Hairy polyps are benign embryological tumors of the head and neck region that are derived from two germinal layers, the ectoderm and mesoderm. At an incidence of 1 : 40000, hairy polyps are considered rare growths. Presenting symptoms of hairy polyps can vary greatly depending on the size and location of the tumor. To the best of our knowledge, our reported case is the first in the literature to highlight an extremely rare occurrence of two separate hairy polyps being simultaneously present in one patient, in the clivus and the nasion, with the presence of meningothelial cells within both tumors, histopathologically. With surgical resection as the management of choice, the approach of surgery differs greatly depending on many factors. Since the patient in our reported case had a cleft palate, we used a combined transnasal and transoral approach to fully release the clival mass and a direct skin incision for the nasion mass. Recurrence after complete surgical resection is rare, and if happens, it should rise suspicion of histopathologically misdiagnosed tumor. We amplified the importance of advanced radiological investigations along with proper multidisciplinary teamwork to exclude CNS connections and other histologically malignant tumors and to early pick up a possible simultaneous lesion.

Abstract Background: Radiation-induced sarcoma (RIS) is a rare complication following radiotherapy of head and neck carcinoma. It occurs mostly within the limits of the irradiated area and always suggests a poor prognosis. Case summary: Herein, we reported one case of a 61-year-old male with laryngeal squamous cell carcinoma (SCC), who had a history of surgery and radiotherapy. In 2014, this patient was admitted in our hospital for increasing hoarseness five years after surgery and radiotherapy. Finally, the patient was diagnosed as fibrosarcoma and was given total laryngectomy, the patient was died in August 2019 within following up. In addition, the clinical and pathological characteristics of similar cases and their probable tumorgenesis were also reviewed. Conclusion: RIS is increasingly important. They usually occur mostly within the limits of the irradiated area. SCC is the most common malignant tumour in the head and neck region, and radiotherapy is a primary adjuvant therapy method. For patients receiving radiotherapy, physicians should follow up more carefully for early detection the RISs. For sarcomas occurring in head and neck region, especially RIS, complete surgical resection is the primary treatment. The choice of radiotherapy and chemotherapy should be more cautious. The prognosis of primary sarcomas or RISs is still controversial. No matter primary sarcomas or RISs, we believe that complete surgical resection should be considered as a top priority in surgery.

<p class="abstract">Lipomas are uncommon in the head and neck region and rare in the retropharyngeal space. Lipoma of the retropharyngeal space may cause aerodigestive tract obstruction presenting as dyspnea and or dysphagia. The presenting symptoms will depend on the size and site of the lipoma in the retropharyngeal space. We present the case of a 57 years old male with 6 months’ history of progressive dysphagia to solid foods only and no positive sign on clinical examination. The barium swallow was normal. The computed tomography (CT) scan showed a moderate-sized homogenous mass of fat density in the hypopharyngeal section of the retropharyngeal space. Diagnosis of dysphagia caused by retropharyngeal lipoma was made. This was surgically excised via the trans-cervical route with no complications, complete resolution of symptoms, and no recurrence 2 years after on follow-up. In the dysphagia of unknown cause, retropharyngeal space lipoma should be considered.</p>

SummaryGlutathione labelled with 99mTc was used to study blood clearance and normal distribution in 3 healthy volunteers and in 10 patients with biopsy-proven tumors in the head and neck region. Static scintigrams were obtained at 1, 3, 6, and 24 h. ROIs over tumors and normal soft tissues were compared to obtain T/N ratios. In normal subjects blood clearance reached a plateau at 6 h; no radioactivity accumulation in the head and neck region was observed. Only the cardiac blood pool, the liver, the kidneys and the urinary bladder were evident. Excretion was via the kidneys. Malignant tumors and metastases were well visualized in 7 patients (true-positive), starting at 1 h. The mean T/N ratio was 2.69 ± 0.77. The best images were obtained at 3-6 h. 1 false-positive (granula-matous reaction), 1 false-negative (malignant epithelial tumor in the radix of tongue) and 1 true-negative (angiofibroma) results were obtained. 99mTc-GSH is a potential radiopharmaceutical for the scintigraphic visualization of head and neck tumors. Further clinical studies are warranted to show its sensitivity and accuracy.

