Oral Health Status of Children with Congenital Heart Disease and the Awareness, Attitude and Knowledge of their Parents

The oral health status of children with congenital heart diseases and the parental awareness on maintaining good oral health and attitude towards preventive dental health measures were evaluated. A total of 170 children between the age group of 1-16yrs belonging to both genders, with the history of congenital heart disease from Sree Chitra Tirunal Institute of Medical Science and Technology, Thiruvananthapuram and Narayana Hrudayalaya Institute of Medical Sciences, Bangalore were examined. Oral lesions and caries experience were recorded using modified WHO oral health assessment form. Oral hygiene of the children with congenital heart disease was found to be poor with tongue coating (50.6%), plaque (41.8%), calculus(35.3%), and caries (42.4%). Parental awareness on the importance of maintaining good oral hygiene, preventive dentistry, medicinal decay and its systemic effects has been found to be very poor. Dentistry should give priority to patients whose general health may be put at risk by poor dental health. Closer cooperation between Pediatrician, Pediatric Cardiologists and Pediatric Dentists could help improve dental care for these children.

Congenital cardiac disease is one of the most common developmental anomalies in children. Higher caries prevalence and gingivitis compared to healthy children have been shown in children with congenital heart disease which has a significant implication in the medical care of these patients associated with bacteraemia and endocarditis. Sialic acids being terminal sugar components and marker of chronic inflammatory response are found to be present at higher levels in children with poor oral health status. So the present study aimed to evaluate the oral health status and salivary sialic acid levels among the children with congenital heart disease and normal healthy siblings. A total of 71 children with heart diseases aged 6-10 years attending various heart institutes were examined for oral hygiene status, gingival status and dental caries status by using modified WHO oral assessment form. A case-matched control group of 50 children were also examined. Salivary flow rate, pH and sialic acid levels were measured after saliva collection. The results were subjected to Unpaired t test and Pearson’s Correlation Coefficient Test. The salivary pH, salivary flow rate, dental caries status, oral hygiene and the gingival status were significantly compromised with a positive correlation with the sialic acid levels in saliva, leading to increased treatment needs in the study group. Therefore the amount of sialic acid in the saliva can be a useful index of the severity of oral disease.

Autism is one of the most severe childhood neuropsychiatric disorders. Autistic individuals are characterized by impairment in social interaction with a restricted range of interests and often, stereotyped repetitive behaviors. Studies on oral health conditions in children with autism are sparse. The complicated disability itself makes clinical research difficult. Aim: The need for baseline information regarding the oral health status of children with autism is essential. Method: The present study assessed the oral health status of 106 autistic children aged 4 to 15 years in Bangalore city, India. The dental caries was recorded according to the WHO criteria; oral hygiene was assessed using the Oral Hygiene Index-Simplified (OHI-S) and its modification for deciduous dentition. The behavior of children towards dental treatment was also assessed using the Frankel's behavior rating scale. Data obtained was subjected to statistical analysis. Results showed that caries experience among autistic children was lower; however they were found to have more debris and calculus deposits. Conclusions: Negative behavior towards dental treatment was seen in autistic children.

Objective: To examine the degree to which children, considered to be at risk from infective endocarditis, had received professional education and preventive procedures in regard to dental health, and to evaluate the knowledge of their parents of the link between oral health and infective endocarditis. Materials and methods: Questionnaires were distributed to the families of 38 children under the care of paediatric cardiology. A short dental examination was carried out. Parents were asked if they knew why oral health was of particular importance in their child. Results: Of the children, 58% demonstrated evidence of previous or current dental disease, with 24% having had at least one filling, 13% with one or more teeth showing deficiency of enamel, and 39% with untreated dental caries. Only 79% of the children were registered with a dentist. According to Chi squared test, there was no difference in the dental health of registered and non registered children. Of the study group, 29% had received instruction in oral hygiene, 42% had received dietary advice, 13% had received advice regarding fluoride supplementation or had had fluoride professionally applied, and 8% had had fissure sealants. These percentages remained relatively low even if only registered children, or only registered children with previous or current dental disease, were considered. Only 64% of parents were aware of the link between the oral health of their children and infective endocarditis. Parents of children who were registered were more likely to be aware of this link than parents of children who were not registered. Conclusions: In spite of being registered with general dental practitioners, few children with congenital heart disease had received basic education in dental hygiene. Even children known to have had dental disease and, therefore, considered to be more vulnerable, were overlooked.

