Abstract Background: Overaggressive liposuction of the infragluteal region can lead to iatrogenic infragluteal fold deformity and result in aesthetic defect of gluteal contour.Case presentation: We report a case of using autologous fat transplantation to correct severe post-liposuction infragluteal fold deformity. In the process of reconstruction, the patient experienced fat graft over absorbed, fat graft translocation and gluteal ptosis aggravation. Despite multiple operations, the effect of fat transplantation is limit.Conclusions: Severe post-liposuction infragluteal deformity is very difficult to correct. We suggest that the infragluteal region should be reserved as possible during liposuction to avoid deformity.

Adipose-derived stem cells (ASCs) transplanted along with autologous adipose tissue may improve fat graft survival; however, the efficacy of ASCs has been diluted by low vascularization. This study was designed to test the hypothesis that basic fibroblast growth factor (bFGF) may improve the effects of ASCs because it owns the property to boost angiogenesis. In the present study, human fat tissues were mixed with ASCs, ASCs plus 100 U bFGF, or medium as the control and then injected subcutaneously into immunologically compromised nude mice for 12 weeks. Our findings demonstrated that mixture with the ASCs significantly increased the weight and volume of the fat grafts compared to control grafts, and histological analysis revealed that both ASCs and ASCs plus bFGF grafts consisted predominantly of adipose tissue and had significantly less fibrosis but greater microvascular density compared with control and also grafts mixed with ASCs had a high expression of angiogenic factors. More importantly, the bFGF treated fat grafts shown elevate in survival, vascularization, and angiogenic factors expression when compared with the grafts that received ASCs alone. These results indicated that bFGF together with ASCs can enhance the efficacy of autologous fat transplantation and increase blood vessel generation involved in the benefits from bFGF.

Introduction: Tissue expanders have become a popular modality in plastic and cosmetic surgery. Many victims of burns and severe trauma, premalignant lesions, congenital malformations, unwanted or regretted tattoos over large areas, women undergoing postmastectomy breast reconstruction, or men with extensive male-pattern alopecia may seek treatments that use tissue-expansion techniques. Tissue expansion has been an excellent tool for expanding normal skin to provide good quality, cosmetically appealing, full-thickness skin coverage of defective areas. However, like most treatment modalities, tissue expanders have some undesired pitfalls, sequelae, and complications associated with their use. We used autologous fat transplantation to remedy asymmetrical arm circumference after skin expansion on one arm and report our results in this case report. Case Report and Procedure: The patient is a 16-year-old female who presented with a congenital hairy nevus measuring 6.5 × 5.5 cm in the middle part of the posterolateral aspect of her left arm. Treatment included the use of tissue expanders, followed by excision of the nevus to treat the area. After the treatment, the patient complained that her left arm had become thinner than the right. Circumference of both arms measured at symmetrical points revealed a variance of 3 cm. Autologous fat transplant surgery, with a slight overcorrection that was expected to reduce over time, was offered as a solution. Conclusions: Autologous fat transplantation can be used as an adjunct to tissue expansion to fill the lipo-atrophic defects that may result from tissue expansion. Results: Postoperative follow up revealed sound healing of the wound. Her left arm circumference decreased over the following 6 weeks postoperatively (the overcorrection). The final result was symmetrical arms, both visually and numerically.

Folliculitis decalvans is a rare clinical disorder classified as primary neutrophilic scarring alopecia with a slight preference for the male gender. Here, we report the use of autologous fat transplantation as a source of stem cell therapy for hair re-growth assisted by inflammatory action of the fat itself in a female patient. The patient underwent adipose transplantation in April and September 2017. After treatments, the patient had no new pustules and no longer had pain or burning sensation in the affected area. The hair has re-grown at the periphery area of alopecia appearing stronger and shinier.
