High-frequency rTMS applied over bilateral leg motor areas combined with mobility training for gait disturbance after stroke: A preliminary study

The results of a preliminary study on the emulation of radius-to-fix legs in turning departures by means of standard track-to-fix legs are presented in this technical note. While radius-to-fix equipage rates keep gradually increasing over time, a traffic mix containing aircraft with and without this navigation capability can impede an optimal management of the traffic flows whenever precise and repeatable curved flight paths are required. After test-flying various sequences of track-to-fix segments to emulate a reference constant-radius arc in a typical departure environment, the deviations to the desired flight path and the flyability aspects were assessed. The first results for a given scenario and aircraft type show an adequate path conformance for a navigation specification RNAV/RNP 1 within a certain range of aircraft performances and weather conditions. However, the reduced predictability of the roll steering when transitioning between track-to-fix segments and the high frequency in the oscillations of the bank angle may pose a problem to flight crews and passengers, respectively.

Objective: It is difficult to stimulate leg motor areas with magnetic current using a figure-of-eight coil due to the deep anatomical location of the areas. However, a double cone coil is useful for stimulating deep brain regions. We postulated that the use of the same coil may allow repetitive transcranial magnetic stimulation (rTMS) to modulate the neural activity of the same areas. The purpose of this study is to investigate the effect of high-frequency rTMS applied over bilateral leg motor areas with a double cone coil on walking function after stroke. Materials and methods: Eighteen post-stroke hemiparetic patients with gait disturbances attended two experimental sessions with more than 24 hours apart, in a cross-over, double-blind paradigm. In one session, high-frequency rTMS of 10 Hz was applied over the leg motor area bilaterally in a 10-sec train using a double cone coil for 20 minutes (total 2,000 pulses). In the other session, sham stimulation was applied for 20 minutes at the same site. To assess walking function, walking velocity and Physiological Cost Index (PCI) were evaluated serially before, immediately after, and 10 and 20 minutes after each intervention. Results: The walking velocity was significantly higher for 20 minutes after stimulation in the high-frequency rTMS group than the sham group. PCI was lower in the high-frequency rTMS group than the sham group, but this was significant only immediately after stimulation. Conclusions: High-frequency rTMS of bilateral motor areas using a double cone coil can potentially improve walking function in post-stroke hemiparetic patients.

Asthma is a significant contributor to missed school days, especially for children living in urban settings. This preliminary study examined the impact of caregiver asthma on school absenteeism in a sample of 102 urban children with asthma from African American, Latino, and non-Latino White backgrounds. Caregivers and children participated in a single research session; 32 caregivers had asthma. Children of caregivers with asthma missed more days of school than children whose caregivers did not have asthma (3.97 vs. 2.43 days, p < .05, Cohen’s d = .40). Other indicators of child asthma morbidity (e.g., hospitalizations) did not differ across caregiver asthma status. Findings highlight that caregiver asthma may be an added risk factor for school absenteeism among children with asthma. For children with a high frequency of asthma-related school absenteeism, school nurses may find it useful to provide educational resources and referrals for caregivers with asthma.

Objective: To describe the use of a novel electric vein stripper (EVS) for use in patients with varicosities of the greater saphenous vein and tributaries. Methods: In addition to standard procedures as performed in varicose veins of the lower limb, an EVS, powered by a standard high frequency electrocoagulation generator (HF), is introduced. The application of this new EVS makes it possible to avoid tearing of the structures of the subcutaneous tissue surrounding the greater saphenous vein, leaving the channel of the vein blood dry. Results: A preliminary study including two groups of patients - 20 in the Babcock group and 24 in the EVS group - resulted in significantly reduced pain and discomfort. An added benefit for the patients was the fact that postoperative compression bandages were not needed. Conclusions: Application of EVS can prevent bruising and haematomas in vein stripping can be avoided.
