The Prevention of Tooth Wear

With adult dentitions being retained for longer, issues relating to the wear of teeth are increasingly becoming of relevance to the general dental practitioner. As with any treatment plan, prevention is a fundamental and an important stage in the management of tooth wear. This article outlines the contemporary preventive features that the general dental practitioner should be aware of when dealing with patients presenting with tooth wear or exhibiting a susceptibility to this. CPD/Clinical Relevance: An understanding of the factors relevant to the prevention of tooth wear will enable an effective preventive regimen to be prescribed. This will play an integral role in the management of tooth wear and its sequalae.

This is the first article in a three-part series considering uncommon dental anomalies and treatment approaches. Dental transpositions are rare anomalies which may present to the general dental practitioner or orthodontist. The management of transpositions is dependent on several factors, including the sub-type, severity and the other salient features of the malocclusion. Transposition may either be accepted or orthodontically corrected and the adoption of either approach must be preceded by thorough assessment of the patient. CPD/Clinical Relevance: The presentation and management of transposition is variable. The general dental practitioner must be aware of the clinical features of a developing transposition as certain treatment options are only possible before the transposition is fully established. In addition, the orthodontist must be mindful of the potential for iatrogenic harm if these dental anomalies are managed injudiciously.

Body Dysmorphic Disorder (BDD) is a relatively common psychiatric condition in which the individual is disproportionately concerned about an aspect of his/her appearance. People with BDD are highly likely to seek cosmetic dental treatment. However, the provision of such treatment is contra-indicated. This article will identify simple techniques for practitioners to screen for and manage patients with BDD. CPD/Clinical Relevance: This article addresses the management of patients with a specific psychiatric illness who are likely to present for cosmetic dental treatments.

Aim: The aim of this report is to describe the management of a prosthodontic patient expressing unrealistic expectations with respect to the transition to edentulousness. Objectives: To outline (1) the diagnosis and explicit expectations of the patient on presentation (2) considerations made during treatment planning to address the wishes of the first time prosthodontic patient (3) a sequential treatment plan utilizing transitional partial removable dentures to manage the change to edentulousness (4) functional and aesthetic result achieved. Results: Delivery of immediate removable partial dentures retaining key abutment teeth in upper and lower arches was a viable prosthodontic solution in the transition to edentulousness of a patient expecting unrealistic treatment outcomes. Conclusions: Addressing impractical expectations and devising a treatment plan amenable to both clinician and patient is difficult. Strategies to manage these wishes in prosthodontic dentistry can include transitional partial dentures. Clinical relevance: Practitioners who encounter similar situations may consider this report valuable.

A total of 126 participants were asked to select, from a series of photographs, the dentist which they perceived as: most caring, most competent, and which they would wish to have as their own dentist. White European male dentists were most likely to be chosen as the respondents' general dental practitioner. Men were generally perceived as more competent than women. Women were more likely to be chosen as the most caring dentists. It is concluded that, in the absence of other information, sex and ethnicity of the dentist exert an influence on patients' perceptions of general dental practitioners.

The final article in our series on orthodontic conundrums considers the extraction of a single lower incisor as part of a wider orthodontic treatment plan. This is a relatively uncommon approach; however, when used in the appropriate clinical scenario, it can be highly effective. Extraction of a lower incisor may be required to provide space for alignment of the teeth and serve as a pragmatic way to facilitate treatment while preserving posterior units. This article explores some of the possible orthodontic indications and contraindications for extraction of a lower incisor. CPD/Clinical Relevance: In certain clinical situations, extraction of a lower incisor has significant advantages. Dental professionals should be aware of this extraction pattern and its limitations.
