Adnexal mass in pregnancy

Adnexal masses in pregnancy are a rare finding. The majority of these masses are discovered incidentally during routine follow-up. The differential diagnosis of adnexal masses discovered during pregnancy is broad and the management of such lesions has been a subject of debate for years with no consensus regarding the best management plan. We report a case of a 38 year-old pregnant woman who was diagnosed at the time of the first trimester ultrasound with a multilocular solid arising form the right ovary. A laparoscopy followed by left oophorectomy was performed at 22 weeks and the definitive histology revealed a borderline ovarian cyst/proliferative mucinous atypical cyst. The patient delivered via vaginal at 38 weeks of gestation. This case was discussed and a secondary staging surgery (peritoneal washing, total abdominal hysterectomy, salpingo-oophorectomy, omentectomy, appendectomy, and peritoneal biopsies) was performed with no evidence of disease found. The surgical approach is controversial due to the increased risk of complications. In the absence of large prospective randomized trials it is difficult to know which are the best management practices and especially to determine the right moment during pregnancy to perform surgery in these patients.

Abstract One hundred seventy-seven tracts in South Carolina were evaluated for compliance with site preparation Best Management Practices (BMPs). South Carolina Forestry Commission foresters evaluated compliance with BMPs in each of four categories of site preparation: (1) mechanical treatments, (2) herbicide applications, (3) prescribed burning, and (4) minor drainage. In addition, each site was evaluated for overall BMP compliance, which was based on the level of BMP compliance and evidence of off-site water quality impacts. Overall BMP compliance was 86.4%. Compliance was highest for mechanical (92.0%) and herbicide (88.3%) site preparation treatments and lowest for minor drainage (76.9%) and prescribed burning (70.4%). Eleven variables were analyzed to determine their influence on BMP compliance. Four variables were found to be significantly related to BMP compliance: (1)physiographic region, (2)proximity to streams, (3)percent slope, and (4) terrain type. The four significant variables described the increased risk involved in site preparing land that is in close proximity to streams. Noncompliance with BMPs was clearly focused on the Piedmont physiographic region where 22 of the 24 inadequate sites were located. South. J. Appl. For. 22(2):74-80.

We report a case of a 33 year-old pregnant woman who was diagnosed at the time of the first trimester ultrasound with a multilocular solid arising form the right ovary. An abdominal MRI was performed afterwards and it revealed a pelvic mass, developing from the right ovary, with a liquid component but with a major solid area. CA 125 was within the normal range values. A laparotomy followed by right salpingo-oophorectomy was performed at 14 weeks of gestation and both the frozen section and the definitive histology revealed a borderline mucinous ovarian tumor. Ovarian tumors of low malignant potential comprise 10%–20% of all ovarian malignancies. They carry an excellent prognosis with 95%–99% long-term survival. Whereas in the past, radical surgery (hysterectomy and bilateral salpingo-oophorectomy with peritoneal staging) was standard regardless of the age of the patient, unilateral salpingo-oophorectomy with or without staging has become the recommended management for women who desire childbearing. In the absence of large prospective randomized trials it is difficult to know which are the best management practices and especially to determine the right moment during pregnancy to perform surgery in these patients.

Abstract Kid rearing is the foundation of goat milk production, yet little is known about how to raise replacement does efficiently to make healthy and productive dairy animals. This study aimed to identify the common rearing practices of Canadian commercial dairy goat farms (≥40 goats/farm), from birth to weaning, and to determine best management practices to improve herd performances. A survey was sent to dairy goat producers across Canada by post or email, and 104 respondents were selected for analysis. The 70-questions survey collected information regarding kidding management, care of the newborn, feeding in the preweaning period, housing, weaning and herd performances. Respondents were from Ontario (69%), Quebec (22%) and the Western provinces (9%). Farm sizes ranged from 42 to 2,500 (median: 190) goats, and most producers (64%) were relatively new to goat milk production (≤10 yrs). A large amount of variation in rearing practices was seen across farms. Ad libitum milk was offered on 55% of farms, and there was no consistency as to when concentrates, forages and water were first offered to kids. Weaning criteria was predominantly a mix of age and weight of the kid (36%), followed by age only (27%) and weight only (22%). Weaning age varied between 4.5 and 20 (median: 8) wks and weaning weight varied between 9 and 35 (median: 15) kg. Weaning methods ranged from abrupt (37%) to different progressive strategies (20% skipping milk feedings, 19% reducing milk quantity, and 10% diluting milk with water). This research provides the dairy goat industry with information concerning current common kid rearing practices used on Canadian goat farms, and the lack of consensus indicates that further research is necessary to determine and refine the best kid rearing practices for Canadian farms.
