Kasus celah bibir dan palatum, adalah kasus yang memiliki tingkat kesulitan lebih tinggi dalam hal perbaikan bila dibandingkan dengan kasus kecacatan lain yang disebabkan oleh cacat bawaan maupun dapatan lain. Karena kasus celah bibir dan palatum meminta perhatian khusus pada tahap-tahap perbaikannya, serta memerlukan kolaborasi antar disiplin ilmu kedokteran untuk mendapatkan hasil yang optimal. Prostodonsi adalah salah satu bidang kedokteran gigi yang memegang peranan menentukan hasil akhir dari rangkaian perawatan yang telah dilakukan. Gigi tiruan overlay adalah gigi tiruan lepasan yang digunakan untuk mengurangi penyempitan rahang atas dan perbedaan dalam dimensi vertikal. Tujuan dari artikel ini adalah membahas perawatan yang lebih nyaman dengan gigi tiruan overlay thermoplastic resin untuk kasus celah bibir dan palatum. Seorang pasien perempuan, 25 tahun, datang dengan keluhan ingin perbaikan estetik pada regio anterior rahang atas, untuk menutupi defek sisa hasil operasi penyatuan celah palatum yang telah dilakukan ketika balita. Tatalaksana kasus: 1) Anamnesa, 2) Pemeriksaan klinis dan radiografis, 3) Pencetakan model studi, 4) Pencetakan model kerja, 5) Pemasangan pada artikulator, 6) Penyusunan gigi, 7) Pasang coba model malam, 8) Processing laboratorium, 9) Insersi, 10) Kontrol. Kesimpulan: Perawatan pasien celah palatum dengan gigi tiruan overlay thermoplastic resin memberikan kenyamanan, perbaikan estetis sehingga menimbulkan dampak psikologis yang baik pada pasien. Thermoplastic Resin Overlay Denture On Cleft Lip And Palate Patient (A Case Report). Background: Cleft lip and palate has a degree of difficulities in habilitativeness among other congenital or acquired handicaps. Because this case requires a special attention on the habilitation stage, also requires an interdiciplinary collaboration in medical professions in order to achieve optimal results. Prosthodontics is a part in dentistry that has a role in determining the final results of a series of treatments that have been performed. Overlay denture is a removable prosthesis which are used to alleviate maxillary constriction and discrepancies in vertical dimension. Purpose: To give more comfortable treatment with overlay denture on cleft lip and palate case. Case: A 25 years old female patient, with the chief complaint of the aesthetic on her maxillary anterior region. Requires a treatment to covered her defect, post cleft lip and palate operations that have been performed when she was a child. Case management: 1) Anamnesis, 2) Clinical and radiographic examination, 3) Study model impressions, 4) Working model impressions, 5) Articulator mounting, 6) Artificial teeth arrangement, 7) Wax model try-in, 8) Laboratorium processing, 9) Insertion, 10) Control. Conclusion: Treatment of a cleft lip and palate case with a thermoplastic resin overlay denture can delivered comfort, and aesthetic improvement which can gives the patient a positive physiological impact.

Objective: To determine the rate of revision alveolar bone grafting (ABG) in patients with cleft lip and palate (CLP) before and after the introduction of postoperative computed tomography (CT). Design: Retrospective case–control study analyzing the incidence of revision ABG in patients with and without postoperative CT scans for graft success evaluation. Setting: Academic tertiary care pediatric hospital. Patients: Eighty-seven patients with CLP or cleft lip and alveolus treated with autologous iliac crest bone grafting for alveolar clefts over a 10-year period (January 2009 to March 2019) with minimum 6-month follow-up. Fifty patients had postoperative CT evaluation; 37 did not. Interventions: Postoperative CT to determine ABG success, versus standard clinical examination and 2-dimensional radiographs. Main Outcome Measures: Requirement for revision ABG, defined as failure of the original graft by clinical or radiographic examination. Results: Fifty-eight percent of patients underwent a postoperative CT scan at median interval of 10 months after surgery. Patients with postoperative CT evaluation had a 44% rate of revision ABG (22/50) for inadequate graft take, compared to 5% (2/37) in patients without postoperative CT ( P < .001; 95% CT, 31%-58% in the CT group, 1%-16% in the non-CT group). Conclusions: Computed tomography evaluation after ABG is associated with a significantly increased revision rate for inadequate graft take. The presence of a secondary palatal fistula at the time of original ABG is not associated with revision requirement. Lack of standardized dental and orthodontic records complicates the study of ABG outcomes and presents an area for systems-based improvement.

Statement of problem. Cleft Lip and Palate (CLP) are common congenital defects of the present day. They result from a failure of facial bud sticking during embryological phenomena. Their therapeutics based on multi-disciplinary care to restore as much as possible the aesthetic and functional prejudices generated by these anomalies. Purpose. Through this work, we will illustrate, through a clinical case supported at the Consultation and Dental Treatment Center of Rabat, clinical and laboratory stages of realization of Nasoalveolar Molding (NAM). Material and methods. We describe original techniques used in our service for manufacturing of NAM fireworks and we define recommendations after insertion and follow-up procedures.Result. Place of NAM in management of these anomalies has increasingly recommended, given limitations of surgery alone to ensure satisfactory aesthetic results. Several fixed or removable appliances has used, in order to reposition gaps in cleft and / or to reshape affected bordering tissues. Conclusion. Despite the controversies surrounding this therapy, it remains highly recommended and scientifically based.

