AimsWe assessed whether a quality improvement plan initiated in 2018 had sustained benefits for improving attendance rates at addiction prescriber reviews, after 13 months.MethodThe QIP re-audit had Humber Teaching NHSFT approval. We assessed electronic healthcare records of patients prescribed OST at a specialist addictions service, spanning a large geographical area, split into three Hubs. Data were analysed via Microsoft excel.Baseline data for the whole addictions service were collected in April 2018 (n = 343), followed by QIP implementation. The QIP included a new appointment letter explaining the importance of the prescriber review, text message confirmation and reminder the day before, verbal reminder from keyworker and a call from the prescriber explaining the importance of attending (for persistent non-attenders). In the event of nonattendance, a medication safety review was completed. Further data were collected in December 2018 (n = 339) and a re-audit of one Hub (n = 91) was completed in Jan 2020.ResultAt baseline in April 2018, half (50% n = 170/343) of all patients had attended an addictions prescriber review in the last 3 months; Hub 1 (55%; n = 52/95), Hub 2 (34%; n = 45/133) and Hub 3 (65%; n = 73/115). The Quality Improvement Plan was implemented. Attendance rates for subsample (Hub 1) conducted in Oct 2018 showed a reduction in attendance (51%; n = 48/92). This led to the enhanced Quality Improvement Plan.After the enhanced Quality Improvement Plan implementation in Dec 2018, attendance rates improved for all Hubs to 76% (n = 258/339); Hub 1 (77%; n = 72/93), Hub 2 (73%; n = 97/133), Hub 3 (79%; n = 89/113). For non-attending patients, a medication review was conducted in their absence by the prescriber for most (94%; n = 74/81) patients (see table 1 and Figure 1).In January 2020, reassessment of attendance rates for Hub 1 (subsample), in January 2020 (n = 91) which showed attendance had increased to 86% (n = 78/91). All (100% n = 13) patients who did not attend for the prescriber review in person, had a medication review in their absence. In addition, the reasons for nonattendance were discussed with the patient and their keyworker, following which they were booked for a subsequent appointment.ConclusionNonattendance at clinical appointments causes a significant financial burden across the NHS. It was fantastic to see that the QIP improved patient attendance rates and this was sustained and improved, over a year later. Serial non-attenders may need an enhanced strategy.

AbstractRisks of pediatric tracheostomy are well known. The objectives of this quality improvement study were to organize tracheostomy supplies into a comprehensive care kit and demonstrate that the kits improved nursing and parental comfort in providing tracheostomy care routinely and emergently. Kits were assembled using roll-up toiletry style bags and organized in a uniform fashion with necessary supplies. Nurses and parents were surveyed using a 5-point Likert-type. Feedback was overall very positive; the kits were found to ease the transition of caring for a child with a new tracheostomy from hospital to home. This intervention can easily be adapted at other pediatric institutions.

Introduction: A novel continuing professional education CPE training program and simulation were used to teach pharmacists and pharmacy technicians about continuous quality improvement and how to identify, report, and communicate information regarding medication related errors using root cause analysis.   Methods: Pharmacists and pharmacy technicians attending a statewide pharmacy association meeting voluntary attended a CPE training program and simulation.  During the simulation, learners investigated and identified medication related errors in three different pharmacy settings.  A collection of items found at each pharmacy and audio recordings were used by learners to identify the medication related error.  After each simulation, facilitators led a debriefing to discuss the learners’ experiences.  Data was collected using online surveys.  Descriptive statistics and chi-square tests were used to analyze the data. Results:  Fourteen months following the program, 15 of the 67 participants responded to an anonymous survey.  Of the 15 responding participants, 73.3% (11/15) were confident or very confident they could establish or maintain a high-quality continuous quality improvement plan at their practice site.  Sixty percent (9/15) felt the experience reinforced their current practices, 13.3% (2/15) had implemented changes to their practice, and 13.3% (2/15) felt they needed more information before considering changes to their practice.  Reported barriers to establishing a continuous quality improvement program were time constraints, 40.0% (6/15), system constraints, 26.7% (4/15), or lack of staff 20.0% (3/15). Conclusion: A CPE training program and simulation reinforced practice for pharmacy personnel, resulted in changes to practice, and positively increased participants’ confidence in establishing a continuous quality improvement plan in the workplace.    Article Type: Note

