For more than 70 years, the inhibitors of carbonic anhydrase (ICA) have been used in the treatment of glaucoma. Since 1995, topical forms of ICA have appeared among antiglaucoma drugs — dorzolamide, and since 1999 — brinzolamide. The hypotensive efficacy and safety of topical ICA gave rise to the widespread use of these drugs in the treatment of various forms of glaucoma. The peculiarities of the mechanism of action made it possible to create on their basis effective fixed combinations, which allow reducing the intraocular pressure by up to 40 % in patients with glaucoma. The attractiveness of the drug has expanded due to the effect of ICA on various tissues of the organ of vision. Thus, ICAs effectively improve the hemodynamics of the eye and optic nerve. The antioxidant activity of the drug makes it possible to expand the indications for its use in patients with vascular patho-logy of the retina and optic nerve. The biomechanical characte-ristics of the drug allowed its effective and safe use in the complex treatment of macular edema of various etiologies. The adjuvant use of topical ICAs expands the possibilities of anti-VEGF therapy in the treatment of macular edema of various etiologies. Features of the action of ICA on the endothelial pump function of the cornea made it possible to formulate the rules for the use of topical ICA in patients with an insufficient function of the corneal endothelium.

In patients with uveitis associated with systemic diseases, a common cause of low vision is macular edema, which develops at any localization of the inflammatory process in 38–84% of cases. Autologous platelet-rich plasma is widely used in various branches of medicine, however, in the treatment of uveitis accompanied by macular edema, the clinical efficacy of its use has not been sufficiently studied. Purpose. To conduct a clinical and laboratory assessment of the effectiveness of complex treatment of uveitis with macular edema associated with systemic diseases using autologous platelet-rich plasma. Material and methods. The study included 120 people (120 eyes) from 18 to 50 years old with uveitis and macular edema associated with systemic diseases: 51 men, 69 women. Patients were divided into 2 groups: the main group consisted of patients who received anti-inflammatory treatment and administration of autologous plasma platelet-rich; comparison group – patients who received anti-inflammatory treatment. All patients underwent a comprehensive ophthalmological examination. Results. It has been established that the administration of autologous platelet-rich plasma against the background of anti-inflammatory treatment in patients with uveitis associated with systemic diseases accompanied by macular edema improves the maximally corrected visual acuity, a statistically significant decrease in the thickness of the retina in the fovea and an increase in the light sensitivity of the retina, relative to the data obtained in the comparison group. Conclusion. The use of autologous platelet-rich plasma in combination with anti-inflammatory treatment leads to a decrease in the incidence of recurrence of uveitis associated with systemic diseases, helps to reduce macular edema and improve functional parameters. A new method of treatment of endogenous uveitis have been developed. The method is based on the application of metabolic plasmapheresis with ozonization of cellular weight. Comparative analysis of the treatment results of three groups of 179 patients (209 eyes) has revealed significant superiority of original modification of plasmapheresis. The effect of the presented method of treatment, in authors’ opinion, is caused by immunomodulating effect of plasmapheresis and ozonotherapy, bactericidal and antiviral effects of ozonotherapy. The paper also presents some sanogenetic mechanisms of a new method of treatment – a liquidation of T-cellular immunodeficiency, a decrease of level of circulating immune complexes and some others.

Background. A common cause of visual impairment in patients with non-infectious uveitis is macular edema, developing in 38–84 % of cases. Plasma enriched with platelets is widely used in various branches of medicine, the effectiveness of its use in the treatment of non-infectious uveitis, accompanied by macular edema, has not been sufficiently investigated. Aim: To evaluate the effectiveness of autologous platelet-rich plasma in the complex treatment of non-infectious uveitis accompanied by macular edema.Material and methods. The study was conducted on the basis of the academician S.N. Fyodorov Eye Microsurgery Federal State Institution in the period from 2016 to 2018, which included 123 people (176 eyes) from 18 to 50 years with non-infectious uveitis, accompanied by macular edema: 46 men, 77 women. Patients were divided into 2 groups. The main group consisted of patients receiving autologous platelet-rich plasma and anti-inflammatory treatment; the comparison group consisted of patients receiving anti-inflammatory treatment. The results of visual acuity, intraocular pressure, biomicroophthalmoscopy, optical coherence tomography of the macular zone, microperimetry, ultrasound examination on the side of the affected eye were evaluated. Statistical processing of the data was carried out in the program Statistica 10.Results. Maintenance of autologous platelet-rich plasma contributes to a statistically significant improvement in visual acuity on the 10th day of treatment by 64.2 %, a decrease in the thickness of the retina in fovea by 36.3 % and an increase in retinal photosensitivity by 34.6 % compared to the group of patients receiving only anti-inflammatory treatment. Conclusions. The use of autologous platelet-rich plasma in the complex treatment of non-infectious uveitis allows to accelerate the natural mechanisms of tissue regeneration, contributing to the reduction of macular edema, and improve visual performance.
