Currently, a unified approach to the treatment of peptic ulcer of the upper gastrointestinal tract has been developed, based on knowledge of the pathogenesis of the disease. However, standard methods of conservative and surgical treatment do not bring positive results to a number of patients. The reason may lie in the impossibility of establishing the true etiological factor of this pathology. It must be remembered that patients with of the gastroduodenal zone are not a homogeneous group and require a differentiated approach to diagnosis and treatment. The article describes a clinical case of successful of complicated ulcerative lesions of the gastric and duodenal mucosa, resistant to various methods of conservative and surgical treatment.

Relevance of research: аdrenalectomy remains the only method of potentially radical treatment of adrenocortical carcinoma (AСС), and the complete resection is considered one of the leading factors in the prognosis of disease. The question of the possibility and feasibility of using video- endoscopic technologies in the treatment of patients with ACC is still being discussed. Objective: the purpose of our study was to evaluate the effectiveness of laparoscopic adrenalectomy in the treatment of ACC patients. Material and methods: the study is based on the analysis of the results of diagnostics and surgical treatment of 12 patients who underwent laparoscopic adrenalectomy and verified ACC during histological and immunohistochemical studies. Results: stage I were verified (according to the ENSAT) in 4 cases, in 4 - II stage, and in 4 - III stage. The median of tumor size was 4.9 cm. Intraoperative and postoperative complications did not occur. In all cases, the R0-resection was confirmed by morphological examination. The median patient follow-up was 71 months (1; 141), during this period 11 patients were alive, 1 patient, who had surgery in stage III died of disease progression (distant metastases) 49 months after surgery. During the follow-up period tumor recurrence did not occur. Overall 5-year survival was 75% (95% CI; 30-95%). Conclusion: despite the small number of patients included in this study, satisfactory immediate results (absence of complications, R0-resection) and long-term results allow us to consider video- endoscopic surgery as an effective treatment for patients with ACC.

Relevance: Styloid process syndrome – a disease which is caused by irritation of the styloid process of the temporal bone surrounding the nerve, vascular and muscle structures. The syndrome is manifested by chronic painin deep division of the facial area referred to the root of the tongue, throat and ear, dysphagia, symptoms of circulatory disorders of the brain. Because of the low awareness of the disease practical specialists usually miss the diagnoses. Patients with different diagnoses are treated by different specialists; most often the prescribed symptomatic treatment is ineffective or does not give any result at all. The syndrome causes severe suffering to patients, reduces the quality of life. Purpose of the study: Assess the most significant clinical signs of the disease, mutual work between hypertrophied styloid process with the neurovascular neck formations, the effectiveness of conservative and surgical treatment. Materials and methods: 144 people were examined and treated by us. Of these, 103 women (71,5%), 41 men (28,5%), aged 25 to 70 years. The disease lasts from one year until 10-15 years. Tomography of the styloid processes with 3D reconstruction and contrasting of the main vessels of the neck was used. Results of the study and the discussion: we proposed a grading scale for the quality of the treatment, the five-point scale. The quality of the treatment was assessed by the number of reduction in scores and, accordingly, patient complaints. Of the total number of patients – 144 people, the success of the treatment by 0 points was assessed by 60 people (that is, absence of any symptomatology) (41,7%), 1 point – 44 (30,5%), 2-3 points – 27 (18,7%), 4-5 points – 13 (9%), that is significant effect of the treatment was not achieved. To all those people with the result of the treatment was estimated at 4-5 points, respectively, was proposed an operative method of the treatment – the resection of the styloid process from the corresponding side. Conclusions: Because of the lack of knowledge of such important topic it is necessary to improve diagnostic methods. Hypertrophy of the styloid process is not the main reason of the development of styloid syndrome; the degree of deviation of the process is important. Conservative treatment in most cases has a lasting effect, but has repeated symptoms after a certain period of time; surgical treatment should be directed to the maximal resection of styloid process for decreasing the axis pressure on the neurovascular bundle of the neck.

The tendency for bisphosphonate and non-bisphosphonate (eg.: antiresorptive or anti-angiogenesis drugs) induced osteonecrosis is increasing. Treatment of these patients is a challenge both for dentists and for oral and maxillofacial surgeons. Cooperation with the drug prescribing general medicine colleagues to prevent osteonecrosis is extremely important. Furthermore, prevention should include dental focus elimination, oral hygienic instructions and education, dental follow-up and, in case of manifest necrosis, referral to maxillofacial departments. Authors outline the difficulties of conservative and surgical treatment of a patient with sunitinib and zoledronic acid induced osteonecrosis. The patient became symptomless and the operated area healed entirely six and twelve months postoperatively. A long term success further follow-up is necessary to verify long-term success. Orv. The material of investigation was the results of treatment of 131 patients with foot bones tumours. The largest number of patients referred, to age interval from 11 to 30 years (69,6%). More than half of cases were osteochondromas (54%), then solitary bone cyst (14,5%) and chondromas (13%). Other nosologic forms were met significantly seldom. Two groups of patients were examined: the main group (with crio-influence) - 44 patients and group of comparison (without crio-influence) - 87 patients. The plot of operation was in flat, border-line, intrafocusal or segmental resection of damaged section, crio-instillation or contact curio-processing of bone and auto- or allopathic of respected defect. The results of treatment were estimated in a year after operation. After usage of curio-surgical method there were observed positive results in 41 patients, satisfactory - in 2 and unsatisfactory - in 1. The results of treatment with traditional method were positive in 79 cases, satisfactory - in 2, unsatisfactory - in 6. The worked-out method of curio-surgical treatment of foot bone tumours includes resection of pathological focus, itraoperative crio-influence on bone tissue and bone plastic transplantation of resected, defect. The analysis of criosurgical operations of foot gave the foundation to consider such interventions significant and perspective in treatment of patients with tumours and tumour similar damages of foot bone.

The authors present immediate and long-term results of treatment of 117 patients with superior thoracic outlet syndrome (STOS). There were different reasons for compression of neurovascular fascicle in outlet of the thorax. The costaclavicular syndrome was a reason in 48 patients, additional cervical ribs had 36 patients. Skalenus syndrome was noted in 26 cases, rudimentary cervical ribs or hypertrophy of cervical vertebrae C7 had 7 patients. Raynaud’s syndrome took place in 19 cases. The required volume of diagnostic procedures and surgical treatment of STOS were determined according to the cause of the syndrome. Differentiated approach to the different forms of STOS was used in relation to dominant symptoms of the disease and reasons for compression of neurovascular fascicle. This allowed getting positive results in majority of patients (90,4%) in long-term period.

AbstractPeptic ulcer is one of the most common gastrointestinal tract disorders worldwide, associated with challenges such as refractory morbidity, bleeding, interference with use of anticoagulants, and potential side effects associated with long-term use of proton pump inhibitors. A peptic ulcer is a defect in gastric or duodenal mucosa extending from muscularis mucosa to deeper layers of the stomach wall. In most cases, ulcers respond to standard treatments. However, in some people, peptic ulcer becomes resistant to conventional treatment or recurs after initially successful therapy. Therefore, new and safe treatments, including the use of stem cells, are highly favored for these patients. Adipose-derived mesenchymal stem cells are readily available in large quantities with minimal invasive intervention, and isolation of adipose-derived mesenchymal stromal stem cells (ASC) produces large amounts of stem cells, which are essential for cell-based and restorative therapies. These cells have high flexibility and can differentiate into several types of cells in vitro. This article will investigate the effects and possible mechanisms and signaling pathways of adipose tissue-derived mesenchymal stem cells in patients with refractory peptic ulcers.