ABSTRACT. Morphology, characterization, and geographical distribution of Blechnum cordatum (Blechnaceae-Pteridophyta). Specimens of Blechnum cordatum from localities of its large geographical area were analized. The species grows in Mesoamerica, Antillas and South America, from Venezuela and Colombia to Bolivia, SE and centre of Brazil, Paraguay, Argentina, centre and S of Chile, and Juan Fernández Islands. Blechnum cordatum is a tolerant, not vulnerable species, with large sporophytes; erect, scaly rhizomes; dimorphic fronds with scaly stipes and axes; lanceolate sterile laminae with lanceolate to oblong, coriaceae, finely denticulate to serrate, superficially scaly and hairy, attached by costa (peciolulate) pinnae, with cuneate- truncate to subcordate or auriculate bases, and narrowly lanceolate fertile laminae with vegetative tissue of pinnae reduced to the portion which support the undulate to erose indusia and continuous coenosorus. Veins are free, simple, geminate and furcate, the latter ramdomly dividing at different distances from the costa, all ending in large, active hydathodes. Aerophores, located only at the base of pinnae, may be absent. Spores have a cristate-reticulate perispore with filiform, ramified processes, and a smooth to granulate exospore. Based on this study, a new description of Blechnum cordatum, and its taxonomy is presented, along with comments on affinities with other neotropical and paleotropical species of the genus.Key words. Blechnum cordatum fue estudiado en especímenes de numerosas localidades de su extensa área de distribución. Crece en Mesoamérica, Antillas, Sudamérica, desde Venezuela y Colombia a Bolivia, SE y centro de Brasil, Paraguay, Argentina, centro y S de Chile e islas de Juan Fernández. Es una especie poco vulnerable, tolerante, con esporófitos grandes, rizomas a oblongas con pinnas coriáceas, lanceolado-oblongas, finamente denticuladas a aserradas, superficialmente escamosas y pilosas, unidas al raquis por la costa (pecioluladas), con bases cuneado- truncadas a subcordadas o auriculadas y láminas fértiles estrechamente lanceoladas con el tejido vegetativo de las pinnas reducido a la porción de la lámina que lleva el cenosoro continuo e indusio ondulado a eroso. Las venas son simples, geminadas y bifurcadas al azar a distancias variables de la costa y terminan en grandes hidatodos activos, sobresalientes o más o menos planos. Los aeróforos, presentes sólo en la base de las pinnas, pueden faltar. Las esporas son monoletas, con perisporio crestado-reticulado que lleva procesos filiformes y exosporio subliso a granulado. La especie se describe e ilustra en detalle, se actualiza su taxonomía y se comentan sus afinidades con otras especies neotropicales y paleotropicales del género.Palabras clave. Education has been the greatest tool for human resources development. The advances in information and communication technology has brought out a paradigm shift in the educational sector by making it more accessible, relevant, qualitative, and equitable for the masses. The use of satellite technology like INTELSAT, PEACESAT, and ATS in education has enhanced the opportunities for learners to acquire new skills (Moore & Kearsley, 1996). Both on-campus and distance mode students can be benefited by it. The satellite technology can serve a large geographical area. It allows audio and video signals uplinked from a station to be received to any number of downlink earth stations (Willis, 1995). Oliver (1994) reported that the transmission costs do not increase with the increase in the number of downlink stations. Satellite Instructional Television Experiment (SITE), one of the India’s early experiments conducted during 1975 to 1976, produced and transmitted 150 different science programs of 10 to 12 minutes duration, offering them to more than 2,330 villages in six geographical clusters. According to Shrestha (1997) and Govindaraju and Banerjee (1999), this experiment demonstrates the effectiveness of satellite communication for educational purposes.
