As the government pushes though the HE and Research Bill (Department for Education, 2016), universities have an unprecedented opportunity to add well-funded accelerated degrees to their portfolios. To ensure high quality outcomes and student experiences we need to establish a well-researched pedagogy for accelerating learning alongside an accelerated delivery.

Rhinologic literature has historically relied on lower levels of evidence to make prescriptive recommendations for use of new technology and procedures. However, as the medical profession has moved to embrace the principles of evidence-based medicine, expectations for minimum standards of evidence have risen. The resulting high-quality efficacy outcomes data have become the linchpin of informed decision making by physicians, payers, and health care systems. While many challenges remain in this transition to higher evidence expectations, none are insurmountable. It has become the responsibility of the organized medical profession to play a role in influencing and supporting production of high-quality outcomes research.

Aim: To define the components and organization of a modern leg ulcer service that is acceptable to patients cost effective and produces high quality outcomes. Method: Analysis of data from published literature as well as experience from the organisation of a leg ulcer service in Gloucestershire. Synthesis: Assessment of leg ulcers requires the services of a vascular laboratory to assess the venous and arteria systems. Effective systems of compression must be employed. Staff and patients must be educated to understand the principals behind their use. For some patients pinch-grafting may be appropriate. Where arterial disease is present in the lower limb, reconstructive surgery should be used for the lower limb vessels The organisation of such a service must ensure good liaison between General Practitioners, community nurses and hospital specialists. Many patients may be managed in community leg ulcer clinics. In some cases, the advice of specialists such as rheumatologists and dermatologists may be required. Conclusions: A national framework for leg ulceir management is needed within which resources are made available in a way that satisfy local needs. Such a framework must be based on management protocols derived from evidence-based practices that have been developed through the experiences of modern leg ulcer services.

The success of any endodontics therapy depends on, the diagnosis, treatment planning, access cavity preparation, maintenance and shaping following the canal obturation. Access is the initial and mainly crucial point of root canal treatment. For a good endodontic results well-designed access preparation is required. Perforations are a possible complication during root canal treatment. An assortment of materials including, calcium hydroxide, amalgam, glass ionomer, zinc oxide–eugenol and resin modified GI are being used for perforations. The idyllic material for treating perforations should be radiopaque and nonabsorbable. Mineral trioxide aggregate (MTA) with distinctiveness are as with high-quality outcomes in repair perforations. The idea of this case series is to explain the handling of furcal perforation.
