As shown by numerous studies conducted during the pandemic, the severe course of COVID-19 is accompanied by multiple organ failure. Cytokine storm, hypercoagulation, complement hyperactivation and other arms comprise the overall picture of the pathogenesis of the severe disease course. The frequent diagnosis of multiple microvascular thrombosis in lung, heart, and kidneys, as well as the presence of platelet-fibrin thrombi there and signs of terminal organ damage, suggest a possible involvement of thrombotic microangiopathy (TMA) in the development of multiple organ failure. In this regard, it is especially important to timely diagnose TMA and start pathogenetic therapy. These measures can significantly reduce mortality due to the novel disease. Heparins and direct oral anticoagulants are the mainstay for prevention and treatment of venous thromboembolism in patients with COVID-19, but their effectiveness in the presence of TMA is questionable. It has been proven that anticoagulants use in critically ill patients with COVID-19 for prevention of large vessel thrombosis is effective, but their role in the prevention of microthrombosis is not clear. Here we review the currently available information on thrombotic microangiopathy, as well as a review of literature data describing TMA-like conditions in COVID19, discuss potential pathophysiology of the condition development and proposed therapeutic approaches.

Aim. To compare the possibilities of venoarterial extracorporeal membrane oxygenation (VA ECMO) and intra-aortic balloon pump (IABP) to prevent organ damage and the development of multiple organ failure.Methods. According to the inclusion and exclusion criteria, 51 patients underwent the study. The patients were divided into 2 groups depending on the method of mechanical circulatory support used: VA ECMO (n = 29) and IABP (n = 22). To assess organ functions in the intra- and postoperative period, the results of instrumental and laboratory research methods, as well as data from complex scales of organ dysfunction, were analased.Results. Myocardial depression was observed in the IABP group in the intraoperative period of high-risk percutaneous coronary intervention and worse hemodynamic stability compared to the VA ECMO group was traced. Organ dysfunction and multiorgan failure developed more often in the IABP group, which was confirmed by laboratory specific markers.Conclusion. High-risk percutaneous coronary intervention with VA ECMO is accompanied by a lower incidence and severity of organ damage and multiple organ failure. Thus, the VA ECMO has better organ protective effects.

In November 2019, the coronavirus disease-2019 (COVID-19) outbreak was first reported. The infection of COVID-19 has rapidly spread worldwide. Some have unapparent or mild symptoms, but some patients have fatal forms including respiratory or multiple organ failure. In March 2020, the World Health Organization announced COVID-19 as pandemic disease. The COVID-19 pandemic is a sudden, difficult-to-control, and severe disease that can also appear to patients in dermatology. However, understanding disease and experience with it is still insufficient. Therefore, we would like to review patients with fungal infections in the COVID-19 pandemic era.

One of the most serious problems of modern medicine is sepsis. The number of patients undergoing this complication is 20–30 million (WHO) annually and has no tendency to decrease. Sepsis is characterized by severe multiple organ failure due to a violation of the response of the macroorganism to an infectious agent. Moreover, it is dangerous with high mortality. Sepsis often develops in patients with immunodeficiency conditions, which primarily include pregnant women. The article presents a clinical observation of a case of periostitis in a pregnant woman complicated by a septic state.

Diquat is a widely used herbicide that is substituted for paraquat. With paraquat off the market, cases of diquat poisoning have been gradually increasing. The kidney is the most frequently impaired organ in diquat poisoning. Few cases of multiple organ failure caused by diquat have been reported. We herein describe a 30-year-old man who orally ingested about 160 mL of enriched diquat. Despite aggressive treatment, the patient’s condition progressed to multiple organ failure and death. The pulmonary lesions in this patient were different from those previously reported. This patient did not die of renal failure but of severe respiratory failure. He exhibited three different stages of pulmonary disease. The lung lesions in this case were unique. We hope that doctors will pay more attention to the lung lesions in patients with diquat poisoning in future and find new treatment methods to save the lives of such patients.
