ABSTRACT Smile is an integral part of dentistry. Patients come with the prime aim of achieving esthetics. The dentist may preserve or alter the relationships of the visible components of the oral region to give the desired esthetic result. But, it is our responsibility to attain this result through a scientifically based and thoroughly planned treatment. The appearance of gingival tissue surrounding the teeth plays an important role in the esthetics of anterior region. Abnormities in symmetry and contour can significantly affect the harmonious appearance of natural or prosthetic dentition. There are a variety of cases of teeth wear, wherein interdisciplinary intervention is essential. Also the factor causing severe wear of natural teeth must be identified and eliminated, or reduced before attempting restorative treatment to improve long-term prognosis of restorative treatment. This article deals with a case of anterior teeth wear, stressing upon diagnosis, treatment planning and progression of treatment with regards to perio-prostho esthetics. Esthetics is an increasing concern for patients, particularly in anterior teeth. The current approaches to manage the esthetic demands include options such as prosthodontic rehabilitation, orthodontic treatment, and direct restorations. Direct restorative procedures for anterior teeth have become a reliable alternative, since current composite materials can mimic teeth’s esthetics and function perfectly. This article describes two clinical cases of anterior esthetic rehabilitations where the direct reshaping of the four maxillary incisors was performed using the buccolingual layering technique with the help of a silicone index. In Case 1, a diastema closure was also performed, whereas in Case 2, the replacement of old restorations was also carried out. It was possible to achieve excellent final esthetic results in both cases, recovering the smile symmetry, and both patients were satisfied. The literature supports the use of direct restorations in the anterior region with very good long-term prognosis. It is a minimally invasive approach that is reversible, repairable, cost-effective, and when combined with layering techniques and adequate restorative materials, it is possible to achieve good esthetic and functional outcomes. However, regular maintenance appointments are mandatory, and all cases should be carefully planned to ensure predictable and long-lasting results.

ABSTRACT Immediate esthetic rehabilitation is a common practice in contemporary restorative dentistry. Sometimes without a precise diagnosis and clinical planning, two or three clinical sessions have been sufficient to start and end an extensive all-ceramic oral rehabilitation. It is essential, however, for these treatments to be based on the biological, mechanical, and occlusal principles that have been established for decades, and not only on aesthetic considerations. In this case report, a 26 year-old bruxist patient with functional and aesthetic needs, was rehabilitated with ceramic restorations in the six maxillary anterior teeth, through an interdisciplinary approach. The diagnostic wax-up was essential to predict the outcome; guide the periodontal surgery; serve as a reference for dental preparations, and restore the dynamics of jaw movements. After the adhesive cementation, a protective occlusal splint for use during sleep was introduced. The authors concluded that careful diagnosis and planning was critical to outcome of this case, and that the quality and longevity of restorative treatment depended on the respect for biomechanical, occlusal, and aesthetic concepts.

Accidental tooth avulsion is a grievous injury and common among the children. Management of avulsed tooth within alveolar socket by reimplantation becomes a challenge for the clinician due to extraoral time and media of transportation. Although the long-term prognosis of reimplantation is poor, the time during which the tooth remain within the arch will guide the development of alveolar bone completely. Moreover, reimplantation will maintain anatomical, functional and esthetic rehabilitation of the patient. In this case report, we present a case of accidental avulsion where teeth were gently rinsed of any debris and placed in normal saline during the examination and preparation of the reimplantation sockets. The teeth were then reimplanted, functionally splinted followed by endodontical treatment. After 12 months follow-up the periodontal space was healed perfectly without any resorption or ankylosis.J Enam Med Col 2017; 7(3): 165-169

Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.

The aim of this case report is to present two cases where cone beam computed tomography (CBCT) was used for the diagnosis, treatment planning, and followup of large periapical lesions in relation to maxillary anterior teeth treated by endodontic surgery. Periapical disease may be detected sooner using CBCT, and their true size, extent, nature, and position can be assessed. It allows clinician to select the most relevant views of the area of interest resulting in improved detection of periapical lesions. CBCT scan may provide a better, more accurate, and faster method to differentially diagnose a solid (granuloma) from a fluid-filled lesion or cavity (cyst). In the present case report, endodontic treatment was performed for both the cases followed by endodontic surgery. Biopsy was done to establish the confirmatory histopathological diagnosis of the periapical lesions. Long-term assessment of the periapical healing following surgery was done in all the three dimensions using CBCT and was found to be more accurate than IOPA radiography. It was concluded that CBCT was a useful modality in making the diagnosis and treatment plan and assessing the outcome of endodontic surgery for large periapical lesions.

ABSTRACT All of us as clinicians at one time or another have to attend to dental emergencies involving severly fractured or even lost or avulsed anterior teeth. Treating such cases is often difficult and there is always a time factor involved. Loss of anterior teeth can affect a patient psychologically and socially. This can be minimised by an immediate replacement or replantation of the lost or avulsed teeth. Definitive treatment planning and consultation with specialists is seldom possible at the time of emergency treatment. Replantation of the avulsed tooth can restore esthetic appearance and occlusal function shortly after the injury. This article describes the management of a young female with an avulsed maxillary permanent incisor that had been air-dried for about 72 hours. The replanted incisor retained its esthetic appearance and functionality 8 months after replantation, yet the long-term prognosis remains questionable.

SUMMARY It was possible to restore the shape and function of a severely ankylosed tooth by fabricating a ceramic crown for placement on a resin reconstruction supported by mini fiberglass posts. By increasing the retention of the morphological reconstruction for the future support of indirect restorations, cementable intradentinal fiberglass posts enhance the longevity of these restorations. Ultimately, all proposals that seek to improve the long-term prognosis of restorations on ankylosed teeth, especially severely impacted ones, are of extreme clinical relevance.

Hypomineralized incisor molar syndrome (MIH) results from the action of various etiological factors during the enamel apposition and mineralization stages during dental development. Clinically, it results in changes in the shape, color and contour of the affected teeth, which compromises aesthetics and function. Through a clinical case, this study aimed to report the planning, with the aid of digital smile design (DSD), and the restorative treatment, with ceramic laminate ceramic veneers, of anterior teeth affected by MIH. To provide personalized treatment to the patient's needs and expectations, the use of DSD in case planning to a correct diagnosis, simulation and evaluation of aesthetic rehabilitation. It also favored communication between the patient and the multidisciplinary dental team, and this with the laboratory technician.   Keywords: Dental Enamel Hypoplasia. Dental Porcelain. Através de um caso clínico, este trabalho teve como objetivo relatar o planejamento, com auxílio do desenho digital do sorriso (DSD), e o tratamento restaurador, com facetas de cerâmicas de dentes anteriores afetados por MIH. Para proporcionar um tratamento personalizado às necessidades e expectativas da paciente, o DSD foi utilizado no planejamento do caso para o correto diagnóstico, simulação e avaliação da reabilitação estética. ABSTRACT The scope of orthodontics has widened to include not only children and adolescents but also adults, thereby abolishing the upper age limit. With growing esthetic conscience among the society, a great number of young adults are seeking orthodontic treatment primarily for esthetic reasons. They undergo comprehensive orthodontic treatment involving major occlusal changes to get the utmost esthetically pleasing face. The older adults with poor dental conditions requiring perio-restorative treatment undergo adjunctive orthodontic treatment to attain a long-term prognosis. The orthodontist faces challenges to practice adult orthodontics due to various issues being under considerations, which are quite different than routine orthodontic treatment oriented to children and adolescents. This article highlights the difficulties and limitations faced by the orthodontist while practicing adult orthodontics with remedies to overcome them.
