Clinical assessments often use complex picture description tasks to elicit natural speech patterns and magnify changes occurring in brain regions implicated in Alzheimer's disease and dementia. As The Cookie Theft picture description task is used in the largest Alzheimer's disease and dementia cohort studies available, we aimed to create algorithms that could characterize the visual narrative path a participant takes in describing what is happening in this image. We proposed spatio-semantic graphs, models based on graph theory that transform the participants' narratives into graphs that retain semantic order and encode the visuospatial information between content units in the image. The resulting graphs differ between Cognitively Impaired and Unimpaired participants in several important ways. Cognitively Impaired participants consistently scored higher on features that are heavily associated with symptoms of cognitive decline, including repetition, evidence of short-term memory lapses, and generally disorganized narrative descriptions, while Cognitively Unimpaired participants produced more efficient narrative paths. These results provide evidence that spatio-semantic graph analysis of these tasks can generate important insights into a participant's cognitive performance that cannot be generated from semantic analysis alone.

Η παρούσα διδακτορική διατριβή με τον τίτλο "Απόδοση αναφοράς σε αντωνυμίες σε Ελληνόφωνους ασθενείς με τη Νόσο Αλτσχάιμερ" μελετά την απόδοση αναφοράς σε Ελληνόφωνους ασθενείς με Νόσο Αλτσχάιμερ (ΝΑ). Πιο συγκεκριμένα, η διδακτορική διατριβή ερευνά την παραγωγή αντωνυμιών με τη χρήση αυθόρμητου και ημι-αυθόρμητου λόγου με σκοπό να ερευνήσει κατά πόσο οι Ελληνόφωνοι ασθενείς με ΝΑ υπερχρησιμοποιούν τις αντωνυμίες αφήνοντάς τες με αδέσμευτα, ασαφή και ακυβέρνητα αντικείμενα αναφοράς. Αυτή η διατριβή ερευνά δύο όψεις της κατανόησης αντωνυμιών: (α) κατά πόσο υπάρχει μια παραβίαση των αρχών της αναφορικής δέσμευσης στις προσωπικές αντωνυμίες αντικειμένου ισχυρού τύπου, στα κλιτικά και στις αυτοπαθείς αντωνυμίες και (β) κατά πόσο οι Ελληνόφωνοι ασθενείς με ΝΑ κατανοούν τη συμφωνία αριθμού (και δευτερευόντως) και του γένους στις προσωπικές αντωνυμίες αντικειμένου ισχυρού τύπου και στα κλιτικά και αν η απόσταση μεταξύ των αντωνυμιών και των αντικειμένων αναφοράς τους συμβάλει σε αυτήν την κατανόηση. Τέλος, αυτή η διδακτορική διατριβή εξετάζει αν υπάρχουν συσχετίσεις μεταξύ των εκτελεστικών λειτουργιών (executive functions) της μνήμης εργασίας και του ανασταλτικού ελέγχου και άλλων γλωσσικών επιδόσεων (όπως η σημασιολογική ευχέρεια και η κατονομασία) με την παραγωγή και την κατανόηση αντωνυμιών στη ΝΑ. Τα αποτελέσματα των ερευνών στην παραγωγή και κατανόηση αντωνυμιών έχουν δείξει ότι υπάρχει πρόβλημα και στις δύο τροπικότητες. Πιο συγκεκριμένα, οι Ελληνόφωνοι ασθενείς με ΝΑ υπερχρησιμοποιούσαν τις αντωνυμίες -σε σχέση με το σύνολο των λέξεων- στην μοντέρνα εκδοχή της περιγραφής της Κλοπής του Μπισκότου (modern version of the Cookie-Theft picture description task) και παρουσίασαν έναν υψηλότερο λόγο αντωνυμιών-προς το σύνολο των ουσιαστικών που συσχετίστηκε με ένα πρόβλημα κατονομασίας. Σχετικά με την κατανόηση αντωνυμιών, οι Ελληνόφωνοι ασθενείς με τη ΝΑ αντιμετώπισαν δυσκολίες με τις αρχές της αναφορικής δέσμευσης στις αυτοπαθείς αντωνυμίες, και στα κλιτικά που συσχετίστηκαν με ένα πρόβλημα στη μνήμη εργασίας και ένα πρόβλημα κατονομασίας, αντιστοίχως. Επιπλέον, η συμφωνία αριθμού στις αντωνυμίες βρέθηκε προβληματική ανεξαρτήτως από το είδος της αντωνυμίας (προσωπικές αντωνυμίες ισχυρού τύπου και κλιτικά) με την χαμηλότερη επίδοση στον ενικό αριθμό και στη long συνθήκη, που συσχετίστηκε κυρίως με ένα πρόβλημα στη μνήμη εργασίας και δευτερευόντως, με προβλήματα κατονομασίας και σημασιολογικής ευχέρειας. Ως εκ τούτου, με βάση τα ευρήματα στην παραγωγή αλλά και στην κατανόηση αντωνυμιών, μπορούμε να καταλήξουμε στο ότι το υποκείμενο πρόβλημα στην Νόσο Αλτσχάιμερ δεν είναι ένα και δεν σχετίζεται με μία γνωστική λειτουργία ή μια γλωσσική παράμετρο, αλλά με ένα σύνολο γνωστικών και γλωσσικών λειτουργιών που διεπιδρούν η μία με την άλλη.

ABSTRACTBackground: Alzheimer's disease (AD) is characterized by memory loss and cognitive impairment. Phonological, syntactic, semantic and discursive aspects of language may also be affected. Analysis of micro- and macrolinguistic abilities of discourse may assist in diagnosing AD. The aim of this study was to identify changes in the discourse (lexical errors and syntactic index) of AD patients.Methods: 121 elderly subjects narrated a story based on a seven-figure picture description.Results: Patients with AD presented more word-finding difficulties, revisions and repetitions, and the syntactic index was lower than controls.Conclusion: Performance in microlinguistics at the lexical and syntactic levels was lower than expected in participants with AD.