Introduction. Economic inequality, political instability and globalization have contributed to the constant growth of the migration phenomenon in recent years. In particular, a total of 4.2 million people migrated to Europe during 2019 and most of them settled in Germany, France and Italy. Objectives. The objective of this study was to conduct a systematic review of studies analyzing the oral health condition among migrants from middle- and low-income countries to Europe and assessing the eventual association between their sociodemographic and socioeconomic characteristics and oral health status. Materials and Methods. A systematic review was conducted in PubMed, Cochrane Library, Scopus and Science Direct databases. After titles, abstracts and full-text examination, only 27 articles were selected on the basis of inclusion criteria and consequently included for quality assessments and data extraction. Results. Most of the studies reported a higher prevalence of caries experience, a poorer periodontal health and more difficulties in accessing dentalcare services among migrant groups compared with the non-migrant population. Inequalities were mostly associated with ethnic background, economic condition and social grade. Conclusion. Our review demonstrates the lack of dental health among migrants, underlining that their cultural beliefs and their social and economic living conditions could influence their oral health.

Background:The assessment of oral health status of children in private school which followed dietary advice and the private school which did not follow dietary advice provide data on the oral health status of children from different socio-economic background. Aim:The aim of the following study is to assess and to compare the gingival status and caries experience between children from the private school which followed dietary advice and the private school which did not follow dietary advice in puducherry, INDIA Subjects and Methods:Retrospective cohort study design was done. Exposure status was the school following dietary advice and unexposed was not following. Random sampling was employed to select the study participants. Oral hygiene status, gingival status and caries experience was assessed and compared among 3 to 15-year-old children from the private school which followed dietary advice and the private school which did not follow dietary advice. The examination was carried out by trained and calibrated investigators using a mouth mirror and explorer under natural daylight Results: A total of 233 children (137 students from private school which did not follow dietary advice and 96 students from private school which followed dietary advice) were examined in the study. The mean decayed missing lled teeth were higher among private school children which did not follow dietary advice compared with private school which followed dietary advice children. A signicantly higher number of children in the private school which did not follow dietary advice had poor oral hygiene status, moderate to severe gingivitis and caries experience. Conclusion: The prevalence of oral diseases was relatively less among children from private schools which followed dietary advice in comparison with those from the private school which did not follow dietary advice.

AIM: This study was to evaluate saliva parameters of identical and non identical twins with their dental caries and oral hygiene status. MATERIAL AND METHODS: A cross sectional study was conducted to assess the oral health status and investigate the salivary parameters of identical and non–identical twins in Kodhini Village, Kerala. The WHO Oral Health Assessment Form was used to record the oral health status. The Simplified Oral Hygiene Index and Gingival Bleeding Index were also included. Unstimulated saliva was collected. Immunoglobulin A levels in saliva was estimated by Turbidimetric Immunoassay. Salivary Cortisol was analysed using ELISA method. For TAC thiobarbituric acid reactive from Fenton’s reaction. RESULTS: Since the absolute difference of the salivary parameters between the pairs of twins was not symmetrically distributed, we report median and interquartile range to describe these differences. There were no observed significant variations in the above salivary parameters. CONCLUSION: The similarity in their environmental factors as well as perhaps the similar genetic predisposition on salivary factors has led to the similarity in the dental caries experience in these twin children.

