ABSTRACT Introduction: the assessment and proper management of lower extremity soft tissue injury is very important. Because the mechanism and clinical manifestations of lower extremity soft tissue injury are varied, it is necessary to make an accurate assessment based on the specific situation of the patient. Objective: To explore the mechanism and rehabilitation methods of lower limb soft tissue sports injury. Methods: In clinical practice, external application of Chinese herbal medicine combined with cold compresses, massage, pot dipping, hot immersion irradiation and other methods were used. Results: The self-made Chinese herbal medicine external ointment, swelling and pain relieving Chinese herbal medicine wine combined with electromagnetic wave therapy instrument is one of the commonly used and effective methods to treat sports injury of lower extremity soft tissue in health care experimental centers. It has the most remarkable effect on local burning, stasis, swelling and pain caused by the soft tissue injury of various lower limbs in various parts of the human body. Conclusions: Timely functional rehabilitation training after sports injury can promote the functional healing and rehabilitation of the body after injury, maintaining the good functional state of each organ system of the body. Objective To establish the relationship between circumferential compression on the lower limb during simulated ramp and staircase profile loading, and the resultant relationship with discomfort/pain and tissue oxygenation. Background Excessive mechanical loading by exoskeletons on the body can lead to pressure-related soft tissue injury. Potential tissue damage is associated with objective oxygen deprivation and accompanied by subjective perception of pain and discomfort. Method Three widths of pneumatic cuffs were inflated at the dominant thigh and calf of healthy participants using two inflation patterns (ramp and staircase), using a computer-controlled pneumatic rig. Participants rated discomfort on an electronic visual analog scale and deep tissue oxygenation was monitored using near infrared spectroscopy. Results Circumferential compression with pneumatic cuffs triggered discomfort and pain at lower pressures at the thigh, with wider cuffs, and with a ramp inflation pattern. Staircase profile compression caused an increase in deep tissue oxygenation, whereas the ramp profile compression decreased it. Conclusion Discomfort and pain during circumferential compression at the lower limb is related to the width of pneumatic cuffs, the inflation pattern, and the volume of soft tissue at the assessment site. The occurrence of pain is also possibly related to the decrease in deep tissue oxygenation during compression. Application Our findings can be used to inform safe and comfortable design of soft exoskeletons to avoid discomfort and possible soft tissue injury.

Covid-19 is a pandemic infective disease, which has been erupting throughout the whole world from 2020 to 2021. There have been at the time of reviewing more than a hundred million cases (102,399,513) of infection, and more than 2 million deaths (2,217,005) in more than 200 countries; this information is taken from the Covid-19 daily situation report issued on 1st Feb 2021 by WHO. The pandemic was also declared as an outbreak of Public Health Emergency of International Concern on 30th Jan 2020 by WHO. No effective treatment model has yet been confirmed by conventional western medicine, but some herbal treatments used in China can be recognized as having positive results in Covid-19 cases. This pandemic disaster has been severely damaging to quality of life, disturbing social communication, economic development and the progress of humanity. Successful results were first reported from temporary hospitals (Fangcang Hospitals set up in arenas and exhibition spaces) in Wuhan, China, where more than 90% of patients treated with Chinese herbal medicine were prevented by transferring from the minor or milder stages of the disease to the severe or critical stage. In all of them the positive PCR became negative; herbs were involved in the rescue and treatment of severe and critical cases in ICU’s in hospitals as well. It was an important factor in how Covid-19 was controlled so quickly in China. The author recruited both confirmed and suspected Covid-19 patients through social media (WeChat, WhatsApp, Internet, message etc.) as a volunteer TCM consultant from March 2020, during the first Lockdown phase in UK. It was quickly realized that the appearance of the tongue provides special and significant information for identifying whether Covid-19 is present and shows its severity, from a TCM perspective, as the degree and type of dampness accumulated within the body. Herbal prescriptions were sent by post for patients’ treatment according to their clinical findings, virus exposure history and tongue information (from photos). Similar positive effects were found as in China, and treatment reports and results are summarized here. The author believes there are many herbal medicines that can be effective in controlling the Sars-Cov-2 virus, and that prescribing the appropriate formula to match each individual case is the key point for TCM control of Covid-19. Chinese herbal medicine is the most effective current treatment method that can prevent cases in the minor and ordinary stages from progressing to the severe or critical stage, and so can play a significant role in the decrease and cure of Covid-19.

Introduction: Soft tissue injuries are a common presenting complaint seen in the emergency department following trauma. However, internal degloving injuries are not commonly seen by the emergency provider. Case Report: A 57-year-old male presented with right lower extremity pain, bruising, and swelling after a low-speed bicycle accident five days prior. Physical examination revealed an edematous and ecchymotic right lower extremity extending from the mid-thigh distally. Computed tomography of the thigh demonstrated a hyperdense foci within the fluid collection suggesting internal hemorrhage and internal de-gloving suggestive of a Morel-Lavallée lesion. Discussion: The Morel-Lavallée lesion is a post-traumatic soft tissue injury that occurs as a result of shearing forces that create a potential space for the collection of blood, lymph, and fat. First described in 1853 by French physician Maurice Morel-Lavallée, this internal degloving injury can serve as a nidus of infection if not treated appropriately. Magnetic resonance imaging has become the diagnostic modality of choice due to its high resolution of soft tissue injuries. Treatment has been focused on either conservative management or surgical debridement after consultation with a surgeon. Conclusion: The emergency physician should consider Morel-Lavallée lesions in patients with a traumatic hematoma formation to avoid complications that come from delayed diagnosis.

The aim of this study was to evaluate and compare the efficacy and safety of serratiopeptidase and aceclofenac in reducing swelling and pain following soft tissue injury. This study included 100 patients with soft tissue injury to upper limb, lower limb or both. They were randomly divided into two groups of 50 each to receive serratiopeptidase and aceclofenac. Evaluation of efficacy was made using tape measurement (for swelling), and visual analogue scale (for pain) on day 0, week 1and week 2. Serratiopeptidase showed significant anti-inflammatory effect and mild analgesic effect. None of the patient was required to be put on another analgesic or any alteration in treatment. Aceclofenac showed superior analgesic effect as compared to serratiopeptidase. Mild to moderate adverse effects were reported. The most common adverse effect reported was dyspepsia. All were mild and did not require any alteration or discontinuation of treatment.
