Middle ear adenomatous neuroendocrine tumors (MEANT) are rare, mainly benign neoplasms and account for less than 2% of the middle and inner ear neoplasms. There are no specific radiological or clinical findings; therefore, the diagnosis is often difficult. Biopsy prior to surgery is often proposed. The definite treatment is the complete surgical resection. We present an uncommon clinical report of a MEANT, which was treated successfully with surgical excision.

Otosclerosis is a term derived from oto, meaning“of the ear,”and sclerosis, meaning“abnormal hardening of body tissue.”The condition is caused by abnormal bone remodeling in the middle ear. Bone remodeling is a lifelong process in which bone tissue renews itself by replacing old tissue with new. In otosclerosis, abnormal remodeling disrupts the ability of sound to travel from the middle ear to the inner ear. A Hindu,married 50 yr old Female patient visited the outpatient department of Institute with complaint of Diminished of hearing in both ear (Left ear > Right ear), Ringing sensation in both ear & Blocking of both ear. The patient was diagnosed as case of Otosclerosis with clinical findings & Investigations done. Tablet Lakshadi guggulu 2TDS(each250mg), Tablet Amarsundari Vati2TDS(each250mg),Powder combination of Aamalaki churna (1gm)+ AshwagandhaChurna(1gm)+PunarnavaChurna(500mg)+JatamansiChurna(500mg)+Sunhi churna(250mg)+ Godanti churna (250mg) And Syp Dashmooladi Kwath10ml TDS were administered for 6 months. After the 6 months treatment ,a significant response in various symptoms such as Diminished of hearing, Ringing sensation & Blocking of ear was found.   Key words- Otosclerosis, Hearing loss, Diminished of hearing,Audiometry, tinnitus, Badhirya

Middle ear adenomatous neuroendocrine tumor (MEANT) poses a diagnostic challenge. Clinical symptoms are nonspecific. Definite diagnosis is made by histopathological analysis of the tumor after a complete surgical resection based on an extensive computed tomography/magnetic resonance imaging (CT/MRI). Controversial terminology of the neoplasm arises from the differentiation of this tumor composed of both endocrine and exocrine glands. Middle ear (ME) localization is rare and less aggressive than gastrointestinal tract or lung localizations. Nevertheless, clinical and CT/MRI analyses are necessary follow-ups for preventing or detecting recurrence or metastasis. A case of a female patient aged 26 with recurrent middle ear neuroendocrine adenoma is presented herein.

Isolated cardiac hydatid cyst is a rare entity. It warrants early surgical repair because cyst rupture is potentially fatal. We report the case of a 32-year-old lady with an epicardial ventricular hydatid cyst, which was managed successfully by complete cyst excision.

<b><i>Introduction:</i></b> Koenen tumors are benign, cutaneous manifestations of tuberous sclerosis. These are disfiguring, painful, and challenging to treat as they frequently recur. We report a case of long-standing, multiple Koenen tumors affecting all twenty nails in an elderly female who was successfully treated with a combination of topical sirolimus 1%, surgical excision, and electrofulguration. <b><i>Case Report:</i></b> A 57-year-old lady presented with multiple, asymptomatic periungual, and subungual tumors affecting all twenty nails since 27 years. Cutaneous examination revealed confetti macules, ash-leaf macule, and shagreen patch over trunk. Nail biopsy was compatible with Koenen’s tumor. Computerized tomography of brain showed diffuse patchy sclerosis. The tumors were treated with topical sirolimus 1% ointment for 10 months with excellent regression. Electro­fulguration for both great toenails and surgical excision of right thumbnail periungual fibroma was done. 1% sirolimus was advised after the surgical treatment. There were no adverse effects or recurrence of tumors over a 2-year follow-up. <b><i>Discussion:</i></b> Topical sirolimus 1% was effective in tumor regression and preventing new tumor formation. Larger tumors that interfered in daily chores were treated with excision and electrofulguration. Thus, a combination treatment for this rare presentation of tuberous sclerosis provided optimum results.

<p class="abstract">Pleomorphic adenoma are common benign salivary gland tumours, which are found in majority in major salivary glands such as parotids and submandibular glands. However, Pleomorphic adenoma to originate from dorsum of the nose is a rare entity. In rare cases, it can be found in unusual sites such as upper aero digestive tracts, palate and lacrimal glands. Complete surgical resection is the treatment of choice. Though, the evolution to malignancy and recurrence is not usually encountered, still a long-term follow-up is recommended. Here, we report a 53 year old female , complained of swelling in the left side of dorsum of nose for 10 years with a feeling of heaviness over the left side of face and difficulty in vision on the side of swelling due to the enlarged size of the swelling which gave a feeling of vision disruption , also had a prior history of  incision and drainage 4 years ,done elsewhere. On clinical examination, nodular mass was palpated on left side dorsum of nose which was freely mobile, and Skin over the swelling had blackish pigmentation. Anterior rhinoscopy revealed no abnormalities. Complete surgical excision via a lateral rhinotomy incision was performed. Cytological and histological evaluation revealed the presence of pleomorphic adenoma. We observed a decent cosmetic outcome with no evidence of recurrence.</p>
