AbstractInjuries to the biliary tree during surgical, endoscopic or invasive radiological diagnostic or therapeutic procedures involving the pancreas, liver or organs of the upper gastrointestinal tract give rise to the need to develop a method for clear discrimination of biliary anatomy from surrounding tissue. Hyperspectral imaging (HSI) is an emerging optical technique in disease diagnosis and image-guided surgery with inherent advantages of being a non-contact, non-invasive, and non-ionizing technique. HSI can produce quantitative diagnostic information about tissue pathology, morphology, and chemical composition. HSI was applied in human liver transplantation and compared to porcine model operations to assess the capability of discriminating biliary anatomy from surrounding biological tissue. Absorbance spectra measured from bile ducts, gall bladder, and liver show a dependence on tissue composition and bile concentration, with agreement between human and porcine datasets. The bile pigment biliverdin and structural proteins collagen and elastin were identified as contributors to the bile duct and gall bladder absorbance spectra.

Anatomic anomalies of the exrrahepatic biliary tree are common and often incidental findings at endoscopic retrograde cholangiopancreatography; however, they rarely complicate therapeutic procedures such as stone extraction. The cases of two patients with biliary obstruction due to stones are presented, who were found to have the cystic duct a rising almost directly from the ampulla. This rare anatomic variant posed problems both in terms of interpretation of the cholangiogram and removal of stones. Therapeutic endoscopists should be aware of this and other anomalies of the biliary tract and of the potential problems faced in both interpretation of the cholangiogram and in therapeutic procedures.

Understanding gallbladder and biliary anatomy is paramount to the surgeon. A comprehensive understanding of the gallbladder and biliary tree is necessary to properly treat a variety of surgical pathologies. Understanding the usual anatomy and the variations will help prevent iatrogenic biliary injuries. Moreover, anatomic consideration dictates oncologic therapies for gallbladder and biliary tract malignancies.  Key words: bile duct, bile salts, biliary tree, cholecystectomy, gallbladder

Purpose The work was aimed at presenting the indications, techniques and results of the percutaneous transjejunal approach to the biliary tree in patients with hepatobiliary complications due to surgery. Patients and methods Ten patients, 7 males and 3 females, mean age 50 years (range, 10–62) with hepatico-jejunostomy, who developed cholangitis together with jaundice or bile leakage, underwent this procedure, performed through the anastomotic loop that was not surgically anchored to the abdominal wall in all cases but one. The transjejunal approach was chosen because of non-dilated bile ducts in 3 patients, complex pathologic situations in 5 patients and to avoid complications to a transplanted liver in 2 patients. The jejunal loop was identified using CT, US and fluoroscopy in 4 patients and after its opacification in the remaining 6 (by percutaneous transhepatic or intravenous cholangiography or fistulography). Results The procedure was technically and diagnostically successful in all cases. Therapeutic procedures (stenting, dilation, litholysis) were also performed using the transjejunal approach in 7 patients and in 6 of them complete pathological resolution was achieved. There were no complications. Conclusions Different pathologies of the biliary tree, in patients with bilio-enteric anastomoses, have been identified and treated by this technique; they were fistulas, anastomotic and/or multiple segmental benign or malignant stenoses of the bile duct, and diffuse intrahepatic lithiasis. The procedure was safe and reliable.

