SummaryFrench publications on psychotropics are curiously few. Data are available and French consumption of psychotropics, at least minor tranquilizers has been consistently reported as being higher than that found in other countries. The authors attempt to answer three questions: is French consuption of psychotropics really higher than other countries? Who is consuming those drugs? In which context? After having reviewed comparative data on sales, data from population surveys are analysed together with samples of prescriptions by different categories of physicians. It seems likely that French people consume more anxiolytics but this does not apply to other psychotropic drugs. Given the fact that drugs are almost free of charge in France, it is always difficult to discriminate between what has been prescribed and what has been taken. Overconsumption of anxiolytics is due to the overconsumption of the elderly population (over 60). Minor tranquilizers are mainly prescribed by general practitioners for psychological reasons.

AbstractA prospective study was designed in which General Practitioners were issued with a standard referral letter for hearing aid provision. Unknown to the General Practitioner an independent otolaryngologist assessment was obtained at the time of first attendance. Three hundred consecutive patients selected by General Practitioners applying these guidelines were seen in a designated hearing aid clinic staffed by audiological technicians of senior grade or above. Referral pro formata were incomplete in 75 patients who were not assessed. Ninety-four patients (31 per cent) (95 per cent confidence interval 29–36 per cent) were accepted by the audiologist. Obstructing wax was the most common criterion failed. Clinical agreement between audiologist and otolaryngologist was 57 per cent greater than chance. None of the cases of clinical disagreement altered treatment. A direct referral system as proposed could have processed only 31 per cent of 300 referrals. However, experienced technical staff reliably detected otological pathology and with aural toilet facilities 91 per cent of 225 patients (confidence interval 88–94 per cent) were aided.

Underdeveloped nations have the largest absolute number of the world's elderly population. Approximately 10.7% of the Brazilian population comprises aged individuals. Aging populations are associated with a higher incidence of chronic degenerative diseases such as dementia. Demented individuals place a high burden of care on healthcare systems and family members. General practitioners should be able to diagnose the most common elderly diseases such as dementia since they act as gatekeepers to specialized care. In Brazil, many medical students work as general practitioners upon graduating. The present study shows some scenarios of medical schools worldwide, including Brazilian, regarding teaching on dementia.
