Abstract Purpose: Breast cancer patients scheduled for postoperative radiotherapy undergo radiotherapy-planning computed tomography (CT), and incidental findings (IFs) may appear. This study investigated the interobserver variability between radiologists and oncologists when assessing IFs on radiotherapy-planning CT scans in breast cancer patients prior to adjuvant radiotherapy. Methods: We included 383 breast cancer patients who underwent planning CT at the Aalborg University Hospital between February 1, 2017 and February 28, 2018. IFs noted by the oncologists were identified from medical records. Two specialized radiologists reviewed the scans and described their IFs. IFs were classified as benign or potential malignant lesions. Cohen’s kappa statistic was used to measure interobserver agreement.Results: A total of 513 IFs were registered. The radiologists registered 433 findings, and the oncologists noted 80 (1.1 and 0.2 IFs per patient, respectively). Most potential malignant IFs were found in the liver, lungs, bones, and lymph nodes. The radiologists and oncologists detected potential malignant lesions in 94 (25%) and 34 (9%) patients, respectively. The oncologists’ sensitivity for detecting IFs in the liver and lungs were 29% and 20%, respectively. The agreements on IFs in the liver and lungs were fair (Cohen’s kappa values of 0.33 and 0.28, respectively).Conclusion: Radiologists reported a significantly higher frequency of IFs and potential malignant lesions than oncologists. Additionally, the oncologists had a low sensitivity when reporting IFs in both the liver and lungs. These results emphasize the need for specialized radiologists to scrutinize planning CT scans of breast cancer patients to ensure the intention to treat.

BACKGROUND Incidental findings could be observed in organs close to the spine while reporting lumbosacral spine magnetic resonance imaging (MRI). This study aimed to report the prevalence and clinical potential of extraspinal incidental findings in lumbosacral MRI of patients with suspected disc diseases. METHODS This single-centered cross-sectional study was carried out on 420 consecutive adult patients who underwent lumbar spine MRI for suspected disc disease from January to July 2019. The MRI was checked for the presence of extraspinal incidental findings, and each finding was categorized according to the body organ and its clinical significance. Each MRI plane that best displayed the findings was recorded, and the association between the findings and patient's age and sex was determined. RESULTS Of 420 samples, 135 cases showed extraspinal findings (32.1%), and 7.6% of the patients displayed suspicious lesions. The urinary tract was the most common system (18.6%) to display both clinically significant (5.0%) and benign findings (13.6%), and the axial MRI section was the plane which showed most of the incidental findings. Incidental findings in any body system were rarely found in the younger patients. Females had significantly higher benign findings than males (p = 0.002). CONCLUSIONS The prevalence of extraspinal findings in lumbosacral spine MRI is high, and some are significant. Most findings are related to the urinary tract and best displayed in the axial plane.

Two cases of Sporadic Burkitt’s lymphoma in children aged 11 and 8 years with primary symptoms in the oral cavity are reported. The first symptoms of the disease appeared in the oral cavity and were initially misdiagnosed as an inflammatory condition in one case and incidental findings not associated with the primary reason for visiting the dentist’s office in the second case. Biopsies of the lesions revealed the actual cause of the observed changes and contributed to the prompt initiation of polychemotherapy treatment. A review of current literature presents the known symptoms of Burkitt’s Lymphoma in the oral cavity and the available diagnostic methods. The role of the primary care physicians—the pedodontist and paediatrician—in the diagnostic and therapeutic process is discussed.
