Clinical Relevance Veneered electroformed crowns and In-Ceram alumina crowns made with CEREC 3D produced shade-matching that was significantly better than that obtained with conventional porcelain-fused-to-metal crowns.

Background: Patients positive for three types of antiphospholipid antibodies (aPLs) (triple positivity) have been identified at a high risk for thrombotic events. However, the clinical significance of isolated lupus anticoagulant (LAC) positivity is debated. Objectives: To investigate the clinical relevance of isolated LAC. Patients/Methods 456 patients were enrolled in this study; 66 antiphospholipid syndrome patients and 390 control patients. The control group existed of autoimmune patients (n=91), patients with thrombosis but without aPLs (n=127) and normal controls (n=172). The criteria LAC, anti-cardiolipin (anti-CL) and anti-beta2glycoprotein I (anti-β2GPI) IgG and IgM and the non-criteria IgA anti-CL and anti-β2GPI, anti-domain I (anti-DI) of β2GPI IgG and anti-phosphatidylserine/prothrombin (anti-PS/PT) IgG and IgM were detected according to the ISTH guidelines for solid phase assays. Results: 70 patients were positive for LAC, of which 44 were negative for both anti-β2GPI and anti-CL. We found that isolated LAC proved to be strongly associated with vascular thrombosis (Odds ratio (OR) (95% CI) 7.3 (3.3-16.1)), even better than triple positive samples (OR 4.3 (1.6-12.2)). The titers of the anti-PS/PT IgG and IgM were significantly higher in triple positivity samples compared to samples with isolated LAC positivity. The majority of single LAC positives were anti-PS/PT negative. We observed that LAC positivity was weaker in isolated LAC positive patients compared to LAC activity in triple positive patients. Conclusions: Isolated LAC was highly associated with thrombosis. The presence of anti-PS/PT could not explain LAC positivity in isolated LAC. Isolated LAC showed a weaker LAC activity compared to triple positive patients.

Statement of Problem. In some clinical situations, the vertical length of either a prepared tooth or an implant abutment is short, while the occlusal clearance to be restored by a porcelain crown is large. Incisal thickness of the veneering porcelain should be considered to prevent mechanical failure of the crown. Purpose. The aim of this study is to evaluate the effect of two different incisal veneering porcelain thickness on the fracture resistance of the anterior all-ceramic CAD/CAM zirconia crown system as compared with the conventionally used metal ceramic crown system. Method. CAD/CAM zirconia all-ceramic and metal ceramic crowns were fabricated on the prepared dies with standardized dimensions and designs using standardized methods according to the manufacturer’s instructions. All crowns were then adhesively luted with resin-based cement (Multilink cement system), subjected to thermal cycling and cyclic loading, and were loaded until fracture using the universal testing machine to indicate the fracture resistance for each crown material in each veneering thickness. Results. Statistical analysis was carried out, and the results showed that the fracture resistance of the nickel-chromium metal ceramic group was significantly higher than that of the CAD/CAM zirconia all-ceramic group. Also, the fracture resistance of crowns with 1.5 mm incisal veneering thickness was significantly higher than those with 3 mm incisal veneering thickness in both groups. Furthermore, there was no significant difference in the fracture mode of the two groups where 50% of the total specimens demonstrated Mode II (veneer chipping), while 35% demonstrated Mode I (visible crack) and only 15% demonstrated Mode III (bulk fracture). Conclusion. High failure load values were demonstrated by the specimens in this study, which suggest sufficient strength of both incisal veneering thickness in both crown systems to withstand clinical applications; however, the fracture patterns still underline the requirement of a core design that support a consistent thickness of the veneering ceramic, and it is recommended to conduct long-term prospective clinical studies to confirm findings reported in the present study.

Clinical Relevance Resin-dentin bonds, which may have an influence on the long-term success of restorations, are prone to deterioration after cyclic loading. The tested one-step self-etching system (Etch&Prime 3.0) provided the least reliable dentin adhesion. After acid etching of dentin, alcohol/based adhesives performed better than those containing acetone as solvent.

This report describes a clinical case with an atypical intraoral sinus tract formation from diagnosis and treatment to short-term outcome and definitive prosthetic rehabilitation. In detail, the patient underwent conservative nonsurgical root canal treatment followed by guided bone augmentation of the regions involved in periapical inflammation and sinus tract formation. The removal of the inflammatory source of the lesion as well as the affected tissue clearly led to a healing of the surrounding bone tissues. Subsequently, the tooth was reconstructed using a fibreglass post and a metal-ceramic crown; an implant was successfully placed in the previously inflamed bone region.
