Background. The article describes a clinical case of a successful surgical treatment of a large size follicular cyst pushing the anterior-inferior wall of the left maxillary sinus and the lower nasal concha, with the maxillary alveolar process osteoplasty. Clinical case description. According to the results of cone beam computed tomography, the size of the follicular cyst before the treatment was 45.423.339.7 mm. After the endodontic treatment, cystectomy, cystostomy, plastic surgery of the alveolar process of the upper jaw, the bone defect is reduced by two times, disconnected from the oral cavity. Conclusion. By the example of the presented clinical case, it has been shown that large jaw cysts require a multi-stage treatment with the participation of narrow specialists of related profiles.

А clinical case of treatment of a patient with isolated saccular aneurysm of the aortic arch and aneurysm of the infrarenal aorta of large size is presented. As a сoncomitant pathology, gastric adenocarcinoma was revealed. Multi-stage surgical treatment was performed. The first stage included left-sided carotid-subclavian bypass, the second –TEVAR, the third — EVAR. The final stage was an extended gastrectomy with lymphodissection. No complications were found during the treatment. The technique of operations is described, explanations on the chosen tactics of treatment are given. The conclusion is made about the advantage of endovascular techniques in the treatment of aortic pathology in patients with oncological pathology.

Locked bucket-handle tears of both medial and lateral menisci, called in literature "Jack and Jill lesion", with simultaneous anterior cruciate ligament (ACL) injury is an extremely unusual phenomenon. It was reported only in a few cases in literature, which were unfortunately highly differentiated in the field of trauma mechanism, treatment options and postoperative care. Authors presented a clinical case of patient with a locked knee joint, by the torn both menisci, what masked the simultaneous ACL rupture and supremely imitated a stable joint. The locked knee is a clinical case demanding the urgent intervention to prevent further damages to the joint structures. Surprisingly, the patient after locking of the menisci and migration of their inner fragments to the intercondylar notch, started to feel the affected joint stabile and dismissed from the further treatment for next 2 years. Complex trauma needs a complex treatment, and a single-stage or multi-stage surgical approach is performed. Authors chose a two-steps surgical procedure with primary meniscal repair and secondary, early ACL reconstruction.

A clinical case of successful treatment of a patient with atherosclerotic peripheral artery disease and clinically significant lesion of the coronary and carotid arteries. Patients underwent multi-stage surgical treatment. The first stage performed endovascular treatment the left internal carotid artery, the second — myocardial revascularization, the third — endovascular treatment of the left iliac arteries, the fourth — rotational atherectomy with stenting of the right superficial femoral artery. There were no complications in the perioperative period. The technique of operations is described, explanations are given for the chosen treatment strategy. The conclusion is drawn about the advantage of endovascular techniques in the treatment of patients with multifocal atherosclerosis.

Summary: We review the rationale for behavioral clinical case formulations and emphasize the role of the functional analysis in the design of individualized treatments. Standardized treatments may not be optimally effective for clients who have multiple behavior problems. These problems can affect each other in complex ways and each behavior problem can be influenced by multiple, interacting causal variables. The mechanisms of action of standardized treatments may not always address the most important causal variables for a client's behavior problems. The functional analysis integrates judgments about the client's behavior problems, important causal variables, and functional relations among variables. The functional analysis aids treatment decisions by helping the clinician estimate the relative magnitude of effect of each causal variable on the client's behavior problems, so that the most effective treatments can be selected. The parameters of, and issues associated with, a functional analysis and Functional Analytic Clinical Case Models (FACCM) are illustrated with a clinical case. The task of selecting the best treatment for a client is complicated because treatments differ in their level of specificity and have unequally weighted mechanisms of action. Further, a treatment's mechanism of action is often unknown.