The proposed advantages of pre-surgical naso-alveolar moulding (PNAM) are easy primary lip repair which heals under minimum tension reducing the scar formation and improving the aesthetic results in addition to reshaping of alar cartilage and improvement of nasal symmetry.However, the anatomy and alveolar morphology varies for each cleft child; the procedure for PNAM differs accordingly. In an attempt to categorize unilateral cleft lip and palate cases as per anatomical variations, a new classification system has been proposed. This classification aims to give an insight in unilateral cleft morphology based on which modification in PNAM procedure could be done.

Objective To verify the effects of rapid maxillary expansion performed after secondary alveolar bone graft in one patient. Setting Department of Orthodontics of the Hospital for Rehabilitation of Craniofacial Anomalies, University of Sao Paulo. Patient(s) The patient had bilateral cleft lip and palate, had already had a bone graft, and showed a relapsing maxillary constriction in need of correction. Intervention A fixed dental-mucous-bone-supported expander corrected the maxillary constriction. Main Outcome Measure(s) Measurements were obtained from dental casts, including transverse dimensions (intercanine distance [IC], interfirst premolar distance [IP], and interfirst molar distance [IM]) and measurements of the grafted area (interfirst premolar/central incisors [IPI] and interfirst premolar/canine [IPC]) to observe the changes. Clinical and radiographic analyses were done through direct view. Results The occlusal radiograph of the maxilla after expansion showed opening of the intermaxillary suture in the premaxillary area, which was clinically confirmed by the diastema between the maxillary central incisors. No radiographic alteration was observed in the grafted area. The transverse measurements of the dental casts (IC, IP, and IM) showed a significant increase. The measurements of the teeth adjacent to the grafted area, IPI and IPC, increased. Conclusions The orthopedic effect of rapid maxillary expansion after bone graft was verified. Nevertheless, additional studies are necessary to define any side effects in patients submitted to bone graft.

ABSTRACT The rehabilitation treatment of patients with cleft lip and palate is lengthy and is carried out by various professionals from different areas of healthcare.To this end, planning and specialist knowledge are critical to the success of the treatment, in this way envisioning the aesthetic and functional rehabilitation of the individual in order to integrate them into society. This study consists of a literature review along with the report of a clinical case, demonstrating the treatment performed with the use of a removable partial denture using the abutment teeth treated with milled metal-ceramic crowns and the use of attachments, with a view to promoting the retention and stability of the prosthetic device. With the use of this system as a whole, aimed at improving the facial proportions due to the restoration of vertical dimension of occlusion and adequate lip support, features that are often not present in individuals with cleft lip and palate. In this work, we are seeking to underline the importance of scientific and technical knowledge, combined with a well-executed planning, in order to achieve successful treatment that restores function and aesthetics to the patient, thereby enabling their reintegration into society.

Objective: To determine whether a relationship exists between the aesthetic scores given to photographic records of the nasolabial region of patients with repaired unilateral cleft lip and palate (UCLP) and the 5-Year Olds’ Index scores of study models for the same participants. Design: Retrospective study. Setting: University of Bristol Dental Hospital, United Kingdom. Participants: Patients with nonsyndromic UCLP previously enrolled in the Cleft Care UK (CCUK) Study. Methods: The CCUK participants, who had both study models and photographs (frontal and worm’s eye view), were identified and their records retrieved. These were rated by 2 consultants and 2 senior registrars in orthodontics. The 5-Year Olds’ Index was used to score the study models, and at a separate sitting, a 5-point Likert scale was used to score the cropped frontal and worm’s eye view photographs of the same children. The results were analyzed using intraclass correlation coefficients and Cohen κ. Main Outcome Measures: Correlation between the aesthetic scores of the photographic views and the concordant 5-Year Olds’ Index scores of the study models. Results: The intraclass correlation coefficient scores showed very poor agreement between the photographic views and their concordant study models. The level of inter- and intra-rater reliability was strongest when scoring the study models. Conclusions: There was no agreement between the scores given to various photographic views and their corresponding study models. Scoring the study models using the 5-Year Olds’ Index was the most reliable outcome measure for this age-group.

ABSTRACTObjectives: To present 17 natal/neonatal teeth in 15 patients and describe their clinical characteristics, associated disorders, complications and treatment. Methods: A retrospective study of neonates who visited the Department of Pedodontics and Preventive Dentistry, College of Dental Sciences, Davangere, India, between 2003 and 2006 was carried out. It was a study of clinical data, such as the age and gender of the patients, the history and chief complaints of mothers, the clinical appearance and location of natal/neonatal teeth, and associated complications and treatments. Results: A total of 17 teeth (6 natal, 11 neonatal) were found in 15 patients. No significant gender predilection (8 male, 7 female) was found. Sixteen natal/neonatal teeth were placed in mandibular incisor area (10 on the right side and 6 on the left side) and one tooth in the maxillary incisor area. In 13 patients, the occurrence of natal/neonatal teeth was unilateral, and in 2 patients, it was bilateral. Three cases were associated with enamel hypoplasia, 3 cases with Riga-Fede disease, and 1 case with gingival hyperplasia. One case involved a patient with cleft lip and palate. Radiographic examination confirmed these teeth to be supernumerary, and all teeth exhibited hypermobility. Extraction had been done in all the cases. Eleven of the extracted teeth exhibited only rudimentary roots, and six teeth showed no roots. Conclusions: The occurrence of a natal/neonatal tooth is a rare phenomenon. When it occurs, the teeth have a variety of clinical characteristics and lead to different complications. Knowledge of the management of these structures is essential for the overall well being of a child. (Eur J Dent 2011;5:168-172)
