Aims and MethodTo compare methohexitone and propofol in electroconvulsive therapy (ECT). A retrospective within-subject comparison was made of the use of these drugs in separate courses of bilateral ECT in one clinic over 10 years. Patients taking mood stabilising or anti-epileptic drugs were excluded. The initial seizure threshold and seizure duration were of particular interest.ResultsThe median initial seizure thresholds were identical (75 mC). The median initial seizure duration with threshold stimulation was 25% shorter with propofol (21v.28 s). The median total numbers of treatments in the courses were identical (eight treatments).Clinical ImplicationsThe shorter seizure duration observed with propofol was not associated with a commensurate rise in the initial seizure threshold. The shorter seizure duration may therefore have no effect on the therapeutic efficacy of treatment.

As alternative medicines or dietary supplements, herbal medicines have received increasing interest in recent years. Danggui and Honghua are two of the most popular traditional Chinese herbal medicines. However, little is known about the pharmacokinetics interactions between Danggui/Honghua and prescription drugs. Therefore, the present study was undertaken to investigate the effect of Danggui or Honghua on the gene expression of cytochrome P450 (CYP) using reverse- transcriptase-polymerase chain reaction (RT-PCR) in Wistar rats. Commercial Danggui (0.35 and 0.7 g/kg, twice a day), Honghua (0.35 g/kg or 0.7 g/kg, twice a day) or water (control group) were given to rats (3 rats for each group) for 5 consecutive days. Treatment of rats with 0.7 and 1.4 g/kg per day Danggui or Honghua for 5 days caused mild to strong increase of CYP 3A1 and decrease of CYP 2E1 RNA expression. However, only Honghua (0.7 and 1.4 g/kg per day) induced the increase of CYP 1A2 RNA expression, while CYP 2C11 RNA was unaffected by both Danggui and Honghua. These data demonstrated that Danggui or Honghua affected the expression of hepatic CYP isoforms in the rats; they elevated CYP 1A2 and 3A1 RNA expression but inhibited CYP 2E1 RNA expression. Such alterations may change the therapeutic actions of the drugs metabolized primarily by P450 system when they are co-administered to people with Danggui or Honghua. Therefore, patients should be cautioned about the potential drug-herb interactions between Danggui or Honghua and prescription drugs that were metabolized by CYP1A2, 2E1 and 3A1 isoforms.

Abstract. Background: Previous suicide attempts increase the risk of a completed suicide. However, a large proportion of patients with deliberate self-wrist cutting (DSWC) are often discharged without undergoing a psychiatric interview. Aims: The aims of this study were to investigate the differences in the characteristics and outcomes of patients with DSWC and those with deliberate self-poisoning (DSP) episodes. The results of this study may be used to improve the efficacy of treatment for DSWC patients. Method: We retrospectively reviewed the medical records of 598 patients with DSWC and DSP who were treated at the emergency department of Seoul Saint Mary's Hospital between 2008 and 2013. We assessed sociodemographic information, clinical variables, the reasons for the suicide attempts, and the severity of the suicide attempts. Results: A total of 141 (23.6%) patients were included in the DSWC group, and 457 (76.4%) were included in the DSP group. A significantly greater number of patients in the DSWC group had previously attempted suicide (p = .014). A total of 63 patients (44.7%) in the DSWC group and 409 patients (89.5%) in the DSP group underwent psychiatric interviews. Conclusion: More DSWC patients had previously attempted suicide, but fewer of them underwent psychiatric interviews compared with the DSP patients.
