Abstract Background Postural instability presents a common and disabling consequence of chemotherapy-induced peripheral neuropathy (CIPN). However, knowledge about postural behavior of CIPN patients is sparse. With this pilot study, we used a new approach to i) characterize postural impairments as compared to healthy subjects, ii) allocate possible abnormalities to a set of parameters describing sensorimotor function, and iii) evaluate the effects of a balance-based exercise intervention.Methods We analyzed spontaneous and externally perturbed postural control in eight CIPN patients before and after a balance-based exercise intervention by using a modification of an established postural control model. These findings were compared to 15 matched healthy subjects.Results Spontaneous sway amplitude and velocity were larger in CIPN patients compared to healthy subjects. CIPN patients’ reactions to external perturbations were smaller compared to healthy subjects, indicating that patients favor vestibular over proprioceptive sensory information. The balance-based exercise intervention up-weighted proprioceptive information in patients.Conclusion CIPN patients’ major postural deficit may relate to underuse of proprioceptive information that results in a less accurate posture control as spontaneous sway results indicate. The balance-based exercise intervention is able to partially correct for this abnormality. Our study contributes to a better understanding of postural impairments in CIPN patients and suggests an effective treatment strategy.

AbstractImpaired postural control is often observed in response to neurotoxic chemotherapy. However, potential explanatory factors other than chemotherapy-induced peripheral neuropathy (CIPN) have not been adequately considered to date due to primarily cross-sectional study designs. Our objective was to comprehensively analyze postural control during and after neurotoxic chemotherapy, and to identify potential CIPN-independent predictors for its impairment. Postural control and CIPN symptoms (EORTC QLQ-CIPN20) were longitudinally assessed before, during and three weeks after neurotoxic chemotherapy, and in three and six months follow-up examinations (N = 54). The influence of peripheral nerve function as determined by nerve conduction studies (NCS: compound motor action potentials (CMAP) and sensory action potentials (SNAP)), physical activity, and muscle strength on the change in postural control during and after chemotherapy was analyzed by multiple linear regression adjusted for age and body mass index. Postural control, CIPN signs/symptoms, and CMAP/SNAP amplitudes significantly deteriorated during chemotherapy (p < .01). During follow-up, patients recovered from postural instabilities (p < .01), whereas CIPN signs/symptoms and pathologic NCS findings persisted compared to baseline (p < .001). The regression model showed that low CMAP and high SNAP amplitudes at baseline predicted impairment of postural control during but not after chemotherapy. Hence, pre-therapeutically disturbed somatosensory inputs may induce adaptive processes that have compensatory effects and allow recovery of postural control while CIPN signs/symptoms and pathologic peripheral nerve function persist. Baseline NCS findings in cancer patients who receive neurotoxic chemotherapy thus might assist in delineating individual CIPN risk profiles more precisely to which specific exercise intervention programs could be tailor-made.

El objetivo de este estudio fue el determinar los efectos de un programa de ejercicios de control postural sobre el equilibrio estático y la precisión de lanzamiento del tiro con arco en atletas de la serie infantil (12-14 años) y cadetes (15-17 años). Participaron 12 deportistas para cada serie. Se realizó la prueba de medición de equilibrio a través del Standing Stork Test (ojos abiertos/cerrados) y de la prueba asertividad en la precisión de lanzamiento (6 rondas) en el blanco previo y posterior a la intervención de ejercicios. Ésta última consistió en un programa de entrenamiento de equilibrio bi/unipodal durante 8 semanas. Los resultados mostraron un aumento significativo del equilibrio unipodal con ojos abiertos del grupo infantil en ambas piernas; pero sin cambios en la modalidad de ojos cerrados. La serie cadetes arrojó una diferencia significativa en la prueba de equilibrio unipodal con ojos abiertos sólo en la pierna dominante (derecha); sin cambios en la pierna izquierda, como tampoco diferencias en la modalidad ojos cerrados. La prueba de asertividad del tiro con arco sólo mejoró en el grupo infantil, pero no en la serie cadetes post intervención. Es posible que el entrenamiento de equilibrio postural estático haya optimizado patrones de control postural y visomotores en edades tempranas (bajo los 15 años) en comparación al grupo de cadetes, para así mejorar la precisión del tiro. Se recomienda el potenciar estos ejercicios en edades tempranas para fomentar las destrezas propias del deporte de tiro con arco.Abstract: The objective of this study was to determine the effects of a postural control exercise program on static balance and accuracy of archery throwing in infant (12-14 years old) and cadet (15-17 years old) categories. A total of 12 athletes per each category took part in the study. As measurement tests, we used the Standing Stork Test (open / closed eyes) for balance and the assertiveness in target launch precision test (6 rounds) before and after the exercise intervention. This consisted of a bi / unipedal balance training program lasting 8 weeks. The results showed a significant increase in unipedal equilibrium with open eyes in the infant group and for both legs; however, no changes were detected in the closed-eye modality. The cadet category showed significant differences in the unipedal equilibrium test with open eyes only for the dominant leg (right); no changes were found for the left leg, nor in the closed-eye modality. The assertiveness test of the archery only improved in the infant group, whereas in cadets it did not change at post-test. It is possible that static postural balance training optimized patterns of postural and visuomotor control at early ages (under 15 years old) compared to the group of cadets, and therefore the throwing accuracy was enhanced. It is recommended to endorse these exercises at early ages so to empower the skills needed in the sport of archery.

