Abstract We present a rare and challenging case of left ventricular aneurysm in an African child with no history of previous infection or trauma, admitted for surgical treatment, who presented non reversible cardiorespiratory arrest with cardiorespiratory resuscitation before surgery.

RESUMO Objetivo: relatar a experiência vivenciada por acadêmicos de enfermagem no cuidado a uma vítima de ruptura de aneurisma de artéria aorta, evoluindo com parada cardiorrespiratória. Método: estudo descritivo do tipo relato de experiência vivenciado por acadêmicos da graduação em enfermagem da disciplina de Atenção Integral à Saúde II - Módulo de Alta Complexidade. Resultados: esta experiência forneceu subsídios teóricos e práticos, visando a obtenção de conhecimentos e novos comportamentos com base no processo de enfermagem e na assistência integral no cuidado ao paciente adulto crítico no âmbito hospitalar de urgência e emergência e terapia intensiva, perpassando pelas etapas de diagnóstico, monitoramento e tratamento, como visa o Ministério da Saúde. Conclusão: a vivência no campo de prática hospitalar proporcionou aos acadêmicos de enfermagem o envolvimento em situações complexas que exigem a articulação dos saberes teóricos e experimentais fundamentados em sala de aula. Isso é possível à medida que o hospital se torna um cenário de aprendizagem capaz de suscitar o raciocínio crítico para soluções de problemas. Results: this experience has provided theoretical and practical aiming at obtaining knowledge and new behaviors based on the nursing process and integral assistance in adult critical care to the patient within the hospital of urgency and emergency and intensive therapy covering the steps of diagnosis, monitoring and treatment, as is the Ministry of Health. Conclusion: the experience in the field of hospital practice provided to academics of nursing involvement in complex situations that require the articulation of theoretical and experimental knowledge grounded in the classroom. It is possible to the extent that the hospital becomes a scenario of learning capable of beginning the critical reasoning to solve problems. Background: IVLBCL is potentially treatable but difficult to diagnose. Methods: A case of progressive leg weakness and sphincter dysfunction diagnosed only at autopsy. Results: A 72 year old female presented with three weeks of increasing leg weakness and three days of urine and stool incontinence. Electrophysiological testing showed reduced tibial and peroneal CMAPs, preserved sural SNAPs, and bilateral gastrocnemius fibrillations, consistent with a pre-ganglionic lesion. Lumbosacral MRI with gadolinium showed no abnormalities. CSF was examined three times (protein 1.18-1.25, glucose 2.6-3.1, normal cell count and cytology). Whole-body FDG PET scanning showed hypermetabolic foci at the tongue base and in the mediastinum, but biopsy of both revealed no abnormality. Leg weakness progressed over three months and spread to the arms despite a course of IVIg. Four months later she died of cardiorespiratory arrest. Autopsy revealed the presence of large atypical B-cells within the lumen of small and medium sized vessels in numerous organs. There was evidence of anterior spinal artery obstruction with lymphocytes and anterior horn infarction in the lumbar cord. Conclusions: Although the literature reports that IVLBCL responds well to chemotherapy, this patient illustrates the difficulty of ante-mortem diagnosis. Nerve root biopsy may be warranted in such patients.
