexual intercourse, and athletic lifestyle might be risk factors for acute pigment dispersion and chronic progression of pigmentary glaucoma in young myopic patients. During their ophthalmic examination prior to refractive surgery, greater attention should be paid to detect early signs of pigmentary dispersion, and awareness of these dangerous situations should be raised in affected patients.

Drug facilitated sexual assault (DFSA) can be defined as sexual activity occurring where consent is invalid or absent due to the effects of drugs and/or alcohol. We report a rare case of pro-active drug facilitated sexual assault involving non-oral administration of sedative-hypnotic intoxicant without primary alcoholic ingestion. For eight years, a male nurse administered sedative-hypnotic drugs to patients admitted to the hospital unit, in order to subsequently maintain sexual intercourse with them. The intravenous administration without primary alcoholic ingestion distinguishes this case of pro-active drug facilitated sexual assault from those presented in associated literature.

A 33-year-old pregnant woman presented at 36 weeks gestation to the emergency with acute abdominal pain that started after vaginal intercourse. No bruising was present on the abdominal examination. An emergent cesarean delivery was performed for resistant hypotension and collapse. A fetus with cardiac arrest was delivered, and active spleen bleeding was identified at the splenocolic and gastrosplenic ligament insertion. The patient had a conservative treatment of the spleen and an uncomplicated postoperative course. The infant was resuscitated and discharged after 18 days. In conclusion, traumatic spleen rupture is an etiology to consider in pregnant women presenting with acute abdominal pain following sexual intercourse. Early suspicion and emergent cesarean delivery are the keys to optimize maternal and perinatal outcomes.

Rectovaginal fistulae after sexual intercourse are rare. We report a healthy recently married 21-year-old woman who presented to the Jordan Healthcare Centre, Amman, Jordan in 2014 with a five-week history of passing flatus and stool from the vagina. Six weeks prior, she had sustained a rectovaginal injury during initial consensual sexual intercourse, leading to the development of a distal rectovaginal fistula. A successful transvaginal repair was performed nine weeks after presentation which resulted in the complete resolution of her symptoms.

Electron microscopy and energy dispersive x-ray microanalysis (EDXA) are found to be very useful tools for identification of etiologic agents in pneumoconiosis or interstitial pulmonary disorders. Pulmonary interstitial fibrosis and granulomatosis are frequently associated with occupational and environmental pollution. Numerous reports of pneumoconiosis in various occupations such as coal and gold miners are presented in the literature. However, there is no known documented case of pulmonary changes in workers in the sandpaper industry. This study reports a rare case of pulmonary granulomatosis containing deposits from abrasives of sandpaper diagnosed by using EDXA.
