Hypoglossal canal (HC) which begins from very slightly above the inner part of the anterolateral portion of the foramen magnum and is located above the occipital condyle of the occipital bone. The aim of this study is to examine HC morphology and variations using cone beam computed tomography (CBCT). The morphology and types of HC were investigated with 303 CBCT images (606 side). Type 1 variation in 606 HC examined becomes the most commonly observed type (57.3%) while type 5 variation was the least common type of variation (0.8%). Type 1 HC was statistically higher in males (p=0.004). Because of HC, which is an anthropologically important point and enters the field of images in CBCT scan, it is recommended that dental radiologists should be aware of their variations and be wary of the pathologies that may occur in this region.

ABSTRACT Objective: The aim of this study was to determine the root and canal morphology of the maxillary and mandibular anterior teeth in a Turkish population by analyzing cone-beam computed tomography (CBCT) images. Materials and Methods: CBCT images of 417 females and 410 males with a mean age of ranging from 14 to 70 years were examined in this study. A total of 1453 maxillary central incisors, 1504 maxillary lateral incisors, 1523 maxillary canines, 1582 mandibular central incisors, 1603 mandibular lateral incisors, and 1604 mandibular canines were analyzed. The number of root canals and the canal configurations were investigated and then were classified according to Vertucci's classification. The data were analyzed with the Pearson Chi-square test. Results: The Type 1 canal configuration was most prevalent in the maxillary anterior teeth (96.8-99.7%). The Types 2, 3, 4, and 5 canal configurations were observed within the range of 0-1.3%, 0-0.8%, 0-0.7%, and 0-1.8%, respectively. In the mandibular anterior teeth, the Type 1 canal configuration was most prevalent (77-95%). The Types 2, 3, 4, and 5 canal configurations were observed within a range of 0.2-2.7%, 0.2-1.6%, 0.9-5.9%, and 1.8-14.4%, respectively. The prevalence of two canals was higher in males than in females both in the maxillary and mandibular anterior teeth (P < 0.05). Conclusions: Type 1 was the most prevalent canal configuration of maxillary and mandibular anterior teeth in the Turkish population. Type 5 was the most frequently observed canal configuration of the two canalled teeth. The incidence of root canal numbers and configurations differed with sex.

It is important to understand pulp canal configuration to have a successful endodontic treatment. Cone Beam Computed Tomography (CBCT) radiography is able to assess the pulp canal configuration, especially on the lingual/palatal, compared to the periapical radiography. The objective of the research is to know the prevalence pulp canal configuration of mandibular molar teeth in Indonesia subpopulation Mongoloid race using CBCT radiography. This research is a descriptive survey with a cross-sectional approach, using 38 CBCT radiographs from the patients undergoing dental treatments in hospitals in Medan. The subjects were selected based on research criteria; then their CBCT radiographs were interpreted and analyzed. The result shows the existence of mandibular mesial root type 2-1 (28.94%), 1-2-1 (5.26%), 2-2 (55.26%), 2-1-2-1 (2.64%), 3-2 (5.26%), 3-1 (2.64%), distal root type 1-1 (36.84%), 2-1 (23.68%), 1-2-1 (15.79%), 1-2 (2.63%), 2-2 (13.17%), 2-1-2-1 (5.26%), 1-3-1 (2.63%). The distolingual root of mandibular first molars consisted of type 1-1 (100%).  Mesial root of mandibular second molars of type 1-1 (2.63%), 2-1 (50%), 1-2-1 (5.26%), 2-2 (36.85%), 2-1-2-1 (2.63%), 1-2-1-2 (2.63%), distal root type 1-1 (57.89%), 2-1 (15.79%), 1-2-1 (13.16%), 2-2 (10.53%), 2-1-2 (2.63%). This variation of the pulp canal can be influenced by the shape of roots. A flat root usually contains pulp canal configuration type II – VIII Vertucci, similarly to mandibular molar mesial canals. As a summary, there is a variation of pulp canal configuration in mandibular first and second molars on Indonesian Mongoloid race, as seen from the CBCT radiographs.

Background Dural arteriovenous fistula of the anterior condylar confluence (ACC-DAVF) is a rare subtype of DAVFs that occurs around the hypoglossal canal. Transvenous embolization (TVE) with coils has been performed for most ACC-DAVFs with a high clinical cure rate. However, some reports call attention to hypoglossal nerve palsy associated with TVE due to coil mass compression of the hypoglossal nerve caused by coil deviation from the ACC to the anterior condylar vein (ACV). Herein, we report a case of ACC-DAVF in which an intraoperative cone-beam computed tomography (CT) contributed to avoiding hypoglossal nerve palsy. Case presentation A 74-year-old man presented with left pulse-synchronous tinnitus. An angiography detected left ACC-DAVF mainly supplied by the left ascending pharyngeal artery and mainly drained through the ACV. The two fistulous points were medial side of the ACC and the venous pouch just cranial of the ACC. We performed TVE detecting the fistulous points by contralateral external carotid angiography (ECAG). The diseased venous pouch and ACC were packed with seven coils but a slight remnant of the DAVF was recognized. Because a cone-beam CT revealed that the coil mass was localized in the lateral lower clivus osseous without deviation to the hypoglossal canal, we finished TVE to avoid hypoglossal nerve palsy. Postoperatively, no complication was observed. No recurrence of symptoms or imaging findings were detected during a five-month follow-up period. Conclusion An intraoperative cone-beam CT contributed to avoiding hypoglossal nerve palsy by estimating the relationship between the coil mass and the hypoglossal canal during TVE of ACC-DAVF.

Adenomatoid odontogenic tumor (AOT) is a rare noninvasive odontogenic tumor that occurs mostly in the second decade of life. Based on its tooth association, AOT can be classified into three categories of follicular, extrafollicular, and peripheral types; the follicular classification is considered as the most common type of AOT. This study reported a large extrafollicular case of AOT in a 40-year-old female. She was asymptomatic and tumor was detected accidentally by her dental practitioner. Since the panoramic radiograph showed a well-defined unilocular radiolucent lesion, we observed radiopaque spots within the lesion by using cone beam computed tomography. The extrafollicular type can mimic a periapical radiolucent lesion.
