I was born and raised in a medical family surrounded by physicians of various specialities and their friends. Although I dreamed of becoming a conductor of a symphony orchestra, my destiny was quite predetermined (genetically as well as epigenetically). So, I became a cardiologist. Right at the beginning of my career at the university hospital, I was told by senior physicians that to become a good medical professional you must read one clinical case report per day, at least. Indeed, it is not easy to see diverse patients every day and keep reading the medical literature. Eventually, I realized that following this rule I could easily come up with an initial diagnosis and selection of the right therapy for my patients. Clearly, clinical case reports remain an essential part of lifelong learning in medicine, contributing greatly to the progress of health care. That is why ScienceOpen aggregated extensive content from the medical literature published Open Access in the last 5-7 years and offers this invaluable material freely for every health care professional in the world. Another important aspect of continuing medical education involves the role of mentoring. Mentorship during the medical career can have an exceptional impact both professionally as well as personally. Supporting self-esteem and the importance of lifelong learning, a good mentor empowers a positive attitude and promotes the development of more open and flexible mindsets in medical trainees.  As a great example, I would like to mention a weekly clinical case reports discussion called clinical journal club presented by Professor Friedrich C. Luft, Director of the Experimental and Clinical Research Center (ECRC) at Charité and Max Delbrück Center in the Helmholtz Association in Berlin to his colleagues, physicians and researchers in a freely available open format: http://www.charite-buch.de/. During my own training as a physician-scientist, I met a few outstanding senior colleagues, unique leaders and incredible personalities such as Professor Mirsaid M. Mirrakhimov, Professor Rainer Dietz, Professor Detlev Ganten, and Professor Friedrich C. Luft. Now, Professor Friedrich C. Luft and I decided to initiate a special Open Access collection of clinical case reports across all medical disciplines. We compared the characteristics of the most cited, most downloaded, and most mentioned (the highest Altmetric Attention Score) articles published in general medical journals. We identified the 640 most frequently cited, 662 most frequently downloaded, and 652 most mentioned articles from 48 general medical journals. A comparison was made of the following characteristics of articles in the most cited, most downloaded, and most mentioned articles: medical specialty, publication type, country of origin, year of publication, and accessibility. There was only a 2.5% overlap in these three groups. Original articles were the more frequent among the most mentioned articles, whereas reviews, case reports, and guidelines/consensus statements were more frequent among the most downloaded articles. The most cited articles were more frequently published in 2010 and before, whereas the most downloaded articles were published in 2017−2018. The most mentioned articles were more frequently open-access articles, compared to the most downloaded articles. The most cited were more frequently older, the most downloaded were more frequently recent and educational, and the most mentioned were more frequently original and open-access articles. The results of our study may provide insights into various measures of article impact.

The Faculty of Health at Universidad del Valle determined the conformation, functions, and competencies of the Scientific and Editorial Committees of the Colombia Médica journal (Resolution 099, July 07, 2009 HCF), permitting the naming of the Committees for the Journal and the opening of the internal call to name the Editor-in-Chief and the Associate Editors. Internal regulations institutionally define the types of publications of greatest interest, the frequency of each publication, and types of articles in regular and supplementary numbers, internal operations aspects; define stimuli for excellence in articles published with an award and editorial sanctions for plagiarism of intellectual property and redundant publication. In 1995, Colombia Médica adopted the web publication, now in 2010, fifteen years later, we have adopted we have adopted an agile and secure electronic management and publication system with open access that will permit editors, peers, and authors to work together from anywhere in the world, it will allow greater efficiency in real time with significant reduction of administrative costs. This system is currently used by 235 journals in the world, achieving more expeditious indexation of their articles, which is attractive for local researchers and for the international scientific community. For our Journal, this open electronic system is a true technological leap. The visibility of Colombia Médica has been traditionally good, given that it is indexed in 23 international data bases including the Institute for Scientific Information-ISI Thomson web of knowledge, and because it is a free journal that is easily accessed via the web. International Observatories have reported an increase in the number of downloads of complete articles by three times in the last year, which we feel is correlated to the four-fold increase of complete articles published in English. These facts, along with growing publication requests by authors from five continents are a stimulus for the Journal to publish a greater number of complete articles in English. The number of electronic downloads reported by our counters and by international observatories quickly bring us closer to the average for journals published by ELSEVIER, recognized for their highest standards of scientific visibility and quality in the world. Due to this we are calling for an exchange of ideas and experiences at our home, Universidad del Valle, during a national meeting of editors of health sciences journals, which undoubtedly permit our journals fulfill their goals. The first two numbers for this year report articles of interest for public health, like the critical analysis of the effectiveness of health promotion in Latin America, health services for adolescents, and sexual health for adolescents. The articles also report research studies in basic sciences, qualitative research, report of the Colombian Muisca Mummy (S010-IX mummy), inflammatory response in children with severe malnutrition, population genetic studies, case reports, and brief communications. We are convinced that with the new open editorial system, the growing international presence of our publications, and reports on themes of greater pertinence in different areas of health we are set to compete in the difficult and demanding world of scientific disclosure.
