Esthetic rehabilitations require a broad and thorough evaluation of all the elements that make up the smile such as lips, dental, and periodontal structures to plan the treatment to provide harmony to the whole set. Objective: To demonstrate through a case the association of Periodontics and Restorative Dentistry in the esthetic rehabilitation of anterior teeth replacing resin restorations by modifying the gingival architecture conservatively and harmonically. Case report: It is essential for a multidisciplinary approach to succeed in treatment. The replacement of previous restorations is an increasing demand in currently clinical practice, for many reasons, mainly due to dissatisfaction with the esthetics. It is observed that many of these patients involve not only dissatisfaction with the dental structure but also with the periodontal health, for this reason, it is necessary a previous adaptation of the gingival tissues for proper restorative rehabilitation. Conclusion: Ceramic laminates have proved to be a great choice for anterior teeth restorations, but they must be indicated in specific situations, to obtain excellent results as most conservative as possible.

This case report describes a successful multidisciplinary approach used to improve the smile esthetics of a patient presenting with excessive gingival display, asymmetric gingival margins, and small upper anterior teeth and lower anterior teeth. The treatment combined esthetic crown lengthening, dental bleaching, and restorative dentistry using CAD/CAM veneer. The 6-month follow-up examination confirmed the stability of the modification and absence of adverse effects.

Keeping in mind the final result is the basis of any type of treatment, especially those in which the morphology, size and proportion of the anterior teeth will be changed. This is where a good treatment plan based on a diagnostic wax-up tested with a mock-up and approved by the patient becomes crucial. This case report aims to exemplify how to transfer the diagnostic information to the patient’s mouth and direct it not only to the restorative dentistry, but also to the surgeon when performing the crown length. Diagnostic waxing was performed by the laboratory technician, obeying anterior posterior incisal and gingival criteria and curvatures, which were transported to an aesthetic guide through mock up to mark the surgical points. Then, surgery to increase the clinical crown with bone access and after healing, direct venners in composite resin also guided by waxing. In conclusion, a workflow can be established using the wax-up / mock-up that serves as a guide for the periodontist in the approach to surgical lengthening of the crown and for the rehabilitator who uses it to produce changes in the shape of dental dimensions .

The functional and aesthetic restoration of crown-root fractured anterior teeth with biological width involvement represents a challenge to the clinician, because it requires a multidisciplinary approach. The trans-operative fragment reattachment technique is a feasible and advantageous alternative since it makes it possible to reestablish the original aesthetics and function of the fractured tooth in a single appointment. This work presents the fundamentals of this technique, highlighting its advantages, indications and limitations. The applications of the surgical and restorative procedures are demonstrated by way of a case report.

Severe caries, attrition  and developemental anomalies  of anterior teeth lead to loss of anterior guidance. This in turn causes attrition of posterior teeth and loss of occlusal vertical dimension of  occlusion. To gain the space for esthetic rehabilitation in these cases is challenging task. The required space can be achieved by crown lengthening and increasing the vertical dimension of occlusion within physiologic limits. We hereby have presented a case report showing the treatment procedure of a patient with severly worn dentition in a simple and systematic multidisciplinary approach to improve the function as well as aesthetics, that also remains in harmony with the entire gnathostomatic system.

ABSTRACT: Aesthetic Treatment of Four Maxillary Anterior Teeth with External Resorption. Esthetic aspect has become increasingly important in the modern restorative dentistry and synonymous with a natural, harmonious appearance based on esthetic principles. This case report aimed to inform the esthetically complex restorative treatment of four maxillary anterior teeth. A 17-year-old male had a fractured incisor following a traumatic incident. Four years earlier, the tooth was treated endodontically and restored with a crown. Seven days ago, the crown fell off. A clinical examination revealed that tooth 11 was horizontally fractured on the mid-third of the crown. Tooth 21 had been previously restored using composite, but it had a bad contour and color. There was also a diastema between left maxillary incisors. All teeth responded normally to percussion and palpation and had normal periodontal probing. A periapical radiograph showed apical resorption at teeth 11 with an apical rarefaction 3-4 mm in diameter. After complete examination and esthetic analysis, several treatments were undertaken. Tooth 11 was retreated using MTA to facilitate the apexification followed by indirect, customized composite post and porcelain-fused-to-metal crown. Tooth 21 and the diastema between the left maxillary incisors were closed using composite resin. Tooth 22 was also restored using composite resin to get the ideal width of the tooth. After 2 weeks, the treated teeth were asymptomatic, radiographic examination demonstrated apparent regeneration of periapicular tissue, and the teeth functioned normally. The teeth were successfully treated by endodontic therapy and restoration with crown and composite resin to restore esthetic problems.

This case report describes a patient with discolored and fractured composite resin restorations on the anterior teeth in whom substitution was indicated. After wax-up and mock-up, the composite was removed and replaced with minimally invasive ceramic laminates. An established and predictable protocol was performed using resin cement. Minimally invasive ceramic restorations are increasingly being used to replace composite restorations. This treatment improves the occlusal and periodontal aspects during the planning and restorative phases, such as anterior guides, and laterality can be restored easily with ceramic laminates. In addition, the surface smoothness and contour of ceramic restorations do not affect the health of the surrounding periodontal tissues. Here we present the outcome after 18 months of clinical follow-up in a patient in whom composite resin restorations in the anterior teeth were replaced with minimally invasive ceramic laminates.

Treatment of the impacted and retained teeth is a multidisciplinary approach involving close cooperation between periodontist and orthodontist. Clinical and radiographic examination leading subsequently to diagnosis, remain the most important prerequisites permitting appropriate treatment. Several surgical techniques are available to uncover impacted/retained tooth according to their position within the osseous and dental environment. Moreover, to access to the tooth and to bond an orthodontic anchorage, the surgical techniques used during the surgical exposure must preserve the periodontium integrity. These surgical techniques are based on tissue manipulations derived from periodontal plastic surgery, permitting to establish and maintain long-term periodontal health.

The management of patients with traumatic injuries to their dentition poses a serious challenge in everyday general dental practice. For the rehabilitation of the complicated subgingival crown fracture of anterior teeth, multidisciplinary approach is often indicated. A combination of endodontic, orthodontic, periodontal and prosthodontic approach may be required. Orthodontic or periodontal intervention becomes an integral part for the exposure of the sound tooth structure of fractured anterior teeth with fracture line extending subgingivally. The aim of this paper is to discuss the immediate endodontic management followed by orthodontic extrusion of traumatized upper anterior teeth with fracture at the subgingival level. In order to expose the sound tooth structure for prosthodonticintervention, orthodontic extrusion was performed after endodontic treatment. To avoid extraction of the involved teeth, the multidisciplinary approach was adopted and finally the teeth were restored prosthodontically. The final result was esthetically pleasant and periodontically sound.
