[first paragraph of article]This year marks the 51st anniversary of the first human heart transplant operation by Christiaan Barnard at the Groote Schuur Hospital in Cape Town. This event has had a profound effect on Science, Medicine and Humanity, almost beyond expectations. The anniversary calls for celebrations, and evaluation, as with maturity comes accountability. We here attempt to summarise the reasons for celebrations, and highlight the remaining many challenges and expectations. Regarding the celebrations, this anniversary exemplifies and calls for celebration of at least four human attributes; courage, innovation, compassion, and human spirit.

Heart transplant had already developed by a group of US surgeons in the early 1950s, and it was the American pioneer Norman Shumway who validated the technical feasibility in a dog model at Stanford University in 1958. This milestone in medicine was the beginning of a huge race for numerous physicians and researchers to make this operation a clinical reality. Barnard was intrigued by the idea to perform heart transplantation at Groote Schuur Hospital and therefore made it a major focus in his department in the early 1960s. While Shumway and co-workers were further refining the surgical technique in these year, based on his extensive cardio-surgical experience, Barnard was already convinced about the technical feasibility and wanted to enter this new field of cardiac surgery. To accomplish his goal, he recognized that he had to learn more about immunosuppressive therapy and therefore he spent a few months in Richmond, VA, USA, to obtain this important knowledge for postoperative care. In 1966, when Shumway and colleagues announced that they would be ready for a first human patient, Barnard moved ahead of them on 3 December 1967, and performed the world’s first human-to-human heart transplantation.

The first human heart transplant immediately exposed many unsolved problems of organ transplant existing at an early stage of the development of that kind of medical technology. The review of historical, legal and philosophical aspects of the first human heart transplantation in Russia and abroad is presented. Special attention is paid to the role and destiny of the pioneers of heart transplantation. The dramatic lives of Vladimir Demikhov and famous South African surgeon Christiaan Barnard are compared. The hard way of the heart transplantation founders is not lost, worthy successors continued their work, saving the lives of hundreds and thousands of patients.

<p>First human-to-human heart transplantation performed on the 3th of December 1967 in Cape Town has become one of the most striking events in the history of medicine in 20th century. It also made an unknown surgeon Christiaan Barnard a world celebrity. Nowadays heart transplantation has become a routine procedure in many countries. Nevertheless, the happenings which took place 50 years ago are still exciting with the determination, courage and charisma of those who were related to this history. This paper describes the circumsances of the first heart transplantation and provides the biography of Ch. Barnard whose name is inextricably linked to this event.</p><p>Received 4 September 2017.