A cross-sectional, prospective, between-subjects design was used in this study to establish the differences in sensory reweighting of postural control among different ages during adolescence. A total of 153 adolescents (five age groups; 13–17 years old) performed bipedal standing in three sensory conditions (i.e., with visual restriction, vestibular disturbance, and proprioceptive disturbance). Center of pressure displacement signals were measured in mediolateral and anteroposterior directions to characterize reweighting in the sensory system in static postural control when sensory information is disturbed or restricted during adolescent growth. The results indicate a development of postural control, showing large differences between subjects of 13–14 years old and older adolescents. A critical change was found in sensory reweighting during bipedal stance with disturbance of proprioceptive information at 15 years old. Adolescents of 13–14 years old showed less postural control and performance than older adolescents during the disturbance of proprioceptive information. Moreover, the results demonstrated that the visual system achieves its development around 15–16 years old. In conclusion, this research suggests that a difference of sensory reweighting under this type of sensorial condition and sensory reweight systems would seem to achieve stabilization at the age of 15.

Цель исследования - охарактеризовать микробиологическую картину стоматита до и после его лечения высокоактивными культурами лактобацилл, дать оценку эффективности их применения при экспериментальном стоматите. Методика. Опыты проводились на 32 самках беспородных белых крыс массой 230 г. Моделирование стоматита включало 2 этапа: моделирование травматического стоматита и воспроизведение бактериального стоматита на базе травматического. У крыс контрольной и опытных серий на всех этапах эксперимента брали мазки с поверхности десен для характеристики микробиоценоза ротовой полости. Результаты. После обработки слизистой оболочки полости рта (СОПР) культурой патогенного штамма Staphylococcus aureus существенно снизились частота встречаемости и количество условно-патогенных микроорганизмов. Лечение стоматита у крыс культурами лактобацилл ( Lactobacillus 11 зв., Lactobacillus 2 п.рта, Lactobacillus 24 д.ст.) и их комбинацией приводило к снижению распространенности S. aureus вплоть до его исчезновения, а также к нормализации количества условно-патогенной микробиоты. Заключение. Исследования показали эффективность использования отдельных высокоактивных штаммов лактобацилл или их комбинации при лечении заболеваний СОПР, в частности бактериального стоматита. Aim. To characterize the microbiological picture of stomatitis in intact rats before and after the treatment with highly active cultured lactobacilli and to evaluate the effectiveness of this treatment in experiments on white rats. Methods. Experiments were carried out on 32 mongrel female white rats weighing 230 g. Smears were taken from the gum surface of control and experimental rats at all stages of the experiment to characterize the oral cavity microbiocenosis. Results. After treatment of the oral cavity with cultured Staphylococcus aureus , the occurrence and quantity of opportunistic microorganisms significantly decreased. The treatment of stomatitis in rats with cultured lactobacillus ( Lactobacillus 11 zv., Lactobacillus 2 p.r., Lactobacillus 24 d.st. and their combination) led to a decrease in S. aureus prevalence up to its extinction and to normalization of the quantitative composition of opportunistic microbiota. Conclusions. The study showed the effieacy of highly active lactobacillus strains individually or their combinations in the treatment of oral diseases, in particular, bacterial stomatitis.

(1) Background—Mapping language using direct cortical stimulation (DCS) during an awake craniotomy is difficult without using more than one language paradigm that particularly follows the demand of DCS by not exceeding the assessment time of 4 s to prevent intraoperative complications. We designed an intraoperative language paradigm by combining classical picture naming and verb generation, which safely engaged highly relevant language functions. (2) Methods—An evaluation study investigated whether a single trial of the language task could be performed in less than 4 s in 30 healthy subjects and whether the suggested language paradigm sufficiently pictured the cortical language network using functional magnetic resonance imaging (fMRI) in 12 healthy subjects. In a feasibility study, 24 brain tumor patients conducted the language task during an awake craniotomy. The patients’ neuropsychological outcomes were monitored before and after surgery. (3) Results—The fMRI results in healthy subjects showed activations in a language-associated network around the (left) sylvian fissure. Single language trials could be performed within 4 s. Intraoperatively, all tumor patients showed DCS-induced language errors while conducting the novel language task. Postoperatively, mild neuropsychological impairments appeared compared to the presurgical assessment. (4) Conclusions—These data support the use of a novel language paradigm that safely monitors highly relevant language functions intraoperatively, which can consequently minimize negative postoperative neuropsychological outcomes.
