This paper describes the orthodontic treatment of two cases that were successful in winning the British Orthodontic Society (BOS) Membership in Orthodontics (MOrth) Cases Prize in 2019. The first case describes the management of a 12-year-old girl with a Class II division 2 malocclusion complicated by moderate upper and lower arch crowding, multiple unerupted teeth, restored lower first permanent molars, pseudo-transposition of the lower left lateral incisor and canine, and diminutive upper lateral incisors. Treatment involved a combination of an upper removable appliance followed by upper and lower preadjusted edgewise fixed appliances. Anteroposterior correction and overbite reduction was achieved with triangular Class II elastics with posterior occlusal disengagement. Both upper permanent canines were exposed and aligned, and the diminutive upper incisors built up with resin-based composite. Treatment was completed over a period of 23 months. The second case describes the management of a 13-year-old boy with a Class II division 2 malocclusion complicated by severe upper and lower arch crowding with unerupted UR5, UL4, LR3, rotated LR5, an increased overbite complete to tooth, buccally displaced upper canines and hypoplastic upper first premolars. Treatment involved a first phase of functional appliance therapy, followed by the extraction of UR4, UL4, LL5, LR4 and upper and lower preadjusted edgewise fixed appliances over a 28-month period.

Background: A unique clinical challenge presents when dealing with a compromised first permanent molars with bilateral posterior crossbite, severe crowding and impacted maxillary canines with skeletal class II base malocclusion patient. Case presentation: 14-year-old female patient had dental Class II skeletally, complicated with increase overjet, badly destructed permanent mandibular 1st molars constricted maxillary arch. Extraction of 1st molars followed by expansion have been planned to relieve crowding. Extraction of 1st molars in this time (furcation of 3rd molars start to develop) help in replacement by 2nd molars. In the progression of treatment, Conclusion: A well-balanced and esthetic occlusion by edge wise orthodontic treatment has been archived in this case. Abstract Objective: To test the hypothesis that there is no difference in the distal movement of the maxillary first permanent molars when cervical headgear is used alone or in combination with rapid maxillary expansion. Materials and Methods: The sample was composed of 36 subjects (aged 9 to 13 years), treated in the Faculty of Dentistry, Pontifícia Universidade Cat;aaolica, Rio Grande do Sul, Brazil. The individuals were in good health and in their pubertal growth period. All had Class II division 1 malocclusion. The patients were divided into two groups: group 1 (22 subjects), Class II, with a normal transverse maxilla treated with cervical traction headgear (HG) 400 g 12 h/d, and group 2 (14 subjects), Class II maxillary transverse deficiency treated with rapid maxillary expansion plus cervical traction headgear (RME + HG). An additional group 3 (17 subjects) served as a control group and included individuals with the same characteristics. All subjects had two lateral cephalograms: initial (T1) and progress (T2), taken 6 months later. Differences between T1 and T2 were compared with the Student's t-test, and three groups were compared by the analysis of variance and Tukey multiple comparison test. Results: Results showed greater distal tipping and greater distal movement of the first permanent molars in group 1 (HG) than in group 2 (RME + HG), P < .05. No extrusion of first permanent molar occurred in either group (P > .05). Conclusion: The hypothesis was rejected. Cervical traction headgear alone produced greater distal movement effects in maxillary first permanent molars when compared with rapid maxillary expansion associated with cervical headgear.

ABSTRACTBased on our literature search, we found that the use of miniplate anchorage with Forsus fatigue-resistance device (FRD) has not yet been reported. Therefore, the aim of the present case report was to present the treatment of a patient with skeletal Class II malocclusion with mandibular retrusion using Forsus FRD with miniplate anchorage. Fixed appliances with 0.022-inch slots were attached to the maxillary teeth and after 8 months of the leveling and alignment of the upper arch, 0.019 × 0.025-inch stainless steel archwire was inserted and cinched back. Two weeks after the placement of the miniplates bilaterally at the symphysis of the mandible, Forsus FRD was adjusted to the miniplates with a 35-mm length of rod chosen. Nine months after the skeletal anchored Forsus worn, Class I canine and molar relations were achieved and overjet was eliminated.

This paper reports a Class II, division 2 malocclusion case successfully treated at an early age and in a relatively short period of time using the Trainer for Kids (T4K), a prefabricated functional appliance. Skeletal changes observed in the before and after treatment cephalic radiographs were compared with the expected changes produced by the patient's natural growth. The functional appliance's effects resulting in the outcomes observed in this clinical case, as well as the importance of identifying the etiological factors when treating a malocclusion will be discussed.

ABSTRACT Objective: To compare the facial profile attractiveness of Class II patients treated with Twin Force® or intermaxillary elastics. Methods: Sample comprised 47 Class II patients divided into two groups: G1) TWIN FORCE - 25 patients treated with fixed appliances and Twin Force® fixed functional appliance (mean initial age was 17.91 ± 7.13 years, mean final age was 20.45 ± 7.18 years, and mean treatment time was 2.53 ± 0.83 years); G2) ELASTICS - 22 patients treated with fixed appliances and Class II intermaxillary elastics (mean initial age was 15.87 ± 5.64 years, mean final age was 18.63 ± 5.79 years and mean treatment time was 2.75 ± 0.60 years). Lateral cephalograms from pretreatment and posttreatment were used. Cephalometric variables were measured and silhouettes of facial profile were constructed and evaluated by 48 laypeople and 63 orthodontists, rating the attractiveness from 0 (most unattractive profile) to 10 (most attractive profile). Intergroup comparisons were performed with Mann-Whitney and independent t-tests. Results: At pretreatment, facial profile of the Twin Force® group was less attractive than the Elastics group. Treatment with Twin Force® or Class II elastics resulted in similar facial profile attractiveness, but the facial convexity was more reduced in the Twin Force® group. Orthodontists were more critical than laypeople. Conclusions: Treatment with Twin Force® or Class II elastics produced similar facial profile attractiveness at posttreatment. Profile attractiveness was reduced with treatment in the elastic group, and improved in the Twin Force® group. Facial convexity was more reduced with treatment in the Twin Force® group.

A technique is described for the correction of Class II malocclusion involving a functional appliance and consisting of three phases. 1. Adjustment of posterior arch width and dental alignment, using semi-rapid maxillary expansion by means of an upper removable appliance, to co-ordinate the anticipated positions for the arches. 2. Correction of the Class II dental relationships using a modification of the twin-block functional appliance. 3. Retention, using an upper removable appliance with a very steep anterior bite plane. The technique is illustrated by a case report.

A 17 year-old Bangladeshi female presented with a severe Class II division 1 incisor relationship on a mild Skeletal II base. Crowding was moderate in the upper arch. The lower arch was well aligned. Treatment was commenced using fixed appliances and followed by extractions of upper first premolars. This case illustrates the versatility of the fixed appliances in the treatment of those cases exhibiting Class II division 1 malocclusion with crowding.