The purpose of this study was to comparative histomorphological investigation of the non affected and affected bile duct and gall bladder by fascioliasis in Black Bengal goat. The average weight of affected gall bladder was 26.10±0.70 gm which was significantly (p<0.001) higher than non affected gall bladder (19.40±0.96 gm). The average length and girth of affected gall bladder were 10.30±0.37 cm and 8.24±0.30 cm, respectively which were also significantly (p<0.001) higher than the length (6.10±0.30 cm) and Girth (5.85±0.25 cm) of non-affected gall bladder of Black Bengal Goat. The gross changes in acute form, thickening of the bile ducts and fibrosis in a portal area due to chronic fascioliasis was found in case of affected bile duct but were not found in case of non affected liver. A brownish exudates and a number of mature Fasciola gigantica were found in the lumen. The adult Fasciola gigantica was noticed in cross section in the lumen of the thickened bile ducts. Acute pathological lesions could only be produced by developing flukes prior to their entry to the bile ducts. Microscopically the epithelial layer of the bile ducts were seen to the partially disintegrated, but simultaneous proliferation of epithelial cells occurred. Thickening of the bile ducts was the result of connective tissue proliferation. Deposition of bile pigment in the tissue space and bile duct in some parts showed periductal cellular infiltrations, mainly neutrophiles, lymphocyte and eosinophiles. No calcification in the wall of the bile ducts in chronic Fascioliasis in goat could be seen in this study. The gall bladder was very dark usually contained blood clots, the consistency of bile was also very dense. Microcopically hyperplasia of the tubuloalveolar glands and numerous eggs were seen in the bile of infected goat which were absence in case of non infected goat. Asian J. Med. Biol. Res. A novel system of implementing target identification with hyperspectral imaging system based on acousto-optic tunable filter (AOTF) was proposed. The system consists of lens, AOTF, AOTF driver, CCD and image collection installation. Owing to the high spatial and spectral resolution, the system can operate in the spectral range from visible light to near infrared band. An experiment of detecting and recognizing of two different kinds of camouflage armets from background was presented. When the characteristic spectral wave bands are 680nm and 750nm, the two camouflage armets exhibit different spectral characteristic. The target camouflage armets in the hyperspectral images are distinct from background and the contrast of armets and background is increased. The image fusion, target segmentation and pick-up of those images with especial spectral characteristics were realized by the Hyperspectral Imaging System. The 600nm, 680nm, and 750nm images were processed by the Pseudo color fusion algorithm, thus the camouflage armets are more easily observed by naked eyes. Experimental results confirm that AOTF hyperspectral imaging system can acquire image of high contrast, and has the ability of detecting and identification camouflage objects.

Fine art photography, paper documents, and other parts of printing that aim to keep value are searching for credible techniques and mediums suitable for long-term archiving purposes. In general, long-lasting pigment-based inks are used for archival print creation. However, they are very often replaced or forged by dye-based inks, with lower fade resistance and, therefore, lower archiving potential. Frequently, the difference between the dye- and pigment-based prints is hard to uncover. Finding a simple tool for countrified identification is, therefore, necessary. This paper assesses the spectral characteristics of dye- and pigment-based ink prints using visible near-infrared (VNIR) hyperspectral imaging. The main aim is to show the spectral differences between these ink prints using a hyperspectral camera and subsequent hyperspectral image processing. Two diverse printers were exploited for comparison, a hobby dye-based EPSON L1800 and a professional pigment-based EPSON SC-P9500. The identical prints created via these printers on three different types of photo paper were recaptured by the hyperspectral camera. The acquired pixel values were studied in terms of spectral characteristics and principal component analysis (PCA). In addition, the obtained spectral differences were quantified by the selected spectral metrics. The possible usage for print forgery detection via VNIR hyperspectral imaging is discussed in the results.

Surgical treatment of pathologies of the gall bladder or biliary tree may involve the removal of stones and occlusions from biliary ducts, opening up of the ducts where they have become narrowed, or removal of the gall bladder itself if required. These procedures are often performed under a minimally invasive approach to reduce the risks of infection and scarring, and as such can require imaging guidance. This chapter covers a selection of hepatobiliary procedures that require the demonstration of the biliary vessels and gall bladder, including laparoscopic cholangiogram and endoscopic retrograde cholangiopancreatogram. Each procedure includes images that demonstrate the position of the C-arm, patient, and surgical equipment, with accompanying radiographs demonstrating the resulting images.

The most common cause of gallbladder perforation is calculous cholecystitis. Rarer causes include trauma, iatrogenic injuries, biliary stasis and gall bladder ischemia. We report a case of gall bladder gangrene with perforation, secondary to extensive ascariasis. A 45-year-old woman presented with acute intestinal obstruction and jaundice. She had abdominal distension and right hypochondrial tenderness. Abdominal radiography showed dilated bowel loops and ultrasonogram showed worms in the small intestine and biliary tree. On exploration, a bolus of worms 2 feet proximal to the ileocaecal junction was found causing obstruction. Worms were also present in the bile duct and gallbladder causing gangrene and perforation. She underwent cholecystectomy, bile-duct exploration and enterotomy. However, she died on the third postoperative day of overwhelming sepsis. Enteric complications of ascaris leading to bowel obstruction are well-known. Hepatobiliary complications such as cholangitis and obstructive jaundice are rare. However, such an extreme degree of infestation leading to gangrene and perforation of the gall bladder is extremely rare.
