Cognitive-Behavioral Pain Management in Children With Juvenile Rheumatoid Arthritis

Decreasing chronic joint pain is a major goal in the management of juvenile rheumatoid arthritis. Cognitive-behavioral self-regulatory techniques were taught to children with juvenile rheumatoid arthritis to reduce musculoskeletal pain intensity and to facilitate better adaptive functioning. Subjects were 13 children between the ages of 4.5 and 16.9 years who had pauciarticular or systemic onset juvenile rheumatoid arthritis. Baseline data included an initial comprehensive assessment of pain, disease activity, and level of functional disability, as well as pain intensity ratings gathered over a 4-week period. Subjects were seen for eight individual sessions in which self-regulatory techniques (progressive muscle relaxation, guided imagery, meditative breathing) were taught, and parents were seen for two sessions in which key aspects of behavioral pain management techniques were reviewed. Results indicated that these techniques led to substantial reduction of pain intensity, which generalized to outside the clinic setting. Six- and 12-month follow-up data showed consistent decreases in pain as well as improved adaptive functioning. The data suggest that cognitive-behavioral interventions for pain are an effective adjunct to standard pharmacologic interventions for pain in patients with juvenile rheumatoid arthritis.

Penyakit fisik menimbulkan masalah psikososial ansietas dan gangguan citra tubuh. Masalah psikososial ini menjadi faktor comorbid yang menambah berat kondisi fisik klien. Pelaksanaan tindakan ners spesialis didasarkan pada konsep perilaku dan adaptasi sebagai mahluk yang holistik yang meliputi: Cognitive Behavioral Therapy (CBT), Thought Stopping (TS), Progressive Muscle Relaxation (PMR) dengan mempertimbangkan toleransi fisik klien. Tindakan ners spesialis Family PsychoEducation (FPE) pada keluarga dan Supportive Theraphy (ST) pada kelompok pelaku rawat (care giver) diberikan untuk memperkuat dukungan dari luar klien demi mempertahankan kelangsungan proses perawatan klien. Hasil: Pelaksanaan tindakan ners spesialis menunjukkan penurunan tanda dan gejala pada klien di semua aspek respons terhadap stresor. Penurunan yang paling besar terjadi pada kombinasi terapi CBT+PMR+FPE yang dilakukan pada klien dengan diagnosis ansietas pada aspek afektif sebesar 79.2% dan klien dengan diagnosis gangguan citra tubuh pada aspek perilaku sebesar 86.3%. Kesimpulan: Tindakan ners spesialis Cognitive Behavioral Therapy (CBT), Thought Stopping (TS), Progressive Muscle Relaxation (PMR) dengan mempertimbangkan toleransi fisik klien dan Family PsychoEducation (FPE) pada keluarga serta Supportive Theraphy (ST) pada kelompok pelaku rawat (care giver) dapat membantu penurunan respons klien terhadap stressor.Kata kunci: penyakit fisik; ansietas; gangguan citra tubuh; cognitive behavioral therapy; thought stopping; progressive muscle relaxation

One of the characteristics of physical changes in the elderly involves a decrease in the musculoskeletal system, such as the depletion of the synovial joint fluid which has an impact on the pain felt in joints. This condition can cause a disruption of the quality of life for the elderly and results in increases in the family burden. The purpose of cognitive-behavioral therapy in nurse care planning is to provide an overview of the implementation of family care nursing, community care health nursing, and management of health services by community health nurses to the elderly with osteoarthritis. The innovation and development of this senior citizen community program, initially named Lansia SMART (SMART Elderly), consists of six sessions, including psychoeducation sessions, cognitive restructuring sessions, pain management sessions with progressive muscle relaxation, activity scheduling sessions, problem-solving sessions, and tutorial sessions for the Lansia SMART (SMART Elderly) group with the Android application. The final maintaining challenging stage of these services involved 59 elderly participants. Data analysis used paired T-tests. The results of this innovation showed significant differences in the behavior of the elderly participants after the implementation of the SMART Elderly program (p < 0.05). The SMART Elderly program was effective to improve the elderly participants’ behavior. Nurses can use cognitive behavioral therapy to improve the process of self-management of osteoarthritis among their clients.

Purpose: Osteoarthritis is a progressive disease and the most common form of joint inflammation. Moreover, it is the most common cause of functional disability in the elderly. Among the multiple and predisposing factors influencing the disease are demographic indicators and occupational factors. The present study aimed to investigate the relationship between age, pain severity, Body Mass Index (BMI), occupation, and educational level, and the severity of functional disability in patients with Knee Osteoarthritis (KO). Methods: This descriptive study was performed on 97 KO patients referring to the Novin private physiotherapy clinic of Semnan University of Medical Sciences from April to March 2017. The study participants were selected through a simple nonprobability sampling technique. Literate individuals with the educational level of guidance school and above and diagnosed with KO were included in the study. Individuals with a history of inflammatory arthritis diseases, such as rheumatoid arthritis, soft tissue rheumatoid arthritis, fibromyalgia syndrome, bursitis, tendonitis, the neurological and vascular conditions of the lower extremity, mental problems, and malignancy were excluded from this research. The data related to the variables such as age, gender, occupation, history of osteoarthritis, pain intensity, involved side, educational level, and the Knee Injury and Osteoarthritis Outcome Score (KOOS) for knee disability were collected by a self-report questionnaire. This study was approved by the Research Ethics Committee of the University of Social Welfare and Rehabilitation Sciences. Using SPSS, Spearman’s correlation coefficients were calculated to examine the relationship between the study variables. Results: Spearman’s correlation coefficients revealed no significant correlation between the KOOS scores and age, occupation, and educational level. The obtained results suggested a poor significant association between KOOS and BMI; however, there was a strong significant correlation between KOOS and pain intensity Visual Analogue Scale (VAS) (P<0.05). Conclusion: The severity of functional disability in patients with KO based on KOOS questionnaire scores, was well correlated with pain severity, but poorly associated with age and BMI. The obtained data indicated no significant relationship between disability and occupation and educational level.

The variability of the disease may explain mis-conceptions that JRA is usually a benign disease. Acohort study in which 506 subjects during the periodof 1970-1999 found that approximately one-third ofJRA patients achieve disease remission. The visualcomplications are also important in determining theoutcome. Functional disability is common and can belong-lasting. Recognizing JRA symptoms earlier areimportant to prevent mortality, disability, and long-term complications. Although JRA is the most com-mon rheumatic disease in children, many doctors arenot familiar with this disease. The purpose of thispaper is to report a case of juvenile rheumatoid ar-thritis associated with uveitis in a 4-year, 8-month-old girl.
