Public housing residents have high intake of added sugars, which is associated with sugar-sweetened beverage (SSB) consumption in their social networks. In this feasibility study, we designed and tested a network-oriented intervention to decrease added sugar intake by encouraging reduced SSB consumption. We conducted a 6-month single-arm trial testing a small-group curriculum (9 sessions) that combined behavior change strategies to reduce added sugar intake by promoting SSB reduction with a peer outreach approach. We recruited and trained public housing residents to be “Peer Educators,” who then communicated information and made changes to reduce SSB with their network members. We calculated the median number of group sessions attended and determined the percentage of individuals satisfied with the program. We estimated added sugar intake using a 5-factor dietary screener and compared baseline and 6-month median values using Wilcoxon signed rank tests. We recruited 17 residents and 17 of their network members (n = 34). Mean age was 45.7 years, 79.4% were women, and 97.1% were African American. Median number of sessions attended was 9 (interquartile range: 4-9), and 88.2% were very satisfied with the program. Overall, baseline median added sugar intake was 38.0 tsp/day, which significantly declined to 17.2 tsp/day at 6 months ( P < .001). Residents and network members achieved similar results at 6 months (17.4 vs 16.9 tsp/day, respectively). In conclusion, our results demonstrate that a social network intervention aimed at reducing SSB consumption is feasible and can produce significant decreases in adult added sugar intake, which warrants further investigation in a randomized controlled trial.

Abstract Objective: To test the effectiveness of a social network intervention (SNI) to improve children’s healthy drinking behaviours. Design: A three-arm cluster randomised control trial design was used. In the SNI, a subset of children were selected and trained as ‘influence agents’ to promote water consumption–as an alternative to sugar-sweetened beverages (SSB)–among their peers. In the active control condition, all children were simultaneously exposed to the benefits of water consumption. The control condition received no intervention. Setting: Eleven schools in the Netherlands. Participants: Four hundred and fifty-one children (Mage = 10·74, SDage = 0·97; 50·8 % girls). Results: Structural path models showed that children exposed to the SNI consumed 0·20 less SSB per day compared to those in the control condition (β = 0·25, P = 0·035). There was a trend showing that children exposed to the SNI consumed 0·17 less SSB per day than those in the active control condition (β = 0·20, P = 0·061). No differences were found between conditions for water consumption. However, the moderation effects of descriptive norms (β = –0·12, P = 0·028) and injunctive norms (β = 0·11–0·14, both P = 0·050) indicated that norms are more strongly linked to water consumption in the SNI condition compared to the active control and control conditions. Conclusions: These findings suggest that a SNI promoting healthy drinking behaviours may prevent children from consuming more SSB. Moreover, for water consumption, the prevailing social norms in the context play an important role in mitigating the effectiveness of the SNI.

Our objective was to characterize the relationship between public housing residents’ diet/exercise habits with similar behaviors among their social network. We conducted a cross-sectional survey of randomly selected households in Baltimore, Maryland, from August 2014 to August 2015. Adult heads of household completed questions on diet, exercise, and perceived habits among network members. Our dependent variables were high added sugar intake (≥39.9 teaspoons/day), high fruit/vegetable intake (≥6.1 servings/day), and being physically active (≥moderately activity). Our network exposures were proportion of members perceived to daily consume (1) sugar-sweetened beverages, (2) sweets, (3) fruits, and (4) vegetables, as well as to weekly exercise (1) vigorously or (2) moderately. We used multivariate logistic regression to examine associations between habits with relevant network exposures. Our sample included 266 adults with mean age of 44.5 years, 86.1% women and 95.5% African American. We found a statistically significant association between study participants’ high daily intake of added sugar with perceived network exposure to daily sugar-sweetened beverages (odds ratio [OR] = 1.10, 95% confidence interval [CI] [1.02, 1.20]) and daily sweets (OR = 1.10, 95% CI [1.02, 1.20]). Greater network exposure to weekly vigorous exercise was significantly associated with personally being physically active (OR = 1.15, 95% CI [1.04, 1.28]), but not network exposure to weekly moderate exercise. Among public housing residents, associations exist between individuals’ and perceived networks’ lifestyle habits of high added sugar foods consumption and vigorous exercise, which may hold promise for future social network interventions.
