Scar ectopic pregnancy is a condition where the gestational sac implants into the previous caesarean scar site. Although it is a rare entity, its incidence is increasing due to rising rates of caesarean deliveries. Here authors report a case of caesarean scar ectopic pregnancy managed by laparotomy with caesarean scar ectopic excision following failed medical management. The patient recovered without any intraoperative or postoperative complications. An early diagnosis and management are vital in preventing maternal morbidity and mortality.

Pregnancy implantation within previous caesarean scar is one of the rarest locations for an ectopic pregnancy. Incidence of caesarean section is increasing worldwide and with more liberal use of transvaginal sonography, more cases of caesarean scar pregnancy are being diagnosed in early pregnancy thus allowing preservation of uterus and fertility. However, a delay in either diagnosis or treatment can lead to uterine rupture, hysterectomy, and significant maternal morbidity. We are reporting a rare case of first trimester caesarean scar pregnancy with viable fetus in the process of rupture, where uterine repair could be done, thus preserving the future fertility.

Puerperal uterine inversion is a complication of third stage of labour, which can lead to maternal morbidity and mortality due to haemorrhage shock and infection. Early cases can be managed by manual reposition of uterus but neglected or late cases of uterine inversion are managed by Haultain`s repair. Here we are presenting a case of subacute uterine inversion referred from peripheral hospital managed by Haultain’s technique.

Broad ligament pregnancy is rare form of abdominal pregnancy. There is high risk of maternal morbidity and mortality. Due to its rarity, usually, diagnosis cannot be established before surgery. We reported a 27 year old, G3P2L2 who presented with 4 month amenorrhea with pain in lower abdomen. Ultrasonography revealed a single live extra-uterine pregnancy of 17 weeks in right adnexa. A right sided Broad ligament pregnancy was found on exploratory laparotomy. Excision of mass with right sided salpingo-oophrectomy was performed. Post operative course was uneventful. Early diagnosis with high risk of suspicion and early intervention is needed to overcome it’s related complications.

Introduction Implantation of a gestational sac in a previous Caesarean section scar of the lower uterine segment is a rare form of ectopic pregnancy. Case report We report a case of Caesarean scar ectopic pregnancy in a 25-year-old female, diagnosed by ultrasonography and confirmed by magnetic resonance imaging. We present the clinical details, imaging findings, and management of the patient. Discussion Imaging plays an important role in the diagnosis of ectopic pregnancy and ultrasonography is the modality of choice. Ultrasonography features of scar ectopic pregnancy include empty uterus and cervix with normal endometrium and endocervical canal, gestational sac (with embryo and/or yolk sac) in the anterior part of the lower uterine segment in the region of the Caesarean scar with a thin myometrial layer between the bladder wall and gestational sac. Magnetic resonance imaging may be used as an adjunct imaging modality in cases with inconclusive or equivocal sonographic findings. Termination of pregnancy in the first trimester should be considered and treatment options should be individualized as there is no universal agreement on the best or most preferred treatment modality. Conclusion Awareness of specific ultrasound features of scar ectopic pregnancy is crucial for early recognition, correct diagnosis, and initiating prompt management to prevent complications.

Ectopic pregnancy in a caesarean scar is the rarest form of ectopic pregnancy and probably the most dangerous one because of the risk of uterine rupture and massive hemorrhage. Since the advent of endovaginal ultrasonography, ectopic pregnancy in a caesarean scar can be diagnosed and distinguished from cervical pregnancy early in pregnancy. Here we describe a case of ectopic pregnancy in a caesarean scar in which the diagnosis was done at 6 weeks of pregnancy by transvaginal sonography and confirmed by MRI. The case was successfully managed by ultrasonography guided local injection of methotrexate into the gestational sac through trans abdominal route.Bangladesh J Obstet Gynaecol, 2015; Vol. 30(1) : 43-48

Caesarean scar ectopic is one of the rarest forms of ectopic pregnancy [3]. The diagnosis and treatment of it is challenging. It is important to diagnose the condition as early as possible in order to administer appropriate, timely treatment and to avoid complications. A rare case of laparoscopic management of an ectopic pregnancy in a previous Caesarean section scar is reported here. Laparoscopy enabled the successful treatment of an unruptured ectopic pregnancy in a previous Caesarean scar and made it possible to preserve the patient’s reproductive capability. In this report we present a case of 30 year old gravida 2, para 1, living 1 with cesarean scar ectopic pregnancy managed laparoscopically.