Preschool children are a group of children who are vulnerable to dental and oral diseases because they generally still have behaviors or personal habits that are less supportive of dental health. Disease prevention is effective if parents provide education in an effort to improve children's dental health. The purpose of this study was to determine the relationship between the level of knowledge of mothers and preschool children about the consumption of cariogenic foods on oral health status in Taebenu District, Kupang Regency. This research method is an observational study with a cross-sectional design. The research subjects were 66 consisting of 33 preschoolers aged 4-6 years and 33 mothers of preschool children. Mother and child's knowledge variable was measured by a questionnaire by filling in true and false choices. Oral hygiene status variables were measured using an interview guide. The results of the correlation analysis showed that the mother's knowledge variable (p=0.000) had a significant effect on oral hygiene status. The variable of children's knowledge (p=0.447)  was not significantly related to oral hygiene status. The conclusion is mother's knowledge about cariogenic food consumption is related to oral health status. Children's knowledge about cariogenic food consumption is not related to oral health status. Anak prasekolah merupakan kelompok anak rentan terhadap penyakit gigi dan mulut karena umumnya masih mempunyai perilaku atau kebiasaan diri yang kurang menunjang terhadap kesehatan gigi. Pencegahan penyakit efektif apabila orang tua melakukan edukasi dalam upaya peningkatan kesehatan gigi anak. Tujuan penelitian ini adalah untuk mengetahui hubungan tingkat pengetahuan ibu dan anak pra sekolah tentang konsumsi makanan kariogenik  terhadap status kesehatan mulut di Kecamatan Taebenu Kabupaten Kupang. Metode penelitian ini merupakan penelitian observasional dengan rancangan cross- sectional. Subjek penelitian berjumlah 66 yang terdiri dari anak prasekolah usia 4-6 tahun sebanyak 33 orang dan ibu dari anak prasekolah sebanyak 33 orang. Variabel pengetahuan ibu dan anak diukur dengan kuesioner dengan mengisi pilihan benar dan salah. Variabel status kebersihan mulut diukur menggunakan panduan wawancara. Hasil analisis korelasi menunjukkan bahwa variabel pengetahuan ibu (p=0.000) berpengaruh secara signifikan dengan status kebersihan mulut. Variabel pengetahuan anak (p=0,447) secara signifikan tidak berhubungan dengan status kebersihan mulut. Kesimpulannya adalah pengetahuan ibu tentang konsumsi makanan kariogenik berhubungan dengan status kesehatan mulut. Pengetahuan anak tentang konsumsi makanan kariogenik tidak berhubungan dengan status kesehatan mulut.

Objective: to assess the dental health and oral hygiene status of subjects with cleft lip and/or palate. Methods: dental and gingival examinations were carried out in 105 children with cleft, using standard dental indices dmft and DMFT. Plaque and gingival indices were scored using the indices of Loe and Silness. Results: the prevalence of dental caries was significantly higher in children with cleft (71.34%) than children without cleft in both deciduous and permanent teeth. The dmft and DMFT indices ware highest in children with CLP (4.21±3.68; 3.55±3.24). Similary, plaque accumulation and gingivitis were significant higher in CL, CLP patients (3.29±1.28; 3.23±1.54) compared with CP patients (2.38±1.74). Conclusion: Cleft patients had higher prevalence of caries and poorer oral hygiene than patients without cleft. The severity of cleft affected on the oral health status. These findings not only provide a baseline for oral health status but also emphasize the need for preventive measures. Objectives: Calculate the prevalence and severity of dental caries and periodontal health among school students aged 12-18 years old and their attitudes toward dental health and oral hygiene at Al-Mafraq governate- Jordan.Materials and methods: Demographic and oral behavioral information were collected; students aged 12-18 years old from six public schools distributed over three districts in Al-Mafraq governate over a six-month period had a full oral and dental examination. DMFT index (Decay, Missing, Filling, Teeth) was used to measure caries experience. Plaque index (PI) and gingival index (GI) of Löe and Silness were used to evaluate the oral health status. T - test was used for statistical evaluation. A P-value < 0.05 was considered statistically significant.Results: 1165 students were examined, 51% never had been to a dentist or had any previous dental treatment, 39.6% of the students examined never brushed their teeth and 61.85 % had dental caries. The (SiC) was 4.72 with a decay representing 93.53%. Females brush their teeth more frequent than males with a statistically significant difference; p< 0.001. Females showed lower DMFT index (1.81) and compared to males (2.25) with a statistically significant difference. Females showed lower SiC (4.164) compared to males (5.186)   with a statistically significant difference. There was no statistically significant difference between males and females in PI p= 0.590, and GI; p= 0.852.Conclusions: The prevalence of dental caries among students in Al-Mafraq governate and the significant caries index were substantially higher than the target goals of WHO/ FDA of year 2015 with more than 90 % of unmet treatment needs. Girls showed better oral hygiene habits compared to boys and exhibited less dental caries experience. However, periodontal status did not differ significantly between both genders.
