Management of a Palatal Gingival Groove in a Maxillary Lateral Incisor: A Case Report

The palate-gingival grooves are an anatomical anomaly in maxillary incisors, which might result in local periodontal pocket formation, bone loss and sometimes pulpal necrosis. In the present case, there was a groove on the palatal side of a maxillary lateral incisor with vital pulp. The periodontal problem of the tooth was treated and the 6-month postoperative follow-up showed attachment gain and decreased pocket depth.

Adenomatoid odontogenic tumor (AOT), a benign (hamartomatous) lesion of odontogenic origin, is an uncommon tumor which affects mainly females in the second decade. This lesion is most commonly associated with an impacted maxillary canine. This paper reported a case of AOT, in a 16-year-old female, associated with an impacted maxillary left lateral incisor. The evolution of this tumor was followed over 36 months and 24 months after excision.

Palatogingival grooves (PGG) are developmental malformations infamous for precipitating endodontic-periodontal lesions. Pertaining to their discreet occurrence, variation in groove depth and funnel like shape, adherence of plaque and calculus is apparent thereby jeopardizing the periodontium and pulp. A case of palatogingival groove in maxillary left lateral incisor was diagnosed and managed with endodontic treatment followed by open flap debridement to seal the groove with biodentine and fill the defect with bone graft. On follow up, the periapical lesion and the periodontal pocket were successfully resolved.

ABSTRACT Synodontia or fusion is union of two independently developing primary or secondary teeth. In the case reported here, clinical and radiographic examination suggested fusion between maxillary lateral incisor and a supernumerary tooth with the resultant loss of eruption space for permanent canine. Since the teeth exhibited separate pulp chambers without any pulpal involvement, surgical separation followed by odontoplasty was done. Follow-up revealed the tooth to be asymptomatic and sufficient space for eruption of canine was created. The case report highlights the surgical management in case of fusion and timely intervention to prevent periodontal, endodontic and orthodontic complications.

ABSTRACT This case report highlights the effective management of a maxillary lateral incisor with perio-endo lesion precipitated and complicated by the presence of a deep palatoradicular groove extending up to the root apex. Despite an apparently poor prognosis, the tooth was successfully managed by a collaborative endodontic and surgical periodontal therapy. The periodontal ligament attachment and periradicular healing were appreciated both clinically and radiographically at 3-month follow-up. This case report showed an AOT in a 12-year-old female patient referred for orthodontic-surgical of both impacted right mandibular canine and lateral incisor. Cone beam computed tomography revealed a well-defined mixed hyperdense/hypodense lesion, involving the crown of the mandibular lateral incisor. The surgery consisted in surgical exposure of the mandibular right canine and lateral incisor, bonding of the lateral incisor for orthodontic traction and curettage of the mandibular lesion. Histopathological examination revealed several columnar epithelial cells with minimal stromal connective tissue, lobular pattern and rosettes and duct-like structures, confirming the diagnosis of AOT. After, the patient was referred for orthodontic traction of the impacted teeth. At 1 and 3-year postoperatively, follow-ups examinations showed extensive bone repair, resolution of the tooth-retention and absence of recurrence. Although AOT is an uncommon lesion in the mandible, it should be considered in the differential diagnosis of the mixed profile lesions in this region.
