Systemic lupus erythematosus (SLE) is an autoimmune connective tissue disease that is characterized by a multisystemic involvement and various clinical manifestations. Pulmonary manifestations occur in approximately 50% of patients. One of the rare and mortal complications is lupus pneumonitis. Very few cases have been reported due to its rarity and diagnostic challenge. It must be suspected in case of non-response to conventional treatment for pneumonia. We describe the case of a 29-year-old man with lupus pneumonitis as the initial presentation of systemic lupus erythematosus.

Celiac disease (CD) is a chronic immune-mediated intestinal disease that is characterized by production of autoantibodies directed against the small intestine. The main clinical manifestations of CD are typically defined as those related to indigestion and malabsorption. These manifestations include unexplained diarrhea or constipation, abdominal pain, bloating, weight loss, anemia, failure-to-thrive in children, and decreased bone density. Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by heterogeneous clinical manifestations, which may also involve the gastrointestinal tract. Comorbidity of CD and SLE is rare, and the overlapping symptoms and nonspecific clinical presentation may pose a diagnostic challenge to clinicians. We report here a case of SLE with CD, which mainly manifested as recurrent diarrhea, uncorrectable electrolyte disorders, and severe malnutrition. Through review, we hope to further improve our understanding and diagnostic level of this combination of diseases.

Systemic lupus erythematosus is a multisystem autoimmune disease with wide-ranging pleuropulmonary manifestations. Acute lupus pneumonitis is one of its uncommon complications. We report a 36-year-old woman with acute lupus pneumonitis as the initial presentation of systemic lupus erythematosus. Clinical, biological, radiological and functional improvements were noticed with the administration of steroids, hydroxychloroquine and immunoglobulin.

This paper reviews the pulmonary manifestations of systemic lupus erythematosus with particular focus on comparing global and Saudi Arabian populations. Dozens of articles addressing lupus complications in Saudi patients were published in recent years; to our knowledge, this is the first review focusing on pulmonary complications. In this narrative literature review, we conducted a systematic search of MEDLINE and Google Scholar using the words pulmonary complications of systemic lupus in Saudi Arabia. Th e search covered the period between 2000 and 2019. We found that the rate of pulmonary complications in Saudi lupus patients was consistent with that in global patients; exceptionally higher incidence of acute lupus pneumonitis was reported in Saudi patients. We hope that this study will inform clinicians about the important points regarding pulmonary complications in lupus patients, particularly in the Saudi population.

A case of lung damage in systemic lupus erythematosus (SLE) in a 33-year-old woman is described. This case is of clinical interest due to the complexity of diagnosis due to the fact that SLE is a disease with diverse clinical manifestations involving many organs and systems, which often makes it difficult to timely recognize the onset of the disease. SLE still remains a challenge and requires special attention to the patient s history, clinical and laboratory parameters of the patient, as well as specific immunological examinations.
