Double fatal outcome after ruptured vasa previa in monochorionic twins: case report and review of the literature

Abstract Subarachnoid hemorrhage represents a rare event in pregnancy with a high mortality rate. We present the case of a 39-year-old pregnant woman who developed right vertebral artery dissection with subsequent massive subarachnoid hemorrhage with fatal outcome. The macroscopic and microscopic autopsy findings are described. A review of the literature with a discussion of the varied predisposing factors for vertebral artery dissection and subarachnoid hemorrhage and the rarity of these events in pregnancy is provided.

Fatal lung fibrosis caused by paclitaxel toxicity has not been reported In this report, we describe the case of a 62-year-old woman who received six cycles of paclitaxel and carboplatin as combination chemotherapy for advanced ovarian cancer. Four weeks after the end of the chemotherapy she developed interstitial pneumonitis and irreversible lung fibrosis. Despite treatment with corticosteroids, she had rapid deterioration and died of respiratory failure. Pulmonary fibrosis is a complication of paclitaxel therapy that may occur despite treatments with corticosteroids. While reviewing the literature, we found few less severe pulmonary injuries after intravenous use of paclitaxel, but none of these cases had a fatal outcome. Physicians should keep in mind that taxanes such as paclitaxel have the potential to cause pneumonitis and lung fibrosis.

AbstractObjective:We report a case of diphtheria in an elderly patient, with a fatal outcome.Method:Case report and a review of the literature concerning the current recommended guidelines on detection of, and response to, diphtheria.Results:Although still rare, cases of diphtheria are being reported with increasing frequency in the UK, possibly as a result of immigration and travel. A high index of suspicion, rapid detection of carriers, and instigation of treatment and vaccination are the mainstays of management.Conclusion:Diphtheria can mimic other forms of acute upper airway obstruction and can lead to a fatal outcome. Otorhinolaryngological surgeons need to consider this re-emergent disease as a potential diagnosis in patients with an unusual presentation of airway obstruction.
