Wrongful Life and Wrongful Birth: New Concepts for the Pediatrician

Over the past two decades, numerous suits for damages have been brought against physicians for the injury of wrongful life, or wrongful birth. Within the past 5 years, several precedents have been set that broaden the physician's legal obligation to recognize and act upon foreseeable or potentially recurrent genetic, teratogenic, or chromosomal disorders. These precedents may be expected to affect all physicians, but particularly pediatricians, because of the increased frequency of such disorders in the pediatric population.

Aim. Study of genetic pathology burden, both monogenic and chromosomal, of pediatric population of Krasnohrad district of Kharkiv region. Methods. Epidemiological and clinical-genealogical methods have been used. The information on 252 patients aged 0– 17 years from all major sources of their registration has been analyzed. Results. The monogenic pathology burden of pediatric population was 0.28 %, chromosomal disorders burden — 0.13 %, 0.41 % in total. There were 14 nosological forms of single-gene diseases, the most common ones were sensorineural hearing loss (1:1295), congenital glaucoma (1:3884), congenital hypothyroidism (1:3884), ichthyosis (1:3884). For the past ten years there was a change in the spectrum of single-gene pathology in pediatric population of Krasnohrad district. Chromosomal abnormalities in the area were represented by Down syndrome (1:868) and Prader-Willi syndrome (1:7767). Conclusions. The genetic pathology burden of the pediatric population of the Krasnohrad district of Kharkiv region, the spectrum of monogenic and chromosomal pathology, and prevalence of their individual nosological forms are comparable to those in most European countries. The Past ◽  

Incipient signs of a change in Zeitgeist are being felt. Under a new Zeitgeist, some theories which have been excluded, ignored or suppressed by the past dominant mainstream theories may emerge into the daylight. This article examines processes of popularization of theories and discusses how they may relate to emergence of hitherto excluded, ignored or suppressed theories. Two new concepts are introduced in this article: metaphorizability and anchorability.

As is readily apparent, the problem of reducing radiation exposure to the pediatric patient encompasses many areas. In the past, more attention has been devoted to the concrete aspects such as refined radiographic equipment and gonadal shielding. Less attention has been devoted to the abstract aspects which involve the general philosophy and disposition of the referring physician and his radiologic consultant. In this regard, it cannot be overstated that close communication with the radiologist is mandatory. The (pediatric) radiologist is constantly aware of the aspects of radiation safety and utilizes this information when he obtains radiographic examinations. There is a tendency to treat radiographic examinations in the same context as clinical and chemical laboratory investigations. Perhaps this attitude stems from the fact that radiographs are often ordered at the same time as these laboratory tests. Such an orientation is dangerous, and it behooves the referring physician and the radiologist to work in cooperation to remedy this situation. Indeed, it is simple to improve radiographic equipment and not too difficult to shield the child; but, it is difficult to formulate a proper attitude regarding radiographic examinations and their potential hazard to the pediatric population. The Committee on Radiology plans to prepare additional recommendations on radiographic examinations to provide more detailed and technical information which should be helpful to pediatricians and other physicians providing care for children.

Pediatric integrative medicine (PIM) is of significant interest to patients, with 12% of the general pediatric population and up to 80% of children with chronic conditions using PIM approaches. The field of PIM has evolved over the past 25 years, approaching child health with a number of guiding principles: preventive, context-centered, relationship-based, personalized, participatory, and ecologically sustainable. This manuscript reviews important time points for the field of PIM and reports on a series of meetings of PIM leaders, aimed at assessing the state of the field and planning for its future. Efforts in the first decade of the 2000s led to increased visibility in academic and professional pediatric organizations and through international listservs, designed to link those interested in and practicing PIM, all of which continue to flourish. The PIM leadership summits in recent years resulted in specific goals to advance PIM further in the following key areas: research, clinical practice, professional education, patient and family education, and advocacy and partnerships. Additionally, goals were developed for greater expansion of PIM professional education, broader support for pediatric PIM research, and an expanded role for PIM approaches in the provision of pediatric care.

For the past couple of decades the Latin Americans, like their brethren in Africa and Asia, have been hell-bent in search of ‘development’ or ‘modernization’. While the Latin Americans were on the firing line, scholars and policy-makers in both the rich nations and the poor nations were involved in setting out an intellectual framework for analyzing the developmental process. New concepts to explain the meaning of development were devised; innovative measurements to gauge the level of development were proposed; a new vocabulary to capture the nuances of development was put forth.

Deepwater riser selection is a complex evaluation of technical and commercial project drivers. The free standing hybrid riser (FSHR) has evolved in the last 10 years through major use in West Africa and is now gaining serious consideration in other deepwater provinces. The key benefit of the free standing riser is that the steel riser vertical section is offset from the vessel using flexible jumpers, thereby decoupling the riser from vessel dynamic motion. Early FSHR configuration took the hybrid bundle tower form. The very first free standing riser system, installed in 1988, consisted of the Placid hybrid bundle in the Gulf of Mexico. In the late nineties, a hybrid bundle tower was chosen for the Girassol development in West Africa. Since then, the industry has sanctioned numerous developments using multiple single line freestanding risers. Optimization of the FSHR is continuing with new concepts such as the Grouped SLOR developed to offer the combined benefits of both the bundle and single line multiple arrangements. This paper will describe how the FSHR configuration has evolved to meet increasing industry demands over the past 10 years and will discuss the future of this type of riser system. Increasing applications in ultra deepwater regions, hurricane prone locations and tiebacks to existing payload limited production vessels will be discussed with riser system architecture described including interfaces with the vessel and seabed.
