Transitional medication management, in which individual needs are balanced against organizational priorities, is crucial for safe discharge processes. The aim of this study was to explore hospital nurses’ transitional medication management in the discharge of older patients with multi-morbidity. Using an ethnographic approach the data were collected through participant observations at a mixed medical ward at a Danish university hospital for two weeks. The participants were five registered nurses, responsible for nursing care of 23 patients with multi-morbidity and planned for discharge. The data comprised field notes that were analysed using iterative processes of domain, taxonomic and component analysis. The reporting adhered to the COREQ checklist. Hospital nurses’ transitional medication management was characterized by unpredictability and inconsistency in patient situations, fragmentation and discontinuity in working processes and complexity in communication systems. Special attention to nurses’ needs assessment skills and clinical decision making in caring for patients with multi-morbidity in a single focused healthcare system is required.

ABSTRACT Objectives: to associate pleasure and suffering indicators with aspects related to nursing work in hospitals. Methods: This is a cross-sectional quantitative study conducted with 152 nurses from a university hospital in the city of João Pessoa, Paraíba State, Brazil. Data were collected through interviews, using a tool to obtain sociodemographic data, work characteristics and the Pleasure and Suffering Indicators at Work Scale. Results: pleasure living and its domains were rated as satisfactory, while suffering factors and their domains were critically assessed. Statistically significant associations were observed among pleasure and suffering indicators, and some characteristics of nurses' work. Conclusions: it was evidenced that nurses had critical levels of suffering at work associated with their work practice characteristics.

ABSTRACT Objective: to identify the knowledge and adherence of nurses to the standard precautions in critical care units. Method: descriptive and cross-sectional study, with a quantitative approach, with the population of nurses from a university hospital in the Distrito Federal, Brazil. For data collection, three instruments were used. A semi-structured questionnaire with identification and occupational data of nurses, the questionnaire to evaluate knowledge to standard precautions and the Questionnaire of adherence to standard precautions. Descriptive analysis was used with absolute and relative frequency values. Results: the population was composed of 40 participating nurses, 75% were female, with an average age of 32.5 years, ranging from 24 to 50 years. Professionals with five or more years of training (67.5%) predominated, 25 (62.5%) had only one job and had experience in the area of critical patient care for more than three years. Although with a high percentage of accuracy, with regard to the moments that required hand hygiene, this frequency was 97.5%. Gloves are always used for intramuscular or subcutaneous injection by 67.5% of professionals, but 17.5% still consider it unnecessary to wear disposable caps and surgical shoe covers. The use of sharps, handling and reporting of occupational accidents were not performed as expected. Conclusion: gaps were identified in relation to the knowledge and adherence of nurses in units that provide care to critical patients of a university hospital with regard to standard precautions.
