<p class="abstract">Pruritus of unknown origin is defined as itching lasting for more than 3 weeks without a clear identifiable cause. Aetiology of itching is wide ranging and includes chronic renal failure, cholestasis and internal malignancy. Iron deficiency has been described to be causative of pruritus but the mechanism underlying this association remains unclear. We report the case of a female patient with iron deficiency anemia and generalized pruritus, resolved after intravenous administration of iron-complex supplements and we explained a possible mechanism between this association.</p>

Background Anemia in chronic renal failure (CRF) has beenproved to influence the quality of life, increasing morbidity andmortality. Early diagnosis and prompt treatments of anemia aremandatory to manage CRF appopriately. So far data of anemia inCRF in Indonesia is limited.Objective To find out the profile of anemia in children with CRF atCipto Mangunkusumo Hospital (CMH), Jakarta, with special atten-tion in erythrocyte indices and iron deficiency anemia.Methods Cross-sectional descriptive study was carried out onpatients with CRF and anemia in CMH since October 2003 to April2004.Results There were 20 CRF patients, aged between 1 year 3 month-15 year old, mostly were above 10 year old, 11 patients were malesand 9 were females. The most frequent etiologies were urinarytract infection (UTI) in 10 cases and nephrotic syndrome in 6 cases.Of those 20 patients, 14 suffered from anemia with erythrocyteindices, normochrome normocytic in 9 patients and hypochromemicrocytic in 5 patients. Of 14 anemic patients only 1 patient suf-fered from iron deficiency anemia of less than 10 mg/l and transferinsaturation of less than 12%. Based on serum iron (SI) concentra-tion only, 7 patients were diagnosed as having iron deficiency ane-mia.Conclusion Most patients with chronic renal failure havenormochrome normocytic anemia. Hypochromic microcytic irondeficiency anemia is scarcely found in this group of patients.

A insuficiência renal crônica resulta da perda progressiva e irreversível dos néfrons. Pacientes com insuficiência renal, em sua maioria, desenvolvem anemia, devido os rins estarem comprometidos não conseguindo formar quantidades adequadas de eritropoetina. A anemia da doença renal crônica é atribuída a um déficit relativo de eritropoietina, podendo ter como fatores agravantes a deficiência de ferro. Nesse estudo, foram avaliados em um período de 3 meses,150 pacientes portadores de insuficiência renal crônica, em uma unidade de Terapia Renal Substitutiva na Cidade de Ponta Grossa, PR. Foram analisados os valores de hemoglobina e também parâmetros bioquímicos como ferro, ferritina e índice de saturação de transferrina. Os resultados obtidos foram 50% (75) pacientes do gênero masculino e 50% (75) do gênero feminino, sendo a maior prevalência encontrada entre a faixa de etária de56 a65 anos, com 28% (21) para homens e mulheres. Com relação aos valores de hemoglobina, obteve-se 95% dos pacientes do sexo masculino e 85% do sexo feminino com valores de hemoglobina abaixo de 13,0 g/dL e 12,0 g/dL respectivamente. A dosagem de ferro sérico mostrou que apenas pacientes do sexo masculino apresentam esse parâmetro abaixo do valor de referência, com um percentual de 11%. Para a ferritina 99% e 83% das mulheres e homens, respectivamente, apresentaram resultados acima do referenciado. No parâmetro índice de saturação da transferrina, no gênero masculino houve 6,7% abaixo do valor de referencia e apenas 1% para as mulheres. Correlacionando as variáveis de ferro com as variáveis hemoglobina e índice de saturação de transferrina, observou-se que houve correlação apenas em pacientes do sexo masculino. Dos pacientes masculinos 8 (10,6%) apresentaram ferro e hemoglobina abaixo dos valores de referencia, e quando comparado ferro e índice de saturação de transferrina, 5 (6,7%) apresentaram as variáveis abaixo dos valores referenciados, caracterizando a anemia ferropriva. O diagnóstico de anemia ferropriva em pacientes com problemas renais crônicos é de extrema importância, afim de que, os pacientes tenham melhor resposta ao tratamento com eritropoietina e uma melhor qualidade de vida.

Case study of female patient with hypochromic anemia, who has been treated with iron preparations for iron deficiency anemia for a long time, is reported. A hemoglobin electrophoresis test has been performed due to treatment failure, and β-thalassemia (thalassemia minor) has been diagnosed. The data on β-thalassemia and approaches to differential diagnosis of hypochromic anemias are summarized.

Idiopathic pulmonary hemosiderosis (IPH) is a rare cause of alveolar hemorrhage in children but should be considered in children with anemia of unknown origin who develop respiratory complications. It is commonly characterized by the triad of recurrent hemoptysis, diffuse parenchymal infiltrates, and iron-deficiency anemia. Pathogenesis is unclear and diagnosis may be difficult along with a variable clinical course. A 6-year-old boy was admitted to the hospital with a severe iron-deficiency anemia, but he later developed severe acute respiratory failure and hemoptysis requiring intubation and mechanical ventilation. The suspicion of IPH led to the use of immunosuppressive therapy with high dose of corticosteroids with rapid improvement in clinical condition and discharge from hospital.
