Measurement of the Upper Limit of Vulnerability during Defibrillator Implantation can Substitute Defibrillation Threshold Measurement

We investigated whether defibrillation thresholds (DFTs) could be measured more safely during defibrillator implantation by measuring the upper limit of vulnerability (ULV) without using any special equipment. Nonthoracotomy ICD implantation with endocardial leads was performed in 13 patients, and through the use of the ICD function itself, ULV and DFT were measured using the delayed four-episode up-down algorithm. Myocardial injures caused by high-energy current were assessed by electrocardiograms and serial CPK-MB. ULV was confirmed in all cases, and it strongly correlated with DFT. The average ULV was 5.9 ± 3.3J, while the average DFT was 7.9 ± 4.3J (r = 0.89, p < 0.0001, DFT = 1.20+1.14x ULV). The average ULV was thus significantly lower (p < 0.01). Although six patients were on amiodarone therapy, the strong correlation between ULV and DFT was also maintained (r = 0,97), p < 0.01) in these patients. In all cases, the CPK-MB failed to increase, and no myocardial injuries were detectable on electrocardiograms. We confirmed that ULV could be easily and safety measured during ICD implantation, and that ULV could be used instead of DFT.

Head and neck cancers, including oral squamous cell carcinoma (OSCC), are the sixth most common malignancies worldwide. OSCC frequently leads to oral dysfunction, which worsens a patient’s quality of life. Moreover, its prognosis remains poor. Unlike normal cells, tumor cells preferentially metabolize glucose by aerobic glycolysis. Pyruvate kinase (PK) catalyzes the final step in glycolysis, and the transition from PKM1 to PKM2 is observed in many cancer cells. However, little is known about PKM expression and function in OSCC. In this study, we investigated the expression of PKM in OSCC specimens and performed a functional analysis of human OSCC cells. We found that the PKM2/PKM1 ratio was higher in OSCC cells than in adjacent normal mucosal cells and in samples obtained from dysplasia patients. Furthermore, PKM2 expression was strongly correlated with OSCC tumor progression on immunohistochemistry. PKM2 expression was higher during cell growth, invasion, and apoptosis in HSC3 cells, which show a high energy flow and whose metabolism depends on aerobic glycolysis and oxidative phosphorylation. PKM2 expression was also associated with the production of reactive oxygen species (ROS) and integration of glutamine into lactate. Our results suggested that PKM2 has a variety of tumor progressive functions in OSCC cells.

Importance of vibronic effects has been highlighted for the singlet-fission (SF) that convert one high-energy singlet exciton into doubled triplet excitons, as strongly correlated multiexcitons. However, molecular mechanisms of spin conversion processes and ultimate de-couplings in the multiexcitons are poorly understood. We have analyzed geometries and exchange couplings of the photoinduced multiexcitons in the pentacene dimers bridged by a phenylene at ortho and meta positions [denoted as <i>o</i>-(Pc)<sub>2</sub> and <i>m</i>-(Pc)<sub>2</sub>] by simulations of the time-resolved electron paramagnetic resonance spectra. We clarified that terahertz molecular conformation dynamics plays a role on the spin conversion from the singlet strongly coupled multiexcitons <sup>1</sup>(TT) to the quintet state <sup>5</sup>(TT). The strongly coupled <sup>5</sup>(TT) multiexcitons are revealed to possess entirely planar conformations stabilized by mutually delocalized spin distributions, while the intramolecular de-coupled spin-correlated triplet pairs generated at 1 microsecond are also stabilized by distorted conformations resulting in two separately localized biradical characters.
