The functional and aesthetic restoration of crown-root fractured anterior teeth with biological width involvement represents a challenge to the clinician, because it requires a multidisciplinary approach. The trans-operative fragment reattachment technique is a feasible and advantageous alternative since it makes it possible to reestablish the original aesthetics and function of the fractured tooth in a single appointment. This work presents the fundamentals of this technique, highlighting its advantages, indications and limitations. The applications of the surgical and restorative procedures are demonstrated by way of a case report.

The management of patients with traumatic injuries to their dentition poses a serious challenge in everyday general dental practice. For the rehabilitation of the complicated subgingival crown fracture of anterior teeth, multidisciplinary approach is often indicated. A combination of endodontic, orthodontic, periodontal and prosthodontic approach may be required. Orthodontic or periodontal intervention becomes an integral part for the exposure of the sound tooth structure of fractured anterior teeth with fracture line extending subgingivally. The aim of this paper is to discuss the immediate endodontic management followed by orthodontic extrusion of traumatized upper anterior teeth with fracture at the subgingival level. In order to expose the sound tooth structure for prosthodonticintervention, orthodontic extrusion was performed after endodontic treatment. To avoid extraction of the involved teeth, the multidisciplinary approach was adopted and finally the teeth were restored prosthodontically. The final result was esthetically pleasant and periodontically sound.

ABSTRACT Crown fractures of the anterior teeth are a common form of dental trauma mainly affecting children and adolescents. One of the options for managing crown fractures is the reattachment of the fractured fragment when the tooth fragment is available with minimal or no violation of the biological width. Reattachment of fractured fragments can provide good esthetics, as it maintains the tooth's original anatomic form, color, and surface texture. It also restores function and is a relatively simple procedure. This case report deals with the esthetic management of a crown-root fracture that was successfully treated with endodontic treatment followed by reattachment of fractured fragment with fiber post. A common type of fracture involving tooth is complicated crown root fracture of anterior teeth. The reattachment of the coronal fragment to the remaining tooth will provide better and long-lasting aesthetics, improved function and a positive psychological reaction. It is a simple and less difficult procedure if the original tooth fragments are retained after fracture. This paper reports about a case of complicated crown root fracture that was treated successfully using broken fragment adhesive reattachment and post placement.

Esthetic rehabilitations require a broad and thorough evaluation of all the elements that make up the smile such as lips, dental, and periodontal structures to plan the treatment to provide harmony to the whole set. Objective: To demonstrate through a case the association of Periodontics and Restorative Dentistry in the esthetic rehabilitation of anterior teeth replacing resin restorations by modifying the gingival architecture conservatively and harmonically. Case report: It is essential for a multidisciplinary approach to succeed in treatment. The replacement of previous restorations is an increasing demand in currently clinical practice, for many reasons, mainly due to dissatisfaction with the esthetics. It is observed that many of these patients involve not only dissatisfaction with the dental structure but also with the periodontal health, for this reason, it is necessary a previous adaptation of the gingival tissues for proper restorative rehabilitation. Conclusion: Ceramic laminates have proved to be a great choice for anterior teeth restorations, but they must be indicated in specific situations, to obtain excellent results as most conservative as possible.

Severe caries, attrition  and developemental anomalies  of anterior teeth lead to loss of anterior guidance. This in turn causes attrition of posterior teeth and loss of occlusal vertical dimension of  occlusion. To gain the space for esthetic rehabilitation in these cases is challenging task. The required space can be achieved by crown lengthening and increasing the vertical dimension of occlusion within physiologic limits. We hereby have presented a case report showing the treatment procedure of a patient with severly worn dentition in a simple and systematic multidisciplinary approach to improve the function as well as aesthetics, that also remains in harmony with the entire gnathostomatic system.

Background: Trauma to the anterior teeth is common in young children and in adolescents. Uncomplicated crown fracture to the permanent teeth has an intense effect not only on the patient’s esthetics, but also on speech and function. Aim: Performing an economical, a minimally invasive, time-saving, long lasting, functional and natural alike direct composite resin restorations in a single visit. Conclusion: As restoring a fractured tooth structure is a complex procedure, this technique can prove to be a simple, effective and appropriate technique that will fulfill all the requirements of dental practioner without requiring special skills in providing the patients with direct composite restorations. Clinical Significance: This technique is economical which requires less chair side time compared to indirect restorations.

Microstomia is a condition with abnormally small oral aperture which is associated with compromised aesthetics and function of stomatognathic system. In young children, the leading cause of microstomia is oral electrical or chemical burns. Management of microstomia requires multidisciplinary approach including surgery, physiotherapy and appliance therapy. Appliance therapy in form of microstomia prevention appliance plays a pivotal role in prevention of contraction of tissue during healing. Numerous appliances have been described in literature to prevent microstomia. However, decision to use a particular appliance require meticulous planning considering the efficacy and effectiveness of the appliances and various patient factors like age, dentition status and compliance. This paper presents an unusual clinical condition where a unique customized microstomia prevention appliance therapy was instituted to prevent microstomia in 2 year old female patient following lip surgery. This appliance was constructed easily and inexpensively, could be adjusted so that it was almost painlessly inserted, and was progressively adapted. It was convenient for use in young child with minimal compliance offering improved mouth opening and consequently functional outcomes.