Extrathoracic anthracofibrosis in head and neck region is of extremely rare occurrence and can be confused with malignancy. In this article, we report an unusual case of an anthracofibrotic lymph node of neck that was mistaken for metastatic malignant melanoma in a gastric cancer patient. Because the incidence of an anthracofibrotic lymph node of the neck is very low, it is important to distinguish it from other diseases, including malignancy or metastasis, especially in patients with a cancer history. Thus, pathological diagnosis of anthracofibrosis is necessary to make an accurate diagnosis and find appropriate treatment.

Necrotizing fasciitis (NF) is a fatal and rare disease, mainly located in extremity and body. Due to the good blood supply, the occurrence of this infective disease of skin and subcutaneous tissue/fascia is much rarer in the head and neck region. In this study, we represent periorbital necrotizing fasciitis case in a patient with normal immune system. The patient applied the emergency clinic with the complaints of swelling and redness on the left eye. It was found out that a skin incision occurred at 2 cm below the left eye with razor blade 2 days ago. After taking swab culture sample, patient was started on parenteral Vancomycin + Ampicillin-Sulbactam treatment. It was observed that necrosis spread within hours and an emergent deep surgical debridement was performed. Following the debridement, it was observed that periorbital edema began to regress prominently on the 1st day of the treatment. Treatment was carried on with daily wound care and parenteral antibiotherapy. The patient was discharged from the hospital with slightly cosmetic defect.

In spite of its recognized benefits in the treatment of malignant tumors, radiation therapy have several side effects in the head and neck region. The evaluation of oral conditions by a dentist is important to prevent or minimize these problems. The aim of this retrospective review was to analyze the dental needs in 357 patients who received radiotherapy in the head and neck region and were treated at Orocentro/FOP/UNICAMP, between January 1990 and December 2004. Review of patient files showed that dental examination before radiotherapy was not performed in 148 patients (41.5%) and was done in 209 patients (58.5%). From the total of examined patients, 94 (45%) did not require dental procedures at the moment of examination, while 115 (55%) presented some sort of dental need. Following the patients after the radiotherapy, it was observed that the group of patients that was evaluated before radiation presented less need of restorations, root canal filling and dental extractions than those who were not evaluated. The results of this study confirm that the evaluation of oral conditions prior to radiotherapy is essential to minimize the dental needs, emphasizing the importance of the dentist in the multidisciplinary team that treats cancer patients.

Inflammatory myofibroblastic tumor (IMT) of the head and neck region are rare and benign and of unknown etiology. They mimic as malignant lesions both clinically and radiologically. It encompasses a spectrum of myofibroblastic proliferation along with varying amount of inflammatory infiltrate. Recently, the concept of this lesion being reactive has been challenged based on the clinical demonstration of recurrences and metastasis and cytogenetic evidence of acquired clonal chromosomal abnormalities. Here we report a case of a 16 year old male who presented with a swelling on the nasal dorsum for 6 months. MRI and FNAC revealed a spindle cell neoplasm. The swelling was resected completely under general anaesthesia. Histopathology with immunohistochemistry proved it to be a benign inflammatory myofibrobalstic tumour.  Complete surgical resection is the best treatment modality for IMT.

Primitive neuroectodermal tumour/extraskeletal Ewing’s sarcoma (PNET/EES) is a rare disease of the head and neck region. We report a case of a 74-year-old man with a laryngeal Ewing’s sarcoma. This is the first reported case of extraskeletal Ewing’s sarcoma of the larynx in an elderly male patient. The patient was successfully treated with surgical resection and post-operative radiotherapy.
